Computerized tomography of the foramen magnum: achondroplastic values compared to normal standards.

Computerized tomographic dimensions of the foramen magnum of 63 achondroplastic individuals were compared to standards established for nonachondroplastic individuals. The size of the foramen magnum in patients with achondroplasia was small at all ages, particularly in those with serious neurologic problems. The data suggest that measurement of the foramen magnum may identify achondroplastic individuals at high risk of developing neurologic complications.

[Diagnostic potentials of NMR tomography of the kidneys of patients with symptomatic renal hypertension].

Thirty seven patients with arterial hypertension of renal genesis (chronic diffuse glomerulonephritis, chronic pyelonephritis, renal cystic disease and congenital abnormalities) were subjected to NMR-tomography. The comparison group comprised 12 patients with essential hypertension and 18 normal individuals constituted the control group. The examination was effected in the axial, frontal and sagittal planes using the BMT-1100 NMR-tomograph (Brucker, FRG) with the magnetic intensity of 0.235 T, the coil diameter of 60 cm, and the working frequency of 9.95 MHz. The technique made it possible to …

Perforation of the intra-articular disc diagnosed by arthro-tomography of the temporomandibular joint.

Arthrography was first used almost 40 years ago for evaluation of the non-osseous parts of the TMJ. It did not find wide application and only during the last decade have certain authors re-introduced its use as a diagnostic approach in TMJ pathology. Among the other pathological conditions which can be diagnosed, is a perforation of the intra-articular disc. Four such cases are presented and the value of this method is shown.

[Technic and results of spinal computed tomography in the diagnosis of cervical disk disease].

We give a description of a technique of the patient's positioning with traction of the arms during the cervical spinal computed tomography which allows to draw the shoulders downwards by about one to three cervical segments. By this method the quality of the images can be improved in 96% in the cervical segment 6/7 and in 81% in the cervical/thoracal segment 7/1 to such a degree that a reliable judgement of the soft parts in the spinal canal becomes possible. The diagnostic reliability of the computed tomography of the cervical disc herniation is thus improved so that the necessity of a myelog…

[Contribution of computer tomography in recent traumatology of the spine. Apropos of 31 cases].

The authors report 31 cases of recent spinal injury (14 cervical, 17 thoraco-lumbar) which have been investigated using computerized tomography. After a critical appreciation of the conventional approach to such lesions, the authors consider the advantages and limitations of computerized tomography. Analysis of the data from 31 cases studies confirms that this technique has considerable applications in the assessment of corporeo-discal and pediculo-lamar lesions and, in particular in the determination of the degree of spinal canal stenosis and in the detection of actual potential neuroaggressi…

Computed tomography of the sacral plexus and sciatic nerve in the greater sciatic foramen.

The sacral plexus forms the sciatic nerve, which leaves the pelvis through the greater sciatic foramen. The anatomic boundaries of the greater sciatic foramen and the relation of the sacral plexus and sciatic nerve to the structures within are identified and described on axial computed tomography (CT). The piriform muscle, which passes through the center of the greater sciatic foramen, is a recognizable landmark that is extremely helpful in locating the sacral plexus and sciatic nerve on CT. The pelvic CT images of 25 patients studied for unrelated reasons and two patients studied for complain…

Screening for diffuse and focal liver disease: the case for hepatic computed tomography.

I believe that CT should usually be the first choice for examining patients with suspected liver disease. Precontrast scans may suffice when results are clearly abnormal, but when contrast is given it should be in a high dose, bolus injection with rapid sequential scanning and table incrementation. This opinion is based upon: (1) the greater sensitivity and specificity of contrast enhanced CT compared to sonography and radionuclide scanning in focal liver lesions; (2) the ability of CT to detect diffuse liver abnormalities; (3) the greater ease in standardizing the CT examination and interpret

Liver-spleen studies with the rotating gamma camera. II: Utility of tomography.

A prospective comparison of conventional scintigrams and single-photon emission computed tomography (SPECT) studies of the liver and spleen was carried out. Care was taken to insure high-quality, artifact-free studies. Two hundred patients were imaged by both methods. A definitive diagnosis of normal was established in 86 cases, abnormal in 107 cases, and seven cases were indeterminate. All studies were read by two observers and graded as to degree and type of abnormality seen. ROC curves were constructed and errors of interpretation studied to identify problems. One observer showed no differe…

Computed tomography in suppurative ear disease: a correlation of surgical and radiographic findings.

