Periodic Health Evaluations in the Adaptive Athlete: Redefining "Medical Clearance".

The periodic health evaluation (PHE) is a tool used to assess, monitor, and identify barriers to safe participation in sport. Currently, there are no standards associated with the PHE in the adaptive sport population. These athletes have a higher prevalence and a wide variety of health problems while also having delayed access to appropriate health care. This article provides education and standardized considerations of the PHE for adaptive athletes. With guidance, health care providers are more prepared to perform comprehensive and clinically appropriate care to this at-risk population, while

Utilization and determinants of antenatal care services among women in rural hilly regions of Kalimpong and Darjeeling, India: In light of the revised WHO guideline.

Antenatal care (ANC) is a critical component of maternal and child health, as it enhances outcomes for both the mother and her newborn. We assessed ANC utilization and its key determinants in the rural hilly districts of Kalimpong and Darjeeling, focusing on ANC visits, ANC quality, and complete ANC. A cross-sectional study of 660 women aged 15-49 was analyzed using descriptive, bivariate, and logistic regression analyses. The results of the authors' analysis reveal a substantial gap in both the frequency and quality of ANC. Women's education emerged as a key determinant, positively influencin

"We feel ashamed to talk about it". Navigating mental health problems for women in rural Pakistan: An interplay of financial, sociocultural, and environmental factors.

The authors of this research explored community perspectives on women's mental health in rural Pakistan, using a qualitative approach to identify beliefs hindering women's access to mental healthcare. Data were collected through 15 Focus Group Discussions (FGDs) and 15 Key Informant Interviews (KIIs) with stakeholders in Gilgit-Baltistan, using purposive sampling. The researchers revealed low community awareness of mental health and a lack of diagnostic and treatment services for women. Financial, sociocultural, and environmental factors, along with gender discrimination, poverty, and violence

Evaluating the effect of an educational program on increasing cervical cancer screening behavior among rural women in Guilan, Iran.

BACKGROUND: Cervical cancer is one of the major health problems and the third prevalent cancer in women all around the world. As a simple, inexpensive, and with no side-effects, Pap test is a reliable way to screen cervical cancer. This study aimed to investigate, the effects of educational intervention based on the Health Belief Model (HBM) on doing Pap smear tests among the rural women of the north of Iran. METHODS: In a quasi-experimental study, 160 rural women were randomly divided into control and experimental groups to experience a three-session intervention. The experimental group rece

Recruitment and reach in a pragmatic behavioral weight loss randomized controlled trial: implications for real-world primary care practice.

BACKGROUND: Obesity is a major risk factor behind some of the most common problems encountered in primary care. Although effective models for obesity treatment have been developed, the 'reach' of these interventions is poor and only a small fraction of primary care patients receive evidence-based treatment. The purpose of this study is to identify factors that impact the uptake (reach) of an evidence-based obesity treatment program within the context of a pragmatic cluster randomized controlled trial comparing three models of care delivery. METHODS: Recruitment and reach were evaluated by the

The Oportunidades conditional cash transfer program: effects on pregnancy and contraceptive use among young rural women in Mexico.

CONTEXT: Oportunidades is a large conditional cash transfer program in Mexico. It is important to examine whether the program has any direct effect on pregnancy experience and contraceptive use among young rural women, apart from those through education. METHODS: Data from the 1992, 2006 and 2009 waves of a nationally representative, population-based survey were used to describe trends in pregnancy experience, contraceptive use and education among rural adolescent (15-19) and young adult (20-24) women in Mexico. To examine differences in pregnancy experience and current modern contraceptive u

Mapping chronic illness in the age of globalization: reclaiming the good for the chronically ill.

Until recently, infectious diseases were the main cause of death worldwide. New medical discoveries and the evolution of public health improved life expectancy and the ability to survive acute threats, thus changing the course of diseases from acute to chronic. Today, chronic illness is the most important health concern worldwide. Chronic illness increases existing poverty and pushes other people into it. As nurses, members of the healthcare system and members of this world, we cannot forget that our response toward globalization and chronic disease has to be centered in leadership through reo

An evaluation of surveillance of malaria at primary health care level in Kenya.

