Wide en bloc excision of proximal ulna sections is used to treat traumatic and pathological fractures of the ulna, though poor standardization of clinical treatment often results in long-term failure of such reconstructed biomechanical structures. In order to provide insight into effective ulnar reconstructive treatments, the case of an 80-year-old Chinese Han male presenting with pathological fracture caused by a proximal ulnar metastatic tumor concurrent with metastatic renal cancer complicated by occurrence in the brain and lungs is reported and contrasted with alternative treatment techniq

BACKGROUND: Oronasal fistulas after oromaxillary surgery may sometimes be encountered and remain a challenging problem. They can cause significant disabilities such as phonetic problems and food or liquid regurgitation while swallowing. A few methods are reported to solve this problem, including using a dental appliance, local tissue rotation, or even free-tissue transfer.
MATERIALS AND METHODS: An angular artery cutaneous flap was designed to repair the defect. The flap that included the skin and superficial fascia fed by the flow of angular artery was rotated through the buccal mucosa into

PURPOSE: To describe a technique for modifying a Jones tube length in the operating room.
METHODS: The required length of the Jones tube is determined using an 18-gauge needle passed from the conjunctiva posterior to the caruncle to the middle meatus. A longer Jones tube is ground to the proper length with a 4-mm diamond burr on a saber drill.
RESULTS: This procedure results in shortening of an oversized Jones tube in the operating room with no gross or microscopic defects in the glass surface.
CONCLUSIONS: This technique allows modification of a Jones tube when the appropriate size is not

Large congenital melanocytic nevus rarely presents itself without hairs, with hardened skin and progressive depigmentation. We report a girl who presented with a large congenital melanocytic nevus in the left cheek. Over the years, the nevus became pruriginous, light brown, bumpy, and hard. Histology revealed nevus cells interspersed with dense fibrosclerotic collagen bundles. There are few reported cases of large congenital melanocytic nevus with this evolution, so-called desmoplastic hypopigmented hairless nevus.

It is critical that traumatic intracranial pseudoaneurysms should be removed completely from the parent artery because there is a possibility of significant morbidity and mortality from the high risk of rebleeding from any remnants of the pseudoaneurysm. However, the deconstruction of the parent artery has the risk of ischemic damage to the distal to the trapped artery. We describe a case of a successful reconstructive stent-buttressed coil embolization in a patient with a traumatic pseudoaneurysm of the intracranial internal carotid artery. A 30-year-old man with a stuporous mentality was adm

PURPOSE OF REVIEW: To review the latest advancements in implant materials for orbital fracture repair, including analysis of the last year in research.
RECENT FINDINGS: Advances in the manufacture of highly biocompatible alloplastic materials offer an alternative to traditional autologous implants for orbital fracture repair. The emergence of the composite implant offers the stability and strength of traditional titanium mesh, while the porous polyethylene shell allows for greater fibrovascular integration and a potential for decreased risk of postoperative complications. Surgery is the standard treatment for patients in the chronic phase of Peyronie's disease. Reconstructive surgeries function by either shortening the convex side of the tunica albuginea (Nesbit procedure, Yachia technique and penile plication) or lengthening the concave side by incision of the plaque with subsequent grafting. Tunical shortening procedures are ideal for men with good erectile capacity, penile curvatures less than 60° and predicted postprocedural length loss of less than 20% of erect penis length. Tunical lengthening procedures with grafting are indicated in patients with severe

Advances in autologous reconstructive concepts, principles, and techniques allow us to close wounds of almost any size. The concept of three-dimensional (3-D) functional units of the face is discussed. Five cases of facial deformity are presented to demonstrate the concept of reconstructing different "functional" units of the face with consideration for restoring the 3-D loss of tissue within the anatomic boundaries of the aesthetic units. This article demonstrates that most individual parts or functional units can be reconstructed with autologous techniques in an acceptable way. It is when la

BACKGROUND: This study focused on using the mandibular outer cortex for orbital and maxillary augmentation of oblique facial clefts.
METHODS: Eight oblique facial clefts were reconstructed with the mandibular outer cortex from April 2000 to April 2008.
RESULTS: No visible scars and few complications were observed at the donor site. Postoperative follow-up for 6 months to 3 years demonstrated that all of the patients had good aesthetic facial appearances.
CONCLUSIONS: The mandibular outer cortex can be used for orbital and maxillary augmentation of oblique facial clefts.

Infants with craniosynostosis involving the metopic and coronal sutures require cranio-orbital reshaping to correct craniofacial dysmorphologic feature and to improve facial balance. Currently, surgical techniques to create a balanced fronto-orbital region are based on the surgeon's subjective approach and artistic vision in creating a normal shape to the forehead. To date, the use of age-matched templates and computer-assisted design/computer-assisted manufacturing techniques in optimizing the outcomes of surgical intervention in this area have not been explored. The aim of this article was t

Traumatic auricular amputation due to human bite is an uncommon event. Nonetheless, it poses a difficult challenge for the reconstructive surgeon. Although microsurgery must be performed in some cases, most microsurgical techniques are too complex and must be performed in specialized centers only. Replantation of a severed ear without microsurgery can be a safe alternative as long as a proper technique is selected. Here, 2 successful ear replants performed without microsurgical technique are described. The ear cartilage was used with the perichondrium after degloving of the skin from the carti

Despite the fact that common surgical techniques for the treatment of genu varum usually correct the malalignment in the affected knee, these methods have significant complications and cause problems in the long term. Retro-tubercle opening-wedge high tibial osteotomy is among the newer techniques for the treatment of genu varum. The goal of this study was to compare the results of retro-tubercle opening-wedge high tibial osteotomy with those of medial opening-wedge osteotomy. In a randomized, controlled trial, 72 patients with varus knees who were scheduled for surgery were assigned into eith

Avascular necrosis of the talus is a well-documented complication following fractures, although less is known about this complication following reconstructive foot and ankle surgery. Knowledge of the blood supply of the talus is necessary for surgical dissection of the talus, especially when the approach traverses the sinus tarsi. In this article, we describe a rare case of avascular necrosis of the talus after talonavicular arthrodesis, using a medial surgical approach.