Forty-two patients with chronic otitis media underwent preoperative CT scanning followed by surgical exploration of the middle ear and mastoid. The CT finding of abnormal soft tissue density associated with bone erosion was highly correlated with the surgical finding of cholesteatoma. By contrast, the total absence of abnormal soft tissue on CT essentially excluded cholesteatoma. However, 50% of all patients had abnormal soft tissue on CT scan not accompanied by bone erosion. In this largest group of patients it was not possible to diagnose or exclude cholesteatoma on the basis of CT findings

Doppler echocardiography and computed tomography in diagnosis of left coronary arteriovenous fistula.

A 37 year old man with recurrent episodes of endocarditis was found to have a large left coronary arteriovenous fistula communicating with the right atrium. The origin and termination of the fistula were identified using computed tomography and two-dimensional Doppler echocardiography. Coronary angiography confirmed the diagnosis and the patient underwent a successful operation.

A comparison of computed tomography, ultrasound and scintigraphy in the diagnosis of alcoholic liver disease.

In 35 alcoholics with histologically proven liver disease, computed tomography (CT), grey scale ultrasonography and liver scintigraphy were evaluated for their abilities to detect an abnormal liver and to identify the patients with cirrhosis. Abnormal studies were present on CT in 83% of patients, in 64% on ultrasound and in 94% on scintigraphy. In 10 control patients specificity was 90% by CT, 100% by ultrasound and 70% by scintigraphy. CT and ultrasound were poor in identifying the alcoholics with cirrhosis. Scintigraphy suggested cirrhosis in all but one of the patients with this diagnosis.…

[Effect of computer tomography on the diagnostic findings and therapy planning in diseases of the epigastrium].

Computed tomography is a reliable and non-invasive, however quite expensive diagnostic method. Its use as a screening procedure should be avoided until conventional X-ray and endoscopic examinations are exhausted, as tumours in the gastrointestinal tract might be overlooked and, on the other hand, radiation exposure should not be underestimated. The specific application of CT in finding a diagnosis and planning a therapy of upper abdominal diseases, especially concerning pancreas and malignant tumours, is of considerable importance in spite of occasional errors. In any case a critical point of…

Renal dysplasia with a single vaginal ectopic ureter: the role of computerized tomography.

Renal dysplasia with a single vaginal ectopic ureter is relatively common in Japan. We report 2 cases in which delayed enhanced computerized tomography (1.5 hours later) contributed in detecting a dysplastic kidney that had been nonfunctioning on excretory urography. We believe that computerized tomography is a useful technique for the diagnosis of renal dysplasia with a single vaginal ectopic ureter.

Metrizamide myelography and postmyelographic computed tomography: comparative adequacy in the cervical spine.

Metrizamide myelography and postmyelographic computed tomography (CT) were evaluated for relative efficacy when correlated with operative findings in a series of 30 patients. Fifty-seven levels were operated on in the 30 patients with 27 patients diagnosed as having diffuse cervical stenosis, hyperostotic spondylosis, or herniated nucleus pulposus. Metrizamide myelography and CT metrizamide myelography were equally useful in providing preoperative diagnostic information at 44 of 57 levels. Both radiographic techniques agreed with the degree of canal and neural forman stenosis found operatively…

[Pelvic lipomatosis: value of x-ray computed tomography. Apropos of a case].

Pelvic lipomatosis is characterised by an excessive amount of adipose tissue in the pelvis. Although there are usually few clinical symptoms, the radiological changes are characteristic: pear-shaped bladder, straightened and displaced rectosigmoid junction. However, these changes are not specific and some workers advise histological confirmation with or without surgical exploration. The authors describe a case illustrating the diagnostic value of computerised axial tomography, as has previously been reported. This investigation seems to give reliable results in the diagnosis of pelvic tumours,…

[Contribution of computed tomography to the radiological clarification of a chondroma of the cervicothoracic spinal column].

On the basis of a chondroma which, as is well known, is very rarely found in the cervicothoracic spinal canal, the contribution made by computed tomography to the radiological identification of such benign tumours in explained. Morphology, localisation and incidence of chondromas and their differentiation against other tumours localised in the vertebral column, are discussed. The possibilities offered by the available diagnostic tools are compared with each other, also with regard to indication of operation and surgical planning.
