Cytokeratin-Positive Pleomorphic Chest Wall Sarcoma Mimicking MPNST: A Case Report.

BACKGROUND: Undifferentiated pleomorphic sarcoma (UPS) is an uncommon high-grade soft-tissue sarcoma and remains a diagnosis of exclusion because it lacks specific differentiation and pathognomonic markers. Primary chest-wall involvement is rare, and epithelial-marker expression may complicate diagnosis by broadening the differential diagnosis. CASE PRESENTATION: A 62-year-old male carpenter and active smoker with diabetes mellitus and ischemic heart disease presented with persistent right-sided chest pain radiating to the back. Examination revealed a firm right anterolateral chest-wall mass

Case Report: Iliac perivascular myeloid sarcoma presenting with iliofemoral deep vein thrombosis and hydronephrosis.

Myeloid sarcoma (MS) is a rare extramedullary tumor of myeloid blasts that may occur before, concurrently with, or after acute myeloid leukemia (AML) (1-4). We report a 53-year-old man with progressive left lower-limb pain and swelling, initially admitted to the vascular surgery service, with imaging revealing left external iliac vein and proximal common femoral vein occlusive thrombosis, a peri-iliac mass encasing the iliac vessels, and ipsilateral hydronephrosis. Biopsy of the left inguinal lesion showed MPO positivity, CD34 positivity, partial TdT positivity, and a Ki-67 index of about 80%,

Merkel Cell Carcinoma of the Thigh Presenting as a Hemorrhagic Mass: A Rare Case Report and Literature Review.

BACKGROUND: Merkel cell carcinoma (MCC) is a rare, aggressive primary cutaneous neuroendocrine carcinoma with a marked propensity for early regional lymph node metastasis. Although MCC most often arises on sun-exposed head and neck skin in older adults, tumors of the lower extremity are uncommon and may be mistaken for benign hemorrhagic lesions. CASE PRESENTATION: A 54-year-old woman developed a rapidly enlarging, hemorrhagic mass in the left suprapatellar thigh. Magnetic resonance imaging demonstrated an extracompartmental subcutaneous soft-tissue mass without quadriceps muscle invasion. Wi

EVT-Inspired Simplified Negative-Pressure Nasoesophageal Drainage for Cervical Abscess Secondary to Esophageal Perforation.

BACKGROUND Esophageal foreign body perforation is a critical emergency needing prompt diagnosis and intervention. The innovative esophageal vacuum therapy (EVT) has been proven effective in the treatment of esophageal perforations and fistulas. However, its high cost and technical complexity limit its widespread use in primary care facilities. CASE REPORT A 52-year-old man presented after 4 days of odynophagia and 1 day of fever following accidental fish bone ingestion. Computed tomography (CT) demonstrated cervical esophageal perforation with soft-tissue emphysema. Endoscopy revealed a 5-mm p

Detection of disease recurrence in real-world patients with completely resected stage III cutaneous melanoma.

BACKGROUND AND PURPOSE: Optimal postoperative follow-up for high-risk cutaneous melanomas (CM) is unclear. We aimed to evaluate disease recurrences in patients with stage III CM before modern adjuvant therapies. Patient/material and methods: Three hundred and fifty patients with a median follow-up of 8.9 years at Helsinki University Hospital Comprehensive Cancer Center after complete resection of stage IIIA-D CM between 2008 and 2017 were identified. Information on baseline characteristics, recurrence patterns and survival were collected from electronic medical records. RESULTS: One hundred a

Baseline dual-energy CT iodine uptake predicts systemic but not lesion-level progression in immunotherapy-treated metastatic melanoma.

BACKGROUND: Immune checkpoint inhibitors (ICI) have transformed melanoma treatment, but reliable prognostic biomarkers are lacking. Dual-energy CT (DECT)-derived iodine concentration is a non-invasive quantitative measure of tumor vascularity. We investigated whether spatially resolved baseline iodine concentration predicts ICI outcomes in metastatic melanoma. METHODS: We retrospectively analyzed 73 patients with metastatic melanoma (614 baseline lesions) treated with ICI who underwent baseline abdominal DECT. Each lesion was partitioned into three concentric shells (core, transition zone, an

Peribiliary Inflammatory Phlegmon Masquerading as Cholangiocarcinoma in Janus Kinase 2 V617F-positive Primary Myelofibrosis: A Multimodality Imaging Diagnosis.

Peribiliary inflammatory phlegmon arising from chronic portal venous thrombosis can produce a morphological replica of periductal cholangiocarcinoma at the hepatic hilum. We report a 68-year-old woman referred with an outside magnetic resonance imaging diagnosis of cholangiocarcinoma, in whom multimodality imaging reassessment demonstrated a 68 mm × 37 mm × 32 mm porta hepatis soft-tissue lesion encasing the biliary confluence. The common bile duct measured only 5 mm despite circumferential encasement, disproportionately limited for malignant transmural invasion. Color Doppler ultrasonography

A Minor Sports Injury with Major Consequences: Probable Streptococcal Toxic Shock Syndrome and Necrotizing Soft Tissue Infection in a Young Adult-A Case Report.

