A comprehensive review of the normal, abnormal, and post-operative MRI appearance of the distal biceps brachii.

The biceps brachii myotendinous unit is a common source of shoulder, arm, and elbow pain. Its complex anatomy can present a challenge when interpreting MR images. We discuss the clinical and imaging presentations of injury related to the proximal biceps brachii separately in another manuscript. The purpose of this manuscript is to review the distal biceps anatomy along with pathology and post-operative appearance as seen on MRI.

Clinical relevance of the anatomy of the long head bicipital groove, an evidence-based review.

Pathology in the bicipital groove can be a source of anterior shoulder pain. Many studies have compared treatment techniques for the long head biceps tendon (LHBT) without showing any clinically significant differences. As the LHBT is closely related to the bicipital groove, anatomical aspects of this groove could also be implicated in surgical outcomes. The aim of this review is to contribute to developing the optimal surgical treatment of LHBT pathology based on clinically relevant aspects of the bicipital groove. Medline/PubMed was systematically searched using key words "bicipital" and "gr

Jumper's knee mechanical consequences in professional basketball players: the "Camel's Back curve".

PURPOSE: Jumper's knee is characterized by an anterior knee pain during tendon palpation and can be classified in overuse pathologies, secondary to repetitive jumps. The prevalence is high in professional basketball players. It is responsible for an alteration of the motor control inducing a strength deficit of the quadriceps. We aimed to describe an isokinetic curve anomaly, a double-humped curve called "Camel's Back curve", consequence of a jumper's knee history. METHODS: 170 Professional basketball players were enrolled (24.8 ± 4.6 years; 91.8 ± 12.0 kg, 194 ± 9.0 cm). All players performe

Delayed Rupture of the Flexor Pollicis Longus After Volar Plating for a Distal Radius Fracture: A Case Report.

Distal radius fractures account for 18 percent of all fractures in the elderly age group. It is estimated that the yearly cost of treatment for distal radius fractures approaches $240 million. The frequency of fractures will continue to increase with the aging population. The operative treatment of distal radius fractures has changed dramatically with the advent of the fixed-angle volar plate. Volar plating allows stable internal fixation which permits early return of function. A common and serious complication of volar plating of distal radius fractures is rupture of the flexor pollicis longu

Quadriceps tendon autograft for arthroscopic knee ligament reconstruction: use it now, use it often.

Traditional bone-patellar tendon-bone and hamstring tendon ACL grafts are not without limitations. A growing body of anatomic, biomechanical and clinical data has demonstrated the utility of quadriceps tendon autograft in arthroscopic knee ligament reconstruction. The quadriceps tendon autograft provides a robust volume of tissue that can be reliably harvested, mitigating the likelihood of variably sized grafts and obviating the necessity of allograft augmentation. Modern, minimally invasive harvest techniques offer the advantages of low rates of donor site morbidity and residual extensor mech

Arthroscopic Treatment of Ankle Instability - Allograft/Autograft Reconstruction.

Inversion ankle sprains represent one of the most common traumatic injuries in the active sports population. Although most respond well to conservative treatment, some hide important lesions. Lateral ankle ligament injuries occur in more than 80% of all ankle sprains, with one-third of these developing chronic ankle instability (CAI). Lateral ankle ligament repair or reconstruction procedures aim to restore normal ankle anatomy and function in patients with CAI. Arthroscopic reconstruction techniques allow the surgeon to reach surgery objectives with minimal soft tissue injury. When the indica

Peroneal perforator-based peroneus longus tendon and sural neurofasciocutaneous composite flap transfer for a large soft-tissue defect of the forearm: A case report.

We describe the use of a composite flap composed of a sural neurofasciocutaneous flap and a vascularized peroneus longus tendon for the reconstruction of severe composite forearm tissue defects in a patient. A 43-year-old man had his left arm caught in a conveyor belt resulting in a large soft-tissue defect of 18 × 11 cm over the dorsum forearm. The extensor carpi radialis, superficial radial nerve, and radial artery were severely damaged. A free neurofasciocutaneous composite flap measuring 16 × 11 cm was outlined on the patient's left lower leg to allow simultaneous skin, tendon, nerve, and

Snapping knee caused by the thickening of the medial hamstrings.

We report a case of symptomatic subluxation of the semitendinosus and gracilis over the medial condyle of the tibia caused by the thickening of its tendons. Snapping was reproduced on active extension. Clinical examination and, above all, dynamic ultrasound were the key for the diagnosis because other imaging tests were normal. Due to failure of conservative treatment with physiotherapy and infiltrations, surgery was undertaken, involving desinsertion and excision of distal 8cm segment of the semitendinosus and gracilis tendons. At the present time (6 months postoperatively), the patient is sy

Irreducible ankle fracture-dislocation due to tibialis anterior subluxation: a case report.

Irreducible ankle fracture-dislocations are rare. Several cases of irreducible ankle fracture-dislocation have been reported in published studies secondary to the tibialis posterior tendon, deltoid ligament, or extensor digitorum longus tendon blocking the reduction. We report a case of irreducible ankle fracture-dislocation resulting from posteromedial subluxation of the tibialis anterior tendon around a medial malleolar fracture fragment. Ultimately, the ankle required open reduction of the incarcerated tendon to reduce the joint and proceed with internal fixation of the associated fracture.

Prophylactic decompression of extensor pollicis longus to prevent rupture.

We present a case of a patient with spontaneous rupture of right extensor pollicis longus (EPL) tendon, who had also developed left wrist pain and weakness in his left EPL that MRI studies confirmed to be caused by tendinosis. Subsequently, decompression of left EPL and reconstruction of right EPL with palmaris longus tendon graft was undertaken. In this case, decompression of the left EPL tendon led to resolution of the patient's symptoms as well as preventing tendon rupture. We advocate the use of ultrasound imaging to evaluate EPL in these cases and prophylactic decompression of EPL tendon

[Treatment of extensor mechanism rupture after total knee arthroplasty].

Disruption of the extensor mechanism is one of the most devastating complications in knee arthroplasty with a reported incidence between 0.17 and 2.5 %. Due to a high rate of subsequent complications and poor clinical results, every effort should be made to avoid extensor mechanism disruption. In cases of disruption however, the orthopaedic surgeons must be aware of non-operative and surgical treatment options and their indications, timing, outcome and limitations. Non-operative treatment is feasible in cases of incomplete disruption of the quadriceps tendon with an extension deficit of less t

Symptomatic snapping knee from biceps femoris tendon subluxation: an unusual case of lateral pain in a marathon runner.

Snapping biceps femoris syndrome is an uncommon cause of lateral knee pain and may be difficult to diagnose, resulting in unsuccessful surgical intervention. In this report, we present an unusual case of a 37-year-old male marathon runner with unilateral snapping knee secondary to dislocation of the long head of the biceps femoris over the fibular head during knee flexion. The pain was great enough to interfere with his ability to practice sport. Possible causes of symptomatic snapping knee include multiple intra-articular or extra-articular pathology. Biceps femoris snapping over the fibular
