Antipsychotic drug dosing and study discontinuation in schizophrenia: A systematic review and dose-response meta-analysis.

BACKGROUND: High discontinuation rates compromise the effectiveness of treatment regimens for schizophrenia, because consistent medication adherence is essential for the efficacy of antipsychotics. Understanding the relationship between antipsychotic doses and discontinuation rates is important. This study explores this relationship to identify doses that maximize treatment adherence and minimize discontinuation. METHODS: We systematically searched multiple electronic databases for fixed-dose RCTs assessing 20 antipsychotics in patients with acute exacerbation of schizophrenia and related dis

Incidence and predictors of postoperative delirium following remimazolam administration: a systematic review and meta-analysis of 29 randomized trials.

BACKGROUND: Postoperative delirium is a significant and common complication in surgical patients, particularly in vulnerable populations such as the elderly. Remimazolam, a novel benzodiazepine, has been introduced as an anesthetic agent with a favorable pharmacokinetic profile. However, its potential association with postoperative delirium remains unclear. This study aims to systematically synthesize available evidence on the incidence of delirium following remimazolam administration in surgical patients. We sought to identify significant moderators of delirium incidence and to explore predic

Comparative efficacy and acceptability of novel biologics in the treatment of myasthenia gravis: systematic review and network meta-analysis of randomized trials.

BACKGROUND: Myasthenia gravis (MG) is a chronic autoimmune disorder affecting the neuromuscular junction. The emergence of molecular therapies, such as monoclonal antibodies, B-cell-depleting agents, and chimeric antigen receptor T-cell-based therapies, has the potential to transform the treatment landscape for myasthenia gravis. The clinical efficacy of novel biologics in the treatment of individuals with myasthenia gravis is still a subject of debate. The objective was to compare and rank the efficacy and acceptability of novel biologics in the treatment of individuals with MG through a netw

Efficacy and safety of dexamethasone in postoperative recovery following hysterectomy: a systematic review and meta-analysis.

OBJECTIVES: Hysterectomy, a common surgical procedure, is frequently associated with moderate-to-severe postoperative pain and a high incidence of postoperative nausea and vomiting (PONV). Dexamethasone, a corticosteroid, may help alleviate these symptoms; however, existing evidence is largely drawn from mixed surgical populations and does not specifically address its efficacy and safety in hysterectomy patients. This meta-analysis provides a focused and updated synthesis of randomised controlled trials (RCTs) in this population, incorporating time-stratified pain outcomes and subgroup analyse

Impact of functional re-education and environmental adaptation in cancer patients with respiratory pathology: Study protocol.

BACKGROUND: In recent years, cancer survival rates have increased exponentially. However, this rise in survival comes with a significant drawback. As the number of treatment lines has grown, so too have the side effects, which can severely impact patients' functionality and quality of life. One of the most concerning effects is dyspnoea, a serious health issue that imposes substantial limitations on individuals. While traditional clinical approaches, primarily focused on pharmacological interventions, are commonly employed to manage dyspnoea, we argue that these methods may not always provide

Bundle-Of-Care vs. Single-Intervention RCTs in Neonatology: Clinical Research in Neonatology-Quo Vadis?

Extreme prematurity is a multi-organ medical emergency. Most high-profile studies in this field are done as single-intervention randomised controlled trials (RCTs), often yielding negative or inconclusive results. Given the complexity of extreme prematurity, we suggest applying future RCTs using a bundle-of-care approach. Such an approach should take into consideration: (a) results from RCTs and meta-analyses demonstrating a positive, albeit non-significant effect of a specific intervention; (b) fundamental physiological aspects unique to the neonatal period; and (c) specific interventions aim
