[Behçet's disease].

Clinicopathologically, Behçet's disease is characterized by neutrophil and platlet hyperfunction. Lesions are formed presumably because neut … ve helped to achieve remarkable progress of treatment. Particularly notable is the effect on severe uveitis of cyclosporine, which is more effective than colchicine and conventional immunosuppressants. Low dose weekly methotrexate therapy might have a beneficial effect in the treatment of patients with neuro-Behçet's disease.

[Tropheryma whippelii, an emerging intracellular pathogen causing Whipple disease].

Whipple's disease is a diffuse disease with a wide variety of clinical manifestations. Differential diagnoses include chronic enteritis, chronic joint disease, chronic meningoencephalitis and prolonged fever with weight loss, chronic uveitis and endocarditis with negative blood culture. MOLECULAR DIAGNOSIS: Histologically the characteristic lesions contain PAS-positive inclusions. PCR of RNAr gene 16S in fresh tissue identifies Tropheryma whippelii and confirms bacteriological diagnosis. AN EMERGING PATHOGEN: Tropheryma whippelii is a recently identified pathogen highly difficult to culture. S…

[Magnetic resonance imaging of brainstem lesions in a case of neuro-Behçet disease].

We reported a 28-year-old man with multiple lesions in the brainstem which were clearly identified and followed up with MRI. He had been treated for Behçet uveitis using cyclosporine and colchicine during the past 5 years, and he acutely presented with fever and headache, followed left hemiparesis, dysarthria and dysphagia. MRI on admission revealed multiple lesions in the ventral pontine region which showed mixed-intensity on T1WI, high-intensity on T2WI and enhancement with Gd-DTPA. His clinical symptoms spontaneously and gradually improved, but more rapidly improved due to additional admini…

Chlamydial associated syndrome of arthritis and eye involvement in young children.

The current classification of juvenile rheumatoid arthritis (JRA) consists of several distinct subsets. We describe 6 children (2 boys, 4 girls, mean age 3.7 years, range 2.0-4.9 years) with arthritis and eye involvement associated with infection with Chlamydia trachomatis. In some of the children, the clinical picture was similar to early onset pauciarticular JRA: onset within the first 4 years of life, predominance of girls, pauciarticular arthritis, subacute uveitis, and presence of antinuclear antibodies. Joint involvement was pauciarticular in 4 patients and polyarticular in 2. Two patien…

[Early ocular symptoms in Wegener's granulomatosis].

Wegener's granulomatosis is characterized by necrotizing granulomatous vasculitis that initially occurs in a localized form, disseminates in various degrees and particularly involves the respiratory tract and kidneys. Eye manifestations are common (40 to 60%). Both clinical and histopathological findings are described in eight patients with Wegener's granulomatosis. The initial ocular symptoms included conjunctivitis, episcleritis, scleritis, corneoscleral ulceration, uveitis and ocular proptosis. In one patient histological examination of the orbital tissue was consistent with limited Wegener…

Vascular anastomoses in ocular toxoplasmosis.

Toxoplasmosis is a frequent cause of uveitis seen in clinical practice and fundus scars typical of ocular toxoplasmosis are common. It has been reported that vascular anastomoses between the retinal and choroidal circulation can occur through the damaged Bruch's membrane in fundus scars resulting from ocular toxoplasmosis. Although it has been stated that these vascular anastomoses are a rare occurrence, it has also been suggested that they are relatively common. In order to determine the prevalence rate of patients with vascular anastomoses in toxoplasmic fundus scars, 3,850 consecutive optom…

T cell traffic and the inflammatory response in experimental autoimmune uveoretinitis.

PURPOSE: To quantify S-antigen-specific (S-Ag) T cells in the retina after adoptive transfer, and to evaluate their role in the initiation and progress of destructive ocular inflammation in experimental autoimmune uveoretinitis (EAU). METHODS: Lewis rats were administered 10 x 10(6) S-Ag-specific T cells from the SP35 cell line or 10 x 10(6) concanavalin A-stimulated syngeneic spleen cell lymphoblasts labeled with lipophilic PKH26 fluorescent dye immediately before intravenous inoculation. Labeled cells in each retina were counted at various times from 4 to 120 hours after cell transfer by fl

Type I interferon signature does not correlate with disease activity in Blau syndrome.

Blau syndrome is a rare autoinflammatory disorder caused by NOD2 mutations, characterized by granulomatous arthritis, uveitis, and dermatitis. While type I interferon signatures are biomarkers in several autoinflammatory diseases, their role in Blau syndrome remains unclear. We assessed the interferon score in 11 patients with Blau syndrome and correlated it with disease activity. Our results demonstrate that type I interferon signatures were detected in only a minority of patients and showed no significant correlation with clinical disease activity, inflammatory markers, or treatment response…

The sequential therapy for non-infectious scleritis: A case report.

Scleritis results from an inflammatory response in the sclera and can be associated with a variety of infectious and non-infectious ocular and systemic diseases. It can cause vision loss, uveitis, high intraocular pressure and other complications, resulting in poor prognosis. There are no reports of non-infectious scleritis in children. Herein, we describe a case of sequential tocilizumab treatment on tofacitinib in a child with non-infectious scleritis. Extensive examinations ruled out the cause of infection, and the previous conventional glucocorticoid treatment was effective, but the diseas…

Behçet's Disease with Extremely High Levels of Urinary β2-Microglobulin after Non-Steroidal Anti-Inflammatory Drug Treatment.

Behçet's disease is an inflammatory disease which manifests itself as various symptoms, such as uveitis, oral and genital aphthae, erythema nodosa, gastro-intestinal ulcerations and encephalopathy. Among the manifestations, renal dysfunction is reported in some percentage of the patients with this disorder. We experienced a middle-aged male with Behçet's disease who showed an extremely high level of urinary β2-microglulin, which is one of the markers of renal dysfunction, despite normal serum creatinine levels. The patient was on non-steroidal anti-inflammatory drug (NSAID) therapy for 7 weeks…
