Endoscopic endonasal reduction surgery for diplopia caused by orbital roof fractures.

BACKGROUND: Orbital roof fractures (ORF) frequently occur together with frontal bone fractures, and can cause diplopia as a sequela. However, ORF have never been treated using endoscopic endonasal reduction surgery. CASE: A 22-year-old woman was injured in a traffic accident and diagnosed with right ORF along with frontal bone fractures. She was referred to us for diplopia 2 weeks after the initial trauma. No facial deformation was seen. Endoscopic endonasal reduction surgery was performed under general anaesthesia. Ethmoidectomy was performed and, subsequently, the anterior superior portion …

Nontraumatic orbital conditions: diagnosis with CT and MR imaging in the emergent setting.

Imaging provides crucial information regarding emergent orbital abnormalities, and the radiologist fulfills an important role in guiding patient care and contributing to favorable outcomes. Knowledge of the imaging features of nontraumatic orbital conditions commonly seen in the emergent setting-infections, inflammation, vascular abnormalities, and retinal and choroidal detachments-is necessary to achieve a prompt and accurate diagnosis, thereby avoiding permanent vision loss and other potentially devastating consequences. The ability to distinguish these entities from physiologic calcificatio…

Acute orbital myositis heralding herpes zoster ophthalmicus: report of a case.

We report a rare case that developed orbital myositis before appearance of zoster rashes. A 54 year-old man came to our hospital with a 4-day history of left orbital shooting pain extending to left temporal area. Neurological examinations demonstrated mild left proptosis and hyperemic conjunctiva without ophthalmoplegia. Brain magnetic resonance imaging (MRI) revealed left orbital myositis and periorbital skin eruptions appeared two days after this MRI study. The symptoms were improved after antiviral therapy and a follow-up MRI showed resolution of orbital myositis. Herpes zoster ophthalmicus…

Spontaneous subperiosteal orbital hemorrhage following thrombolytic therapy for myocardial infarction.

A 60-year-old man experienced right orbital pain, periorbital swelling, and double vision 2 hours after treatment with streptokinase and heparin for myocardial infarction. Orbital CT revealed a right superior subperiosteal orbital hemorrhage. Conservative management in the absence of visual compromise was sufficient, as his symptoms and signs resolved completely in approximately 6 weeks with no recurrence during 6 months of follow-up. This case demonstrates that nontraumatic subperiosteal orbital hemorrhage may occur after thrombolytic therapy for myocardial infarction. Conservative treatment …

[Is orbital exenteration indicated for extensive primary mucosal melanomas of the sinunasal system?].

Primary mucosal melanomas are most frequently localized in the sinunasal system. Orbital involvement must be excluded in the mostly advanced tumor stage. The aim of our study was to find possible indications for orbital exenteration in the T3-4 sinunasal tumor stage. We evaluated 14 treatment courses at our department from 1988 to 2004. The analysis disclosed orbital involvement in 43 % of the patients. The one patient submitted to orbital exenteration had a survival of only 28 months, which was not relevantly longer than in the other 9 patients with a T3-4 tumor stage (median survival of 27 m…

Rounding of the inferior rectus muscle as a predictor of enophthalmos in orbital floor fractures.

In spite of established indications for early operative repair of orbital floor fractures 7-10% of patients treated nonoperatively develop enophthalmos. Clearly further indications for repair are required to prevent these post-injury complications. Rounding of the inferior rectus muscle on coronal computerized tomography (CT) scan results from a loss of soft tissue and bony support and may therefore be predictive of late enophthalmos.A four-year institutional review was conducted to identify patients with orbital floor fractures that had been treated nonoperatively. Patients were recruited for…

Endoscopic repair of orbital blowout fractures: use or misuse of a new approach?

OBJECTIVE: To evaluate the successes and challenges of endoscopic orbital floor fracture repairs. METHODS: We analyzed 53 orbital floor repairs and recorded the indications for surgery, factors that complicated the endoscopic repair or necessitated conversion to an open approach, and outcomes for each. RESULTS: Forty-five procedures were completed endoscopically. Repairs of smaller injuries confined entirely to the medial floor were readily accomplished, particularly when entrapment was the primary indication for surgery. Endoscopic repair became very difficult and often not possible when a …

Bilateral orbital myeloid sarcoma as initial manifestation of acute myeloid leukemia.

BACKGROUND: Granulocytic sarcoma is a rare orbital complication of acute leukemia. It concerns primarily children under 10 years of age suffering from primitive acute myeloid leukemia. The diagnosis is made by clinical examination, computed tomography and confirmed by haematological investigations. The treatment approach is based on chemotherapy associated with intravenous steroid therapy. CASE REPORT: We report the case of a 6-year-old girl who presented with bilateral proptosis revealing acute myeloid leukemia. The patient was treated by a combination of chemotherapeutic drugs in two phases…

Orbital myositis involving the oblique muscles associated with herpes zoster ophthalmicus.

An 81-year-old woman with right orbital inflammation and acute retinal necrosis following Herpes zoster ophthalmicus was evaluated and treated. CT showed right massive superior and inferior oblique enlargement and moderate enlargement of the remaining extraocular muscles with tendon sparing. The myositis and acute retinal necrosis dramatically improved following prednisone and intravenous acyclovir therapy.

[Angle-closure glaucoma secondary to nonspecific orbital inflammatory: case report].

UNLABELLED: The nonspecific orbital inflammatory presents several clinical forms. When it evolves the posterior segment of the eye, usually by contiguity, it can lead to serious damage to vision functions. Posterior scleritis causes permanent damage to the vision and rarely progresses to acute glaucoma. CASE REPORT: E.N., a 24-year-old black man, complained of pain in the left eye (OS) for ten days, with low visual acuity, malaise, nauseas and vomiting. On ophthalmologic examination, he presented proptosis, restricted eye movements and edema on the upper left eyelid. Best-corrected visual acu…

Orbital cellulitis as a postoperative complication of sub-Tenon anaesthesia in cataract surgery.

Orbital cellulitis is a rarely reported sight-threatening complication of sub-Tenon anaesthesia. We report a case of orbital cellulitis in a patient who had received sub-Tenon anaesthesia for routine cataract surgery. We discuss the potential under-reporting of complications of sub-Tenon anaesthesia that had a delayed presentation, and the possible association between the use of hyaluronidase in the anaesthetic mixture and orbital cellulitis following sub-Tenon anaesthesia.

Detection of Helicobacter pylori and Chlamydia pneumoniae genes in primary orbital lymphoma.

PURPOSE: Primary orbital non-Hodgkin's lymphoma is a mucosa-associated lymphoid tissue (MALT)-type extranodal marginal zone lymphoma. Chronic antigen stimulation is implicated as a causative agent in the development of some mature B-cell proliferations; for example, there are associations involving Helicobacter pylori with gastric or conjunctival MALT lymphoma and Chlamydia psittaci with ocular adnexal lymphoma. We examined the molecular signatures of H pylori and Chlamydia in eight orbital lymphomas. METHODS: Polymerase chain reaction (PCR) was performed on DNA extracted from microdissected …
