[Relationships between insight and medication adherence in subjects with psychosis].

BACKGROUND: Poor medication adherence in subjects with psychosis has a high prevalence and a negative impact on clinical outcome. Several studies have reported that a poor level of insight was a strong predictor of poor medi-cation adherence. However, few studies have investigated whether insight was associated with medication adherence, independently from other clinical and treatment characteristics. OBJECTIVE: To explore the link between insight and medi-cation adherence in subjects with psychosis, and to assess the impact of potential confounding factors on this association. METHOD: Subje

[Mental disorders induced by experiences of being bullied].

The author investigated 25 adolescent patients (9 males, 16 females) whose mental disorders seemed to be induced by their experiences of being bullied at school. The patients were treated by the author in the outpatient clinic or the inpatient ward of National Defense Medical College Hospital during 10 years, 1978 through 1988. These patients were classified into 4 groups from the viewpoint of their clinical symptoms and signs. The author called each group as follows; 1) paranoid-hallucinatory group, 2) withdrawal group, 3) escaping-into-fantasy group, and 4) somatoform group. The paranoid-hal

Psychiatric symptoms after therapy with new antiepileptic drugs: psychopathological and seizure related variables.

The purpose of this paper is to understand the association between antiepileptic drugs (AEDs), patient characteristics, changes in seizure pattern and emergent psychiatric disorder, i.e. psychosis or affective disorder. To this end we carried out a retrospective casenote study on 89 patients who developed psychiatric symptoms during treatment with topiramate, vigabatrin or tiagabine. The psychiatric problem was either an affective or a psychotic disorder (not including affective psychoses). It was discovered that 99% of the patients suffered from complex partial seizures with or without second

Factors associated with the frequency of manic and depressive episodes in bipolar disorder: a multicenter study in China.

BACKGROUND: Mania and depression are the predominant mood episodes in bipolar disorder (BD), and their frequency significantly affects the long-term prognosis of patients. METHOD: This is a multicenter, longitudinal cohort study in China. Sociodemographic and clinical characteristics of patients were statistically analyzed. Poisson regression analyses were performed to identify factors associated with the frequency of manic and depressive episodes. RESULTS: A total of 520 BD patients were enrolled in this study. Poisson regression model analysis showed that shorter years of education (OR = 1

Schizophrenia comorbid with panic disorder: evidence for distinct cognitive profiles.

Patients with comorbid schizophrenia and panic symptoms share a distinct clinical presentation and biological characteristics, prompting some to propose panic psychosis as a separate subtype of schizophrenia. Less is known about these patients' neuropsychological profiles, knowledge of which may facilitate target-specific treatments and research into the etiopathophysiology for such cases. A total of 255 schizophrenia patients with panic disorder (n=39), non-panic anxiety disorder (n=51), or no anxiety disorder (n=165) were assessed with the Wechsler Adult Intelligence Scale-Revised, the Wisco

Implementing composite quality metrics for bipolar disorder: towards a more comprehensive approach to quality measurement.

OBJECTIVE: We implemented a set of processes of care measures for bipolar disorder that reflect psychosocial, patient preference and continuum of care approaches to mental health, and examined whether veterans with bipolar disorder receive care concordant with these practices. METHOD: Data from medical record reviews were used to assess key processes of care for 433 VA mental health outpatients with bipolar disorder. Both composite and individual processes of care measures were operationalized. RESULTS: Based on composite measures, 17% had documented assessment of psychiatric symptoms (e.g.,

The impact of diagnostic misclassification on the pattern of familial aggregation and coaggregation of psychiatric illness.

Family studies in psychiatry have traditionally been interpreted as if psychiatric diagnoses were made without error. This is unlikely to be true. The first section of this paper presents a model, based on several simplifying assumptions, which examines the impact of diagnostic misclassification on patterns of familial aggregation and coaggregation of two disorders. In the second section, the model is illustrated by examining the effect of varying misclassification rates on the patterns of aggregation and coaggregation in four simulated family studies of psychotic illness. Misclassification is

Effectiveness of Cognitive Behavioral Therapies Targeting Cognitive Biases and Beliefs in Schizophrenia-Spectrum Disorders: A Systematic Review and Meta-Analysis.

