Reverse dorsoradial metacarpal artery flap for reconstruction of large finger skin defect: A case report.

INTRODUCTION AND IMPORTANCE: Plastic reconstructive surgery for defects of hand still pose a challenge to plastic surgeons. The skin envelope of the hand is a complex structure that not only covers the underlying tissues but also includes particular functional and sensory components. The main principle of therapy is to provide adequate and long-lasting coverage to minimize surgery scars. CASE PRESENTATION: A 63 years old man was referred to our hospital from another facility with a soft tissue defect on his right index finger at the level of the interphalangeal joint. The injury occurred 4 ho

Volcanic disruption: the impact of Whakaari White Island on the provision of elective surgical care in Counties Manukau.

INTRODUCTION: On 9 December 2019, Whakaari White Island erupted while 47 people were on the island. Thirty-one people were subsequently hospitalized. Fourteen volcanic burn victims were managed at the National Burns Centre at Middlemore Hospital. Between December 2019 and March 2020 these patients required 124 procedures in theatre, using 23 709 operative minutes. Elective surgical lists were cancelled to fulfil this demand for acute operating theatre time and theatre staff. OBJECTIVES: To quantify the elective surgical resource lost in the aftermath of the Whakaari White Island eruption by s

The Impact of implant-based reconstruction on persistent pain after breast cancer surgery: A systematic review.

INTRODUCTION: With the increasing number of breast cancer survivors and reconstruction operations, persistent pain following breast cancer surgery (PPBCT) and its management is becoming a challenge for plastic surgeons. To date, most studies compared the difference in the level of PPBCT in reconstruction versus nonreconstruction groups. We systematically reviewed the literature to assess the impact of implant-based reconstruction on PPBCT levels. MATERIALS AND METHODS: PubMed, Embase, CINAHL, and Scopus databases were searched for relevant articles. We used five search strategies: (persistent

Clinical and Immunological Characteristics of Patients with Biopolymers and Autoimmune Inflammatory Syndrome Induced by Adjuvants.

UNLABELLED: Biopolymers consist of non-biocompatible allogeneic materials. They have been associated with autoimmune inflammatory syndrome induced by adjuvants, as described by Yehuda Shoenfeld and Nancy Agmon-Levin. Therefore, this study aimed to evaluate the clinical and immunological characteristics of patients with autoimmune inflammatory syndrome induced by adjuvants treated at a plastic surgery clinic in Colombia. METHODS: This cross-sectional, descriptive observational study included 190 patients with biopolymers in the buttocks with no evidence of autoimmune disease who were diagnosed

The Relationship between Age and Facial Asymmetry.

BACKGROUND: Facial symmetry is a fundamental goal of plastic surgery, yet some asymmetry is inherent in any face. Three-dimensional photogrammetry allows for rapid, reproducible, and quantitative facial measurements. With this tool, the authors investigated the relationship between age and facial symmetry. METHODS: The authors imaged normal subjects using three-dimensional photogrammetry. Facial symmetry was calculated by identifying the plane of maximum symmetry and the root-mean-square deviation. Regression analysis was used to assess the relationship between age and symmetry. Subgroup anal

A Multi-institutional Analysis of Insurance Status as a Predictor of Morbidity Following Breast Reconstruction.

BACKGROUND: Although recent literature suggests that patients with Medicaid and Medicare are more likely than those with private insurance to experience complications following a variety of procedures, there has been limited evaluation of insurance-based disparities in reconstructive surgery outcomes. Using a large, multi-institutional database, we sought to evaluate the potential impact of insurance status on complications following breast reconstruction. METHODS: We identified all breast reconstructive cases in the 2008 to 2011 Tracking Operations and Outcomes for Plastic Surgeons clinical

Potential use of craniosynostotic osteoprogenitors and bioactive scaffolds for bone engineering.

The cranial bone has a very limited regenerative capability. Patients with craniosynostosis (the premature fusion of cranial sutures, leading to skull abnormalities) often require extensive craniofacial reconstruction and repeated surgery. The possibility of grafting autologous osteoprogenitor cells seeded on bioabsorbable matrices is of great potential for inducing regeneration of craniofacial structure and protecting the brain from external insult. To this purpose we have studied the behaviour of normal and craniosynostotic mouse osteoblast cell lines, and of human primary osteoprogenitors f

Palmar arch revascularization for arterial occlusion of the distal upper extremity.

During a period of nine years, 10 patients, ages 42 to 66 years (55 +/- 7, mean +/- SD) underwent upper extremity bypass for ischemic changes to the hand not responsive to conservative management. Patients were referred from the vascular department at the authors' institution. Preoperative angiograms were performed and attempts at angioplasty or intravenous attempts to dissolve clots were carried out (with tissue plasminogen activator-tPA) when appropriate. Patients with persistent upper extremity ischemia and an obvious occlusion with reconstitution in the hand were candidates for upper extre

Brain plasticity in Möbius syndrome after unilateral muscle transfer: case report and review of the literature.

BACKGROUND: Möbius syndrome is mainly characterized by bilateral facial palsy. Facial reanimation of these children is achieved by microsurgical techniques, namely free-gracilis muscle innervated by the masseteric nerve. Notorious commissure excursion and speech improvement are reported with such procedure. Several studies have demonstrated the presence of cortical reorganization after injury and repair of different segments of the body. Intensive training of a behaviorally relevant task is key in this process. CASE REPORT: A 4-year-old patient with complete bilateral facial palsy secondary t

Tissue engineered bone versus alloplastic commercial biomaterials in craniofacial reconstruction.

