Severe bilateral paralimbal sterile infiltrates after photorefractive keratectomy.

This study presents a case report of the clinical presentation and management of a 47-year-old male myope who underwent photorefractive keratectomy (PRK) and developed bilateral sterile corneal infiltrates at 1 day post-operatively. The patient was successfully treated with aggressive topical antibiotic and topical steroid therapy. The final corrected distance visual acuity (CDVA) was 20/25 with faint corneal scarring. Peripheral sterile corneal infiltrate can occur after PRK with excellent prognosis. Infectious causes should be suspected in all cases of corneal infiltrate. The most likely cau

Apophysomyces elegans: a novel cause of endogenous endophthalmitis in an immunocompetent individual.

A 20-year-old male presented with sudden onset redness, pain, and decreased vision in his left eye. Best-corrected visual acuity in the left eye was noted to be perception of light positive with inaccurate projection. On clinical examination, he was diagnosed to have left eye endogenous endophthalmitis. Smear and culture of vitreous biopsy were positive for Apophysomyces elegans, a fungus known to cause extensive soft tissue infections systemically characteristically in immunocompetent individual. The patient was treated aggressively with topical, systemic, and intravitreal antifungals but had

Early treatment with infliximab in bilateral occlusive vasculitis as a presenting manifestation of Behçet' disease.

CASE REPORT: A 24 year old woman who complained of maculo-papulo rash, genital and bucal aphthous ulcers, abdominal pain, minor dyspnea and visual loss in both eyes. Funduscopy revealed a bilateral occlusive vasculitis including central vessels. Treatment was initiated with a methylprednisolone bolus (1 g/24h) and infliximab 5mg/kg/day (0-2-6 weeks and every 8 weeks). The treatment prescribed induced a fast remission. Visual acuity improved. The patient did not suffer any other relapse after one year of follow-up. DISCUSSION: An initial treatment with Infliximab should be considered in Behçet

Traumatic anterior dislocation of the crystalline lens and its surgical management.

This paper reports a case of a 57-year old female who had sustained a blunt ocular trauma resulting in anterior dislocation of the crystalline lens and acute painful visual loss in the left eye. The patient was managed with anterior chamber intracapsular phacoemulsification through a small anterior capsulotomy, pars plana vitrectomy, and surgical iridotomy. Aphakia was corrected by a contact lens. Two months after the surgery, the best corrected visual acuity was 0.9 in the left eye. The vision and retina remained stable in her follow-up examination 1 year later. Anterior dislocation of the cr

Descemet membrane endothelial keratoplasty with a stromal rim in the treatment of posterior polymorphous corneal dystrophy.

A 20-year-old patient, diagnosed with posterior polymorphous corneal dystrophy, developed corneal edema for which he underwent Descemet membrane endothelial keratoplasty with a stromal rim (DMEK-S) in the right eye. No intra- or postoperative complications were noted. At the last follow-up 2 years and 9 months after the procedure, the best corrected visual acuity was 1.0 and endothelial cell density declined from 3533 cells/mm 2 to 1012 cells/mm 2 . Despite the endothelial cell loss, DMEK-S appears to be a good alternative to other surgical techniques for the treatment of corneal endotheliopat

Monolateral visual loss due to cryptococcal meningitis.

A case of retrobulbar monolateral optic neuritis due to disseminated Cryptococcosis in a 32-year-old man with a new diagnosis of AIDS is described in the era of combination antiretroviral therapy (cART). The patient presented a monolateral rapid visual loss on day 7 of a liposomal amphotericin B treatment. Neuroradiological imaging showed the presence of retrobulbar neuritis. After starting ART and intravenous metilprednisolone 1 g daily for 3 days, we assisted to a progressive improvement of visual acuity. At 3 months of follow-up, complete clinical resolution was obtained. In this case, in t

Enhanced effect of double-stacked intrastromal corneal ring segments in keratoconus.

We present a patient with poor visual acuity and contact lens intolerance due to advanced keratoconus who had femtosecond-assisted placement of two 0.35 mm intrastromal corneal ring segments (ICRS) in the right eye. Postoperatively, both rings migrated inferiorly and overlapped each other in a double-stacked formation. This resulted in a dramatic central shift of the cone and flattening of the inferior paracentral cornea with significant improvement in vision. To our knowledge, this is the first report of double-stacked ICRS in a human cornea and raises the possibility that significantly thick

[Bilateral central retinal vein occlusion associated with moderate acute rejection of a cardiac graft].

We report a case of bilateral central retinal vein occlusion following moderate acute rejection of a cardiac transplant. A 27-year-old man was admitted for sudden bilateral decreased vision due to bilateral central retinal vein occlusion. Visual acuity was 20/63 in the right eye and 20/25 in the left eye. This patient had undergone a heart transplant 6 months before and had presented with moderate acute rejection for a few days. The moderate acute rejection phenomenon includes inflammatory lymphocyte infiltrates, reflecting persistent immune response activation. Moderate acute rejection of a c

A home care wound care challenge: its rewards, inspiration, and positive outcomes.

