A systematic review of the definitions, prevalence, and response to treatment of nocturnal gastroesophageal reflux disease.

BACKGROUND & AIMS: More than half of patients with chronic gastroesophageal reflux (GERD) report nocturnal symptoms. We performed systematic literature review to define nocturnal heartburn and to determine potential causality between nocturnal reflux and extraesophageal manifestations. METHODS: We performed a search of literature published from 1974-2007. Each study was examined by 2 reviewers and rated on the basis of study type and outcome. RESULTS: Screening of 445 trials identified 59 (13%) studies relevant for analysis. Twenty-two (5%) of the trials described potential changes in sleep

Does melatonin play a role in aging processes?

Increasing number of people in advanced age is one of the most distinctive demographic events of the 21(st) century which raise many social and medical issues. Therefore, there is a search for any therapeutic agent improving the quality of life of the elderly. Melatonin, the hormone of the pineal gland, received recently a great deal of attention because of its suggested role in aging processes and availability as over-the-counter drug or food supplement in some countries, including Poland. In this survey the basic data on the possible role of melatonin in human aging as well as its possible t

Disturbance and strategies for reactivation of the circadian rhythm system in aging and Alzheimer's disease.

Circadian rhythm disturbances, such as sleep disorders, are frequently seen in aging and are even more pronounced in Alzheimer's disease (AD). Alterations in the biological clock, the suprachiasmatic nucleus (SCN), and the pineal gland during aging and AD are considered to be the biological basis for these circadian rhythm disturbances. Recently, our group found that pineal melatonin secretion and pineal clock gene oscillation were disrupted in AD patients, and surprisingly even in non-demented controls with the earliest signs of AD neuropathology (neuropathological Braak stages I-II), in cont

Sleep-related laryngospasm caused by gastroesophageal reflux.

Gastroesophageal reflux is a suspected cause of sleep-related laryngospasm [Thurnheer R, Henz A, Knoblauch A. Sleep-related laryngospasm. Eur Respir J 1997;10:2084-6], but the relationship has not been directly documented by polysomnography (PSG). We report the case of an eight-year-old girl with asthma who was referred for evaluation of awakenings with respiratory distress. The description of her events included stridorous breathing suggestive of laryngospasm. A PSG was performed with esophageal pH monitoring, and during the recording pH dropped below four on nine occasions. Following two of

Management of Parkinson's disease dementia : practical considerations.

Parkinson's disease dementia (PDD) ultimately develops in about 80% of patients with Parkinson's disease (PD), and cross-sectional studies have found that some 30% of these patients will experience neuropsychiatric symptoms, such as visual hallucinations and psychosis. The most consistently reported risk factors for dementia in PD are age, severe parkinsonism and mild cognitive impairment. In PDD, both subcortical cognitive and cortical cognitive profiles are described. Specific disorders of sleep, such as rapid eye movement sleep behaviour disorder, excessive daytime sleepiness and sleep atta

[Drug-induced sleep disorders].

Drug-induced sleep disorders are classified into the following subtypes: a) insomnia, b) hypersomnia, c) pathological sleep d) others, those caused by medicine which can effect on circadian rhythm or sleep apnea. This paper described the relationship between several medical agents (neuroleptics, anti-depressants, anti-anxietics, anti-convulsants, psychostimulants, and drugs other than CNS-effective agents) and those effect on sleep and sleep stages. In the medicated patients, medical drugs are mainly responsible for their insomnia. However, it is sometimes difficult to find these drugs. The pa

Polysomnography in idiopathic muscle pain syndrome (fibrositis).

Muscle pain occurs in various neuromuscular disorders with characteristic physiological or biochemical abnormalities. There is, however, a group of patients in whom there is no clear physiological or structural basis for their pains. This syndrome has been called fibrositis or fibromyalgia. Sleep abnormalities have been reported in some of these patients, but have not been confirmed by others. We studied 8 patients with this disorder and found sleep abnormalities that were characterized by nocturnal myoclonus, alpha-delta sleep, and abnormalities compatible with depression. Polysomnography was

[Sleep disturbances in multi-infarction dementia and trials of treatment with caffeine].

