Mechanisms of traumatic cardiac arrest in Kuwait: A retrospective study.

The objective of this study is to describe the epidemiology and causes of traumatic cardiac arrest (TCA) in Kuwait aiming to provide a preliminary background to update the current guidelines and improve patients' management. This is a retrospective analysis of TCA cases retrieved from emergency medical services archived data between 1 January and 31 December 2017. The TCA cases were sub-grouped based on mechanism of injury then compared in terms of patient demographics, vital signs, patterns of injuries, resuscitation practices, and outcomes.Outcomes; On scene mortality rate and pre-hospital re

Limiting thoracic CT: a rule for use during initial pediatric trauma evaluation.

BACKGROUND: Despite increases in imaging guidelines for other body-regions during initial trauma assessment and the demonstrated utility of chest radiographs (CXR), guidelines for use of thoracic computed-tomography (TCT) are lacking. We hypothesized that TCT utilization had not decreased relative to other protocolized CTs, and mechanism and CXR could together predict significant injury independent of TCT. METHODS: We performed a retrospective review of blunt trauma patients ≤18 y.o. (2007-2015) at two level-1 trauma centers who received chest imaging. Baseline characteristics and incidences

[Degloving injury of the thorax and abdomen].

A 22-year-old man was caught between a truck and a wagon. His chest was torn apart from the sternum downward; both lungs and the heart were visible. The skin and mucosa were covered with oil from the truck. He was immediately treated with intubation, massive fluid infusion and--after stabilisation--surgery. Afterwards, two plastic surgeries were performed. On the 27th day he was dismissed from hospital. Except for a cicatrice on the upper chest, he is in his usual condition. He survived this massive injury because of immediate intubation at the scene of the accident. Fortunately, none of the i

Radiographic sensitivity and negative predictive value for acute canine spinal trauma.

The objectives of this study were to establish the sensitivity and negative predictive value of radiography for acute spinal osseous lesions in the canine trauma patient, and to evaluate the interobserver variability in radiographic assessment of the spine in traumatized dogs. This was a retrospective multiple observer blinded study. The study population included 30 canine patients that presented following acute trauma, with clinical signs attributable to the spinal column. Radiography and computed tomography (CT) were performed in all cases. Radiographic interpretation was performed independe

[The relationship of the nitric oxide level with angiotensin-converting enzyme activity in the chest injury].

We have studied NO production, ACE activity and their correlation in pleural fluid of patients with and without lung wound, in the blood serum of the wounded and blood donors. Chest wound was associated with a significant elevation of NO levels in all study groups versus controls. The greatest increase of ACE activity was observed in pleural fluid of patients with a lung wound. There was a negative correlation between NO and ACE in pleural fluid of patients with a lung wound. In all the other groups, a positive correlation between NO and ACE was revealed. ACE overactivity in the pleural fluid

The impact of obesity on the outcomes of 1,153 critically injured blunt trauma patients.

BACKGROUND: Several small series have had mixed conclusions regarding the impact of obesity on outcomes of trauma patients. The purpose of the present study was to evaluate a large cohort of critically injured patients to better understand the influence of obesity on the outcomes of patients after severe blunt trauma. METHODS: Retrospective review using the trauma registry and intensive care unit (ICU) database of all blunt trauma patients admitted to the ICU at our urban, Level I trauma center over a 5-year period (1998-2003). Obese patients (body mass index [BMI] > or = 30 kg/m) were compar

Late cardiac arrhythmias after blunt chest trauma.

OBJECTIVE: Case reports of two patients who developed fatal cardiac arrhythmias several days after blunt chest trauma. DESIGN: Case reports. SETTING: Surgical intensive care unit of a university hospital. PATIENTS: A 23-year-old man and a 9-year-old girl with blunt chest trauma and multiple further injuries following car crashes were transferred to our institution. Although ECG on admission was normal, both patients developed fatal cardiac arrhythmias after 6 and 4 days, respectively. In both patients, post-mortem analysis confirmed myocardial contusion without coronary artery lesions. Hist

Defining GERD.