Background and Clinical Significance:Streptococcus pyogenes (group A Streptococcus, GAS) can cause rapidly progressive invasive infections, including necrotizing soft tissue infection (NSTI) and streptococcal toxic shock syndrome (STSS). Although invasive GAS disease is often associated with skin barrier disruption, severe infection may also follow blunt trauma without visible skin injury. Case Presentation: A 22-year-old woman presented with persistent right hip and groin pain four days after a blunt fall during recreational sports activity, without disruption of skin integrity. On admission,

Cytologic Features of Inflammatory Rhabdomyoblastic Tumor Using Conventional and Liquid-Based Cytology Preparations: A Case Report.

Inflammatory rhabdomyoblastic tumor (IRMT) is a recently recognized skeletal muscle neoplasm of uncertain malignant potential, characterized by a prominent inflammatory infiltrate. Although its histologic features are increasingly being defined, reports describing its cytologic characteristics remain extremely limited. Herein, we describe the detailed cytologic findings of a case of IRMT across multiple cytologic preparations. A man in his 60s presented with pharyngeal discomfort and was diagnosed with tonsillar diffuse large B-cell lymphoma (DLBCL). Subsequent staging positron emission tomogr

Diffuse large B-cell lymphoma mimicking musculoskeletal nontuberculous mycobacterial infection: A fatal diagnostic challenge.

BACKGROUND: Musculoskeletal infections caused by nontuberculous mycobacteria (NTM) can clinically and radiologically mimic malignant disorders. Accurate diagnosis requires correlation between microbiological, histopathological, and clinical findings. We report a case of diffuse large B-cell lymphoma (DLBCL) initially misdiagnosed as NTM infection based on polymerase chain reaction (PCR) positivity despite negative cultures. CASE REPORT: A 58-year-old man with diabetes mellitus and prior coronary artery bypass grafting presented with a progressively enlarging painful right flank mass. Magnetic

Blumgart Anastomosis Versus Invaginating Pancreatogastrostomy for Reconstruction after Pancreatoduodenectomy: A Randomized Controlled Trial.

OBJECTIVE: To compare postoperative pancreatic fistula (POPF) rates between Blumgart anastomosis (BA) and invaginating pancreatogastrostomy (PG) after pancreatoduodenectomy (PD). BACKGROUND: POPF rates after PD are still high. The only modifiable factor available to improve POPF is the anastomotic technique. BA and PG anastomoses have been previously shown to be feasible and safe, but they have never been compared in a randomized trial. METHODS: A multicenter, randomized, controlled trial was conducted in 13 University Hospitals. Eligible patients were those presenting a pancreatic or periam

Bilateral Asymmetry in Sperm Quality of Bank Voles: Fluctuating, Huge, and Warranting Attention.

Despite its potential significance for male fertility, bilateral testicular asymmetry in vertebrates remains poorly understood. Fluctuating asymmetry (FA), which serves as an indicator of stochastic developmental variation, constitutes a crucial yet frequently overlooked aspect of phenotypic variability. Although FA is commonly examined in morphological traits, its measurement in functional testicular traits has not been previously conducted due to methodological constraints. We analyzed FA in epididymal sperm motility (curvilinear velocity, VCL) and sperm concentration (SEC) in reproductively

Characterization of a bronchoscopically induced transgenic lung cancer pig model for human translatability.

There remains a need for animal models with human translatability in lung cancer (LC) research. Findings in pigs can have a substantial impact owing to their similar anatomy and physiology to humans. Here we present a bronchoscopically induced LC model in Oncopigs carrying inducible KRASG12D and TP53R167H mutations. A total of 12 Oncopigs underwent 29 injections via flexible bronchoscopy: 18 adenovirus-Cre recombinase gene (AdCre) inductions were performed endobronchially (n = 6) and transbronchially (n = 12), and 11 control injections were performed without AdCre. Oncopigs underwent serial ch

Proton radiography interpretation with artificial intelligence for treatment deviation detection in proton therapy.

BACKGROUND AND PURPOSE: Patient setup errors, anatomical changes, and uncertainties in proton range estimation degrade dose conformity in proton therapy. Adaptive proton therapy (APT) mitigates these deviations by monitoring and adjusting treatment plans. Proton radiography (PR) offers direct proton range information, making it a promising method to detect such deviations. In this study, we developed and evaluated an artificial intelligence (AI) PR tool for automated interpretation and classification of treatment deviations. MATERIALS AND METHODS: Computed Tomography (CT) scans from 32 head-a

Detailed microCT imaging protocol for ex vivo rat stomachs with comparative analysis.

Understanding gastric structure is essential for studying gastrointestinal disorders. While traditional histological analysis focuses on two dimensions, micro-computed tomography (μCT) offers high-resolution three-dimensional (3D) imaging but requires staining for proper visualization of soft tissues. This study presents the first detailed 3D anatomical reconstruction of the rat stomach using μCT, investigating optimal preparation and staining protocols to enhance image quality and ultimately help understand structural-functional relationships in rat stomachs. Fifteen rat stomachs were dissect

18F-FDG PET/CT identifies isolated metastasis of renal cancer in a patient with end-stage renal disease: A case report.

RATIONALE: Individuals with end-stage renal disease have a considerably higher rate of malignant tumors, especially renal cancer, in comparison to the general population. However, cases in which bone metastases in the humerus are the initial clinical presentation that results in a diagnosis of renal cancer are extremely rare. Diagnosing renal cancer in patients with end-stage renal disease can be challenging when the clinical symptoms are atypical and do not present the "renal cancer triad." Our case report highlights the diagnostic importance of positron emission tomography/computed tomograph