Atherosclerotic cerebrovascular lesions leading to repeated ischaemic strokes produce a wealth of clinical symptoms and signs with dementia. Sleep disturbances are frequent and take the form of difficult falling asleep and shifting of the sleep/waking rhythm to late night and morning hours. Hypnotic drugs produce often a paradoxical effect with complete reversal of the circadian sleep rhythm. These observations gave the inspiration to a trial of reversed treatment: with administration of analeptics in the evening and sedative drugs during the day. Sixteen patients with this dementia and pronou

Comparative toxicity of amantadine hydrochloride and rimantadine hydrochloride in healthy adults.

The relative toxicities of amantadine and rimantadine were compared in a double-blind, placebo-controlled study involving healthy adults. In separate studies, drugs were administered at a dosage of 200 mg/day (52 volunteers) or 300 mg/day (196 volunteers) for 4.5 days. Both drugs were well tolerated at the lower dosage. At 300 mg/day amantadine recipients had a greater frequency and severity of central nervous system (nervousness, lightheadedness, difficulty concentrating) and sleep (insomnia, fatigue) complaints compared with rimantadine or placebo recipients. Amantadine recipients also perfo

[Clinical trial by perexiline maleate in treatment of angina pectoris (author's transl)].

A controlled double blind clinical trial has been conducted in 16 patients with "angina pectoris" in order to investigate the effect of Perexiline maleate as compared with prenilamine. Perexiline at the dose of 400 mg/die and prenilamine at the dose of 120 mg/die have been administered over a period of 4 weeks each. Between these periods placebo has been administered for two weeks. The number of attacks of angina and the number of tablets of nitroglycerine used per week by the patient during each period has been used for the evaluation. Furthermore ECG at rest and after exercise has been perfo

Effect of ASP5633, a novel peripherally restricted MT1/MT2 receptor agonist, on urethral function in female rats.

Melatonin is a hormone that regulates numerous physiological processes through activation of specific G protein-coupled receptors, termed MT1 and MT2 receptors. We previously reported that stimulation of MT1 receptors in female rat urethra activated intracellular Ca2+ mobilization via a Gq protein-dependent pathway, resulting in urethral smooth muscle contraction. This study aimed to investigate the potential of MT1 receptor-related urethral contraction in the treatment of urological disorders using ASP5633 (3-(2-acetamidoethyl)-6-fluoro-5-methoxy-N-methyl-1H-indole-2-carboxamide), a novel per

Mapping the Connection Between Circadian Rhythms, Metabolism, and Neurodegeneration: Exploring Therapeutic Strategies.

Circadian rhythms are crucial for essential physiological functions such as metabolism, sleep-wake cycles, hormone balance, and cognitive abilities, which are regulated by the central Suprachiasmatic Nucleus (SCN) and peripheral clocks. Disruptions to circadian rhythms, which may be caused by aging, lifestyle factors, and environmental influences, are linked to metabolic disorders and Neurodegenerative Diseases (NDs). This review examines the reciprocal relationship between circadian control and metabolism, highlighting the molecular processes that maintain circadian rhythms and how these proc

Exploring the connection between the Mitochondrial DNA and the Circadian Rhythm: Insights into Mitochondrial Biogenesis and its Dynamics.

Mitochondrial DNA (mtDNA), inherited exclusively from the mother, encodes genes essential for mitochondrial function, including oxidative phosphorylation (OXPHOS), which generates ATP, the cell's primary energy currency. Circadian rhythm is a crucial biological system that refers to the innate biological clock, whose core is in the suprachiasmatic nucleus (SCN) of the brain. This nucleus regulates various physiological processes, such as sleep-wake cycles, hormone secretion, cellular repair, energy homeostasis, and metabolism, on a roughly 24-hour cycle. Peripheral clocks exist in various tiss