As less than twenty five per cent of persons suffering from malaria seek formal treatment in most of sub-Saharan Africa, Facility-based morbidity statistics are inadequate for monitoring malaria control programmes. This explorative study assessed whether a health centre equipped with a microscope and trained personnel could monitor malaria transmission within its catchment area. The study was conducted at Chemase Health Centre in Nandi District in Kenya, an area holoendemic for malaria with Anopheles gambiae as the main vector and Plasmodium falciparum as the commonest cause of malaria. From f

Geographic and Social Equity in Population-Wide Genomic Screening.

IMPORTANCE: Genomic screening for Centers for Disease Control and Prevention Tier 1 conditions, ie, hereditary breast and ovarian cancer syndrome (HBOC), Lynch syndrome, and familial hypercholesterolemia, enables early detection of these rare, but highly penetrant disorders and supports timely, evidence-based interventions. However, states with large rural and socially disadvantaged populations, such as South Carolina, face substantial structural and access-related barriers that limit the reach of population-wide genomic screening (PWGS). OBJECTIVE: To examine whether combining implementation

Associations between pregnancy-related anxiety and positive health behaviors across pregnancy in a sample of low-income, vulnerable women.

PROBLEM: Positive health behaviours during pregnancy are essential for maternal, fetal, and child wellbeing but can be compromised by poor maternal mental health. Pregnancy-related anxiety (PRA) - including concerns about bodily changes across pregnancy, childbirth fears, and worries about fetal/infant health - reduces positive health behaviours in studies of mostly low-risk women. QUESTIONS: It is unclear whether this relation between PRA and positive health behaviours exists in lower-income women with barriers to engaging in positive health behaviours, or how it is influenced by other menta

Investigating environmental heat exposure and its effects on maternal and fetal health in rural Northern Ghana (Physio-HeMAB Study): a study protocol for a cluster randomized trial.

BACKGROUND: Climate change increases the frequency and intensity of heat events. Previous studies have established a correlation between heat exposure and adverse pregnancy outcomes, including preterm birth, stillbirth, and other maternal and fetal health complications. However, evidence on the physiological pathways underlying heat strain during pregnancy and on whether heat adaptation strategies can modify these responses remains limited, particularly in low-resource settings. This study aims to assess the effects of environmental heat exposure on maternal and fetal physiological responses a

Bridging the gap: Evaluating a community-based palliative care intervention for underserved patients with Parkinson's disease.

BACKGROUND: Parkinson's disease (PD) is a progressive neurodegenerative illness with a profound impact on quality of life (QOL). While palliative care has demonstrated benefits in PD, reach and evidence of effectiveness in underserved populations remain limited. The goal of this study was to evaluate the effect of a novel palliative care intervention on QOL among underserved patients with PD. METHODS: We conducted a secondary analysis of data from a stepped-wedge, pragmatic clinical trial (NCT03076671) among 322 participants with PD. We created an "underserved demographic" composite variable

Development of One Health-based community surveillance system to study zoonotic diseases: a study protocol.

Animals are responsible for a substantial burden of infectious diseases and pose a serious threat to human health, particularly in peri-urban areas where human-animal-environment interfaces are intensified due to rapid urban expansion. This protocol aims to develop a One Health-based community surveillance system to assess the prevalence, transmission dynamics and risk factors associated with brucellosis, leptospirosis and scrub typhus in a peri-urban setting in Odisha, India. We will conduct a prospective mixed-methods cohort study over 3 years in two peri-urban blocks of Khordha district. Ab

Telemedicine-based integrated management of atrial fibrillation in village clinics: a cluster randomized trial.

In rural China, where healthcare relies on village doctors (nonspecialized practitioners who work exclusively in their village clinics), delivering integrated atrial fibrillation (AF) management poses challenges. We developed a telemedicine-based, village doctor-led integrated care model and conducted a cluster randomized clinical trial to assess its efficacy compared to usual care. A total of 30 village clinics were randomly assigned (1:1) to the intervention or control group, with 1,039 village residents aged ≥65 years with AF (44.3% women) recruited. The primary outcome in stage 1 is adhere

Exploring Health Care Barriers for the Unhoused: Insights from a Rural Midwestern Community.

INTRODUCTION: People experiencing homelessness are more likely than the general population to have chronic health conditions and often encounter significant barriers to health care access. Many of these barriers can be affected by community-based factors, such as availability of reliable transportation, past experiences with health care systems, and community attitudes toward the unhoused population. This project aims to assess the needs and barriers to health care identified by people experiencing homelessness in a rural Midwestern city. METHODS: The survey used was adapted from a survey pre