This research was developed in order to demonstrate the tissue engineering method as an alternative to conventional methods for bone reconstruction, in order to overcome the frequent failures of alloplastic commercial biomaterials, allografts and autografts. Tissue engineering is an in vitro method used to obtain cell based osteoinductive bone grafts. This study evaluated the feasibility of creating tissue-engineered bone using mesenchymal cells seeded on a scaffold obtained from the deciduous red deer antler. We have chosen mesenchymal stem cells because they are easy to obtain, capable to di

[Modern principles of ophthalmic surgery for combat trauma: analysis of experience from the Special Military Operation].

Combat ocular trauma presents a complex medical and tactical challenge in modern armed conflicts, it is characterized by a high frequency of combined injuries and requires a specialized system of care. The experience of the Special Military Operation (SMO) provides unique clinical material for analysis and for developing modern surgical principles. This review describes the specific features of combat ocular trauma in the context of the SMO: the predominance of multiple radiolucent orbital foreign bodies, extensive lacrimal drainage system injuries, and a pronounced contusion component. The ar

Editorial Commentary: If You're Performing Superior Capsule Reconstruction With Human Dermal Allograft, Prepare a Thick Graft.

Pioneered by Mihata and then popularized in the United States by Burkhart and colleagues, superior capsule reconstruction quickly became an essential treatment for massive irreparable rotator cuff tears. More recently, enthusiasm seems to have faded as the surgery is highly resource-intensive, technically demanding, and clinical outcomes have been inconsistent, with some studies finding significant structural failure rates. The history of orthopaedic surgery is rife with examples of implants, concepts, and procedures abandoned because of suboptimal results and/or unacceptable complication rate

The long head of the biceps tendon as a pedicled autograft for acromioclavicular joint reconstruction: development of an arthroscopic technique.

Acromioclavicular joint (ACJ) dislocation is a frequent shoulder injury, and high-grade cases often require surgical reconstruction. Although multiple techniques have been described, no single method has achieved consensus as the gold standard. The long head of the biceps tendon (LHBT) represents an attractive autologous option, as it is anatomically adjacent to the ACJ, preserves vascularity when used as a pedicled graft, and reduces donor-site morbidity. Building on prior anatomical studies, this cadaveric investigation describes and validates an arthroscopic technique for ACJ reconstruction

A Decade of Craniosynostosis Surgery in the United States: Trends in Management, Payer Mix, and Cost.

BACKGROUND: Cranial vault remodeling (CVR) and endoscopic strip craniectomy (SC) are principal operations for nonsyndromic craniosynostosis. Over the past decade, evolving minimally invasive techniques and shifting payer landscapes may have influenced national practice patterns. We evaluated trends in surgical utilization, payer mix, and cost across US children's hospitals from 2016 to 2025. METHODS: A retrospective analysis of the Pediatric Health Information System identified infants with craniosynostosis who underwent CVR or SC between 2016 and 2025 using ICD and CPT codes. Demographics, p

Prophylactic Immediate Lymphatic Reconstruction for the Prevention of Secondary Lymphedema: A Systematic Review and Meta-Analysis of Comparative Studies.

BACKGROUND: Secondary lymphedema remains a frequent and debilitating complication of axillary and inguinal lymph node dissection within oncological settings. Prophylactic lymphovenous shunting (LVS) delivered as immediate lymphatic reconstruction (ILR) has emerged as a strategy to preserve lymphatic continuity and reduce postoperative lymphedema. This systematic review and meta-analysis evaluated the effectiveness of prophylactic LVS across oncologic indications. METHODS: A comprehensive search of Medline (PubMed), Embase (Elsivier) and Cochrane Central databases identified comparative studie

[Toe-to-hand transplantation for thumb and finger reconstruction].

Reconstruction of missing thumbs or fingers remains one of the most demanding challenges in hand surgery. Over the past century, toe-to-hand transplantation has evolved from early experimental pedicled transfers into a highly refined microsurgical procedure, now widely regarded as the "gold standard" for restoring hand function. This article reviews the historical and technical development of toe transplantation-from Nicoladoni's pioneering concepts in the late 19th century, through Dr. YANG Dongyue's landmark second-toe transfer performed in 1966, to contemporary Chinese contributions such as

Reconstruction of a chronic extensor digitorum communis tendon rupture using a bridge tendon graft.

There were no reports of chronic extensor digitorum communis (EDC) III tendon rupture in zone 5. We report a case of chronic EDC III tendon rupture in zone 5 reconstructed with a bridge tendon graft using palmaris longus tendon. End-to-end tendon repair seemed difficult for chronic EDC tendon rupture, and it was feared that direct repair would result in residual extension contracture of the finger. A 47-year-old right-handed male underwent a bridge tendon graft using the palmaris longus tendon for a chronic EDC III tendon rupture in zone 5 six weeks after the injury. Six months after the surge

What is the ideal age for surgery in single-suture craniosynostosis?

The optimal timing for surgical intervention in single-suture craniosynostosis (SSC) remains debated, despite advances in minimally invasive and open reconstructive approaches. This work integrates Children's National Medical Center (CNMC) institutional data with current literature to clarify age-related outcomes and guide timing recommendations. Published series from CMNC were analyzed and outcomes included perioperative morbidity, morphometric correction, intracranial pressure, and revision rates. Findings were contextualized with recent systematic reviews and multicenter data. Early endosco

Use of Free Flaps in the Reconstruction of the Ventral Skull Base: A Systematic Literature Review.

BACKGROUND: Reconstruction of the ventral skull base represents a challenge in head and neck surgery. When local or regional flaps are insufficient, microvascular free flaps offer a reliable reconstructive option. METHODS: A systematic literature review was conducted following PRISMA guidelines to identify studies on ventral skull base reconstruction using microvascular free flaps. Eligible studies included cases with an explicit description of the flap type, recipient vessels, and transposition corridor. The authors propose a categorization of the surgical corridors used for pedicle transpos