BACKGROUND AND HYPOTHESIS: Cognitive models of schizophrenia-spectrum disorders (SSD) posit dysfunctional beliefs and cognitive biases as maintenance mechanisms of positive and negative symptoms. Although cognitive behavioral therapy (CBT) targets these processes, its effects on mechanism-level outcomes remain unclear. This review examined whether CBT modifies dysfunctional beliefs and cognitive biases in SSD using rigorous randomized evidence. STUDY DESIGN: PRISMA 2020-compliant systematic review and meta-analysis (PROSPERO registered). Primary analyses were restricted to intention-to-treat

Cognitive Behavior Therapy for Psychosis in People with Intellectual Disability-A Scoping Review.

BACKGROUND AND HYPOTHESIS: Cognitive behavior therapy for psychosis (CBTp) is effective in reducing psychiatric symptoms. Most CBTp trials have excluded patients with intellectual disabilities, even though intellectual disabilities are associated with an at least 3 times higher psychosis prevalence. More understanding of evidence-based approaches for this population is needed. The aim of this review was to assess the current state of knowledge about CBTp for people with both intellectual disabilities and psychosis. Specifically, the interests were the efficacy and feasibility of CBTp for this

Clinical High-Risk for Psychosis in Latinx Populations: Common Issues and Recommendations for Improving Care.

AIM: Current estimates suggest that about 1.7% of the general population meets criteria for clinical high-risk for psychosis (CHR), but Latinx individuals are more likely to be at higher risk due to factors that could inflate rates of symptomatology. Disparities in screening, access to services, and intervention for Latinx individuals at CHR persist despite calls to action. METHODS: Through the lens of service utilisation models, the present narrative review builds on prior calls to action by summarising the literature and providing culturally sensitive recommendations for clinicians and rese

Evaluation and Treatment Considerations for Children and Adolescents Presenting With Psychosis Symptoms in Primary Care.

Pediatricians and all other pediatric primary care clinicians are likely to encounter youth who report symptom concerns of psychosis, such as auditory or visual hallucinations. It is essential for clinicians to be comfortable evaluating and triaging these symptoms, including assessment of co-occurring safety concerns, to ensure access to appropriate treatment. Due to limited specialty mental health resources nationwide, pediatricians may also be tasked with initiating medications or referring directly to evidence-based psychosocial interventions in the community. This article reviews different

The Association Between Attachment Style and Symptomatic, Social, and Personal Recovery: A Comparison Between Male and Female Patients Remitted From Their First-Episode Psychosis.

BACKGROUND: Sex differences in recovery outcomes for psychosis have been observed. In patients with psychosis, rates of insecure attachment are significantly higher in patients of both sexes compared to the general population and have been linked to several aspects of recovery. However, the possible differential effect of attachment style on recovery between men and women with psychosis is currently unknown. METHODS: This study was performed in a subsample of 299 patients remitted from their first-episode psychosis (FEP) within the Handling Antipsychotic Medication Long-term Evaluation of Tar

Duration of untreated psychosis and long-term outcomes: A systematic review examining the critical window hypothesis.

BACKGROUND: The "critical window hypothesis" posits that prolonged duration of untreated psychosis (DUP) causes irreversible neurobiological damage during a time-sensitive period, leading to worse long-term outcomes. While widely accepted as justification for early intervention psychiatry, empirical evidence supporting this hypothesis remains contentious. This systematic review evaluates DUP-outcome relationships across symptomatic, functional, cognitive, and neurobiological domains. METHODS: Following PRISMA guidelines, we searched five databases through December 2025 for longitudinal studie

Long-range temporal correlations in violence and psychosis: An EEG-based study.

BACKGROUND: A subgroup of individuals with psychosis have increased violence risk. Long-range temporal correlations (LRTC) in electroencephalography (EEG) reflect neural dynamics, but their relevance to violence risk remains unclear. METHODS: Resting-state EEG was collected from four groups: healthy controls (HC, n = 134), individuals with psychosis and violence history (violent-PSY, n = 16), individuals with psychosis without such history (nonviolent-PSY, n = 38), and violent individuals without psychosis (violent non-PSY, n = 23). Avolition predicts variability in optimal risk-taking in early psychosis.

BACKGROUND: Adolescence and early adulthood are associated with increased risk-taking, with a growing understanding that risk-taking may be adaptive during this developmental period. In psychosis, failure to appropriately engage in optimal risk-taking due to altered reward processing may contribute to poor social and role functioning. We investigated differences in optimal risk-taking in early psychosis (EP), as well as the relationship between optimal risk-taking and the negative symptom of avolition. METHODS: Fifty-six EP participants and 52 healthy controls (HC) completed a modified versio