"It is not the death of GERD that I seek, but that it turns from its evil ways and follows the path of righteousness." The reflux world is fully aware of what GERD is and what GERD does. What the world does not know, however, is the answer to the most important yet least asked question surrounding GERD's raison-d'etre: Why is GERD here and why do we have it? What GERD is: abnormal gastric reflux into the esophagus that causes any type of mischief. What GERD does: causes discomfort and/or pain with or without destroying the mucosa; causes stricture or stenosis, preventing food from being swallo

[Blunt thoracic trauma with hemorrhage into a mediastinal lymphangioma--case report].

We describe a young snowboarder suffering from dyspnea, chest pain and dysphagia following a blunt chest injury. These symptoms were caused by an accident-related hemorrhage within a preexisting lymphangioma of the right posterior mediastinum. At the same time a lymphangioma of the left anterior mediastinum was found. The lymphangioma of the posterior mediastinum was resected through a right posterolateral thoracotomy after thoracoscopy had demonstrated that a complete resection could not be performed on this way. The lymphangioma of the left side was resected 4 month later by a left-sided hem

Pulmonary complications of trauma.

Pulmonary complications can be the result of direct chest trauma or can occur from indirect trauma outside of the thorax. Understanding the mechanism of pulmonary function in determining intrapulmonary shunt and physiologic deadspace can assist the clinician in assessing the severity and monitoring the progression of pulmonary injury in patients. This article reviews assessment parameters, physiology, and treatment of direct and indirect pulmonary trauma.

Cardiac injury complicating traumatic asphyxia.

During a 3-year period (1986-1989), 8 patients were seen at St. Louis University Medical Center exhibiting the stigmata of traumatic asphyxia. Fewer than 200 cases of traumatic asphyxia have been reported and there is only a single report of a cardiac injury. In this series, 3 of 8 (37.5%) patients were found to have an injury to the heart: two cardiac contusions and a ventricular rupture. Five patients were crushed in motor vehicle collisions, one by an elevator counterweight, and two patients by river barges. Injuries associated with these patients include pulmonary contusion, hemopneumothor

Cardiovascular evaluation in blunt thoracic trauma using transesophageal echocardiography (TEE).

Blunt chest trauma can result in significant cardiothoracic injury, which can include cardiac contusion, aortic injury, and myocardial valvular injury. Nineteen patients with no prior history of cardiac abnormalities who sustained severe blunt chest trauma and had widening of the mediastinum on chest radiographs were prospectively evaluated using transesophageal echocardiography (TEE). In each instance TEE was performed without difficulty, excellent images were obtained of the aorta and heart, and no complications were noted. Abnormalities were seen in 12 (63%) patients, with hypokinetic regio

Pulmonary contusion causes long-term respiratory dysfunction with decreased functional residual capacity.

To elucidate the mechanism of persistent dyspnea after blunt chest trauma, we prospectively studied the pulmonary function of 18 patients with blunt chest trauma for 6 months. Nine of the patients had flail chest and 12 had pulmonary contusion (PC). Pulmonary function was evaluated using spirometry, arterial blood gas analysis, chest x-ray studies and CT scans. Functional residual capacity (FRC) remained significantly reduced throughout the 6 months in patients with PC. Such patients experienced a fall in Pao2 when changed from a sitting position to a supine position and they had fibrous chang

Blunt and penetrating diaphragmatic injuries with or without herniation of organs into the chest.

From 1970 to 1984, 189 patients with penetrating injury and 20 with blunt injury were treated at Grady Memorial Hospital. One hundred eight-five patients with penetrating injury (Group 1) and 9 with blunt injury (Group 2) required emergency laparotomy. In the remaining 15 patients (Group 3), the diagnosis of diaphragmatic injury was delayed from 18 hours to 15 years (mean, 8 months) after injury. The vast majority of the Group 1 and all Group 2 patients had injury to other organs, and the diagnosis of the diaphragmatic injury was made in almost all of them during the emergency laparotomy. The

Subclavian vein catheterization.

Massive fluid replacement is occasionally useful for resuscitation of severely injured patients. The use of MAST garment and the inability to obtain peripheral intravenous access may necessitate the use of infraclavicular subclavian cannulation. Two modifications of the standard subclavian technique have been described which enable quick access for infusion of large fluid volume and autotransfusion. The first involves the placement of multiple catheters in the same vein. The second method utilizes a modified Seldinger technique to place an 8 F. Swan-Ganz introducer into the subclavian vein. Co