Quite often, home health team members are presented with a challenge to provide management for a patient who has undergone a complex gastrointestinal surgery. An appropriate plan of care includes the incision, wound, and colostomy care, as well as the psychosocial support, time management, and cost issues. Many times this requires ingenuity and flexibility on the part of the home care team. Comorbidities often increase the acuity of the nursing care as well. In any case and no matter how difficult the circumstances, the goal for the patient's plan of care is to achieve an optimal outcome that

Traumatic expulsive iridodialysis after small-incision sutureless cataract surgery.

Phacoemulsification with intraocular lens (IOL) implantation was performed in the right eye of an 84-year-old woman using a small clear corneal incision. Ten weeks after surgery, the patient fell, which caused traumatic iridodialysis and hyphema with expulsion of the iris through the surgical corneal incision. Apart from a transient rise in intraocular pressure, no other complication was identified and the visual acuity returned to 20/20 within 2 months. The IOL remained in situ and undisturbed in the capsular bag, and the retina remained flat, suggesting that an IOL in the capsular bag may pr

An ocular cat-scratch disease patient positive for cytoplasmic anti-neutrophil cytoplasmic antibody.

BACKGROUND: We report a case of ocular cat-scratch disease with permanent vision reduction in a patient who was cytoplasmic anti-neutrophil cytoplasmic antibody (C-ANCA) positive. METHODS: Case report and review of the literature. RESULTS: While taking steroids and antibiotics, a 52-year-old man with uveitis associated with cat-scratch disease developed retinal vein occlusion and a macular exudate. His final visual acuity was poor because of residual macular degeneration and optic atrophy. Serum C-ANCA increased and decreased in parallel with ocular inflammatory activity. CONCLUSION: C-ANCA

Managing a dropped nucleus during the phacoemulsification learning curve.

Three patients had a pars plana vitrectomy to remove retained nuclei within 72 hours after phacoemulsification performed by a surgeon making the transition from extra-capsular cataract extraction to phacoemulsification. After vitrectomy, the nuclei were brought to the midvitreous cavity from the retinal surface with a posterior segment phacofragmenter, emulsified, and completely removed. Then, a posterior chamber intraocular lens was implanted in the sulcus through the previous cataract surgery incision and remained well centered postoperatively. Postoperative complications included cystoid ma

Cryotherapy in the treatment of cyclodialysis cleft induced hypotony.

A 26-year-old man with juvenile glaucoma developed persisting hypotonic retinopathy after trabeculotomy ab externo. An unintended cyclodialysis cleft at the site of the trabeculotomy was suspected, but visualization of the chamber angle was impossible due to a shallow anterior chamber. Seven months after the trabeculotomy the patient was treated with transconjunctival cryotherapy at the presumed localization of a cyclodialysis cleft. Eight days later an episode of acute ocular hypertension occurred, otherwise the postoperative course was uneventful with reversal of the hypotony and complete no

Nocardia brasiliensis keratitis successfully treated with therapeutic lamellar keratectomy.

A 36-year-old man sustained an indolent keratitis following a minor corneal trauma. Nocardia brasiliensis was isolated from cultures of the corneal scrapings. Fortified topical antibiotics, based on in vitro drug sensitivity testing, were intensively applied for weeks without clinical improvement. The patient underwent therapeutic lamellar keratectomy, which led to a rapid and complete resolution. The site of the keratectomy reepithelialized within 2 days and became a faint subepithelial haze thereafter. The visual acuity improved to 20/20 and there was no evidence of recurrence during the sub

Penetrating corneal fish-hook injury.

A healthy 13-year-old female was struck in the left eye by a fish-hook while fishing. The fish-hook penetrated the cornea, passed the anterior chamber and exited from a second corneal site near the limbus. The fish-hook was removed by cutting its shank and pushing it out of the wounds. Visual acuity was 20/20 OS after surgery. Mild corneal scarring was present at the penetration sites. We would like to report this case because there is a limited number of patients with penetrating corneal fish-hook injuries and this case presents the only female patient in the ophthalmological literature who s

Blind drunk: alcoholic pancreatitis and loss of vision.

Acute loss of vision and alcoholic intoxication suggests the possibility of methanol poisoning. In this report we describe an alcoholic patient who complained of blindness after recovering from alcoholic pancreatitis and delirium tremens. Visual acuity was severely impaired and fundoscopy revealed florid bilateral cotton wool spots and a cherry red spot at the right macula. Such appearances have previously been reported in cases of post-traumatic visual loss, and may share a common aetiology of fat embolization. The association between alcoholic pancreatitis and visual loss is rare, and fundal
