Small-cell gallbladder neuroendocrine carcinoma (GB-SCNEC) is a relatively rare histological type of gallbladder cancer, and the genomic landscape of GB-SCNEC is rarely considered in treatment decisions. Whether endosonography can replace mediastinoscopy as the initial procedure for mediastinal staging of non-small cell lung cancer remains controversial. OBJECTIVE: A high sensitivity of sentinel lymph nodes (SLN) for pelvic lymph node (LN) staging has been repeatedly shown in patients with cervical cancer. The authors'surgical experience on 14 patients with ovarian cancer recurrences in paraortic lymph nodes --infrarenal and suprarenal-celiac and triad, were presented. Complete cytoreduction was achivied in all recurrent patients (no visible tumor). The median time to recurrence was 16,5 months (from 10 to 36 months). The median operating time was 90 min (range from 30 to 120 min). The median blood loss was 200 ml (from 100 to 300 ml.). No intra- or postoperative complications were noted. A case of acinic cell carcinoma of the right parotid gland metastasizing to the right iliac crest is presented. Generally, for this rare low-grade malignant salivary gland neoplasm, 20% of cases may have local recurrences whereas about 10% of cases have distant metastases. They may arise many years after the initial presentation of the original tumor. The most frequent locations are the cervical lymph nodes, liver, lungs, contralateral orbit and bones. Occurrence in the appendicular skeleton is very rare and in our knowledge this is the first report of metastases to the pelvis.

Well-differentiated thyroid cancers can present with nodal metastasis that have undergone cystic degeneration. Rarely, mediastinal nodes may undergo cystic changes and pose a diagnostic dilemma, especially if the primary lesion is occult. We describe the case of a patient who presented with a large mediastinal cystic lesion which turned out to be metastasis from thyroid cancer.

Eccrine porocarcinoma (EPC), or malignant eccrine poroma, is an uncommon malignant tumor presumably arising from the intraepidermal ductal portion of the sweat glands. The variable clinical appearance of EPC lesions can make diagnosis challenging for physicians and could delay appropriate treatment. We report the incidental discovery of supernumerary breast changes on sequential ⁶⁷Ga scans in a woman with dermatofibrosarcoma protuberans excised 5 years before. The appearance of the accessory breast fluctuated with physiological ⁶⁷Ga uptake of the breast tissue was evident. Awareness of the concomitant changes in accessory breast with breast tissue could help narrow the differential diagnosis.
