Talar impingement by the anteroinferior tibiofibular ligament. A cause of chronic pain in the ankle after inversion sprain.

Seven patients who had pain in the anterior aspect of the ankle were found to have a thickened distal fascicle of the anteroinferior tibiofibular ligament. Each patient had a history of an inversion sprain of the ankle followed by chronic pain in the anterior aspect of the ankle. The thickened distal fascicle was resected without loss of stability of the ankle. Five patients needed débridement of an area of abraded hyaline cartilage on the anterolateral aspect of the talus. Six patients were followed for a mean of thirty-nine months (range, twenty-four to fifty-nine months). Four of them had n…

Gait comparison of subjects with hemiplegia walking unbraced, with ankle-foot orthosis, and with Air-Stirrup brace.

The effects of the Air-Stirrup (AS) standard ankle brace on the gait of 19 subjects with hemiplegia resulting from a cerebrovascular accident who exhibited excessive subtalar joint motion were studied. Videotaped trials and footprint analyses were used to measure subjects' hip, knee, and ankle sagittal plane angles; inversion and eversion of the calcaneus; and time-distance gait characteristics. A one-way analysis of variance for repeated measures was used to compare the gait of 19 subjects with the AS brace and unbraced and 11 subjects with the AS brace, unbraced, and with an ankle-foot ortho…

Effect of ankle-foot orthosis (AFO) on body sway and walking capacity of hemiparetic stroke patients.

Body sway, the total length of the sway of the center of foot pressure (CFP) and maximum walking speed were examined with and without AFO in … improvements in body sway and walking capacity. The AFO compensated only for the instability of the ankle joint but not for the dysfunction of the central nervous system after the stroke.

Diabetic neuro-osteoarthropathy. Rehabilitation of a patient with both ankle joints involved and associated skin problems.

Neuro-osteoarthropathy in diabetes mellitus is not rare. The case of a 53-year-old diabetic with involvement of both ankle joints and multiple associated skin problems complicating rehabilitation management is discussed. By applying a team approach, closely monitoring the patient's skin and joint problems, and treating them at an early stage by adaptive shoes and orthoses, we found that function could be sustained at an optimal level for a prolonged period. In addition to prevention of ulceration, modifying the weight bearing or temporarily non-weight bearing is the treatment of choice. Amputa…

Valgus deformity of the ankle in myelodysplastic patients. Correction by stapling of the medial part of the distal tibial physis.

Thirteen patients with myelodysplasia and associated valgus deformities of the ankle had stapling of the medial part of the distal tibial physis. The pattern of neural involvement in these patients was responsible for characteristic muscle imbalances across the tibiotalar and subtalar joints which led to the valgus deformities. Stapling was performed in twenty-five ankles, fifteen of which had previously been treated unsuccessfully with various soft-tissue and bone procedures. Twelve of the patients were followed until closure of the physis. The average preoperative valgus deformity at the tib…

A study of anatomy and biomechanics of the ankle region in normal and club feet (talipes equino varus) of infants.

Three normal feet and three feet exhibiting talipes equino varus from infants were dissected and their anatomy studied and compared. The tibiae were immobilised in plaster and held in a bench clamp while the range of dorsiflexion (extension) was studied before and after immobilisation of either the medial or lateral, or of both groups of tendons. Movements at the subtalar and ankle joints were studied as well as factors controlling movements at the subtalar joint. There was minimal subtalar movement in normal feet during dorsiflexion (extention) but for the clubbed feet the joint showed far gr…

Restraining forces in various designs of knee ankle orthoses: their placement and effect on the anatomical knee joint.

A biochemical evaluation was conducted on double upright knee ankle orthoses, which were instrumented with strain gauge transducers to determine the magnitudes of the restraining forces exerted on the leg. Measurements were made on six commonly used designs of orthoses worn by spinal cord injured persons ambulating with a swing-through gait. The measurements were used to determine distribution of forces on the limb as well as their effect on anatomical knee shear. Based on the experimental data, the following basic principles of optimal orthosis design were identified: The forces required to s…

An x-ray study of valgus ankles in spina bifida children with valgus flat foot deformity.

Radiological examination of the ankles of spina bifida children with flail lower limbs and valgus flat foot deformity has revealed a high incidence of abnormal valgus tilt of the ankle-joint. A study of ankle X-rays of spina bifida children before and after extra-articular subtalar fusion in which the bone graft is removed from the ipsilateral tibia has shown that the valgus tilt of the mortice may increase after this procedure.

Prediction of severe aortic stenosis using peripheral arterial waveforms during brachial-ankle pulse wave velocity testing.

With the increasing use of minimally invasive catheter-based therapies, early identification of severe aortic stenosis (AS) has become essential. The slow upstroke waveform (Tardus) observed in carotid artery wave analysis is a known feature of AS, and quantitative assessments of Tardus can estimate the severity of AS. Prolongation of ejection time (ET) and upstroke time (UT) is a characteristic of Tardus. However, the clinical relevance of ET and UT measured during brachial-ankle pulse wave velocity (baPWV) assessment remained unclear. This study aimed to develop a predictive model for screen…

From Sprain to End-Stage Osteoarthritis: Why Earlier Intervention in Ankle Cartilage Damage Deserves More Attention.

➢ Ankle cartilage damage ranges from osteochondral lesions of the talus to end-stage osteoarthritis. End-stage ankle osteoarthritis has been associated with a health burden comparable with that of major chronic diseases such as congestive heart failure and kidney disease, while predominantly affecting young and active patients.➢ Ankle osteoarthritis often has a posttraumatic nature, making it possible to study the long-term results of traumatic articular injuries and identify potential targets for early therapeutic interventions.➢ The scientific literature has been predominantly focused on out…

Gait, ankle mobility, muscle strength, and functional performance in chronic venous insufficiency: A CEAP-based analysis.

ObjectivesChronic venous disease (CVD) is a progressive condition affecting the lower limb venous system, leading to venous hypertension, muscle dysfunction, and gait alterations. Patients with CVD often experience muscle weakness, reduced ankle mobility, and impaired gait biomechanics. However, the impact of CVD severity on spatiotemporal gait parameters and plantar flexor strength remains little explored. The present study aimed to compare gait biomechanics, plantar flexor strength, dorsiflexion ankle range of motion, and functional performance among patients with mild, moderate, and severe …

An Unpowered Exoskeleton Enabling Cross-Limb Energy Transfer From Arm Swing to Assist Contralateral Ankle Plantarflexion.

Unpowered exoskeletons commonly assist walking by storing and releasing elastic energy, with existing designs largely limited to energy transfer within the lower limbs. However, arm swing, an integral coupled component of human gait, remains underexplored as a mechanical energy source for locomotor assistance. Motivated by gait biomechanics, we propose a cross-limb energy transfer concept that redirects positive mechanical work from unilateral arm swing to the contralateral ankle. Based on this concept, we developed a novel unpowered exoskeleton consisting of a waist-mounted force-routing modu…

Relationship between ankle mobility and vertical jump performance across playing positions in professional female volleyball players.

The purpose of this study was to examine the associations between ankle range of motion (ROM), inter-limb ROM asymmetry (ASYM), and countermovement vertical jump (CMJ) force-time metrics across five playing positions in professional female volleyball players (n = 124). Following a standardized warm-up protocol, ankle ROM was assessed bilaterally during a weight-bearing front lunge using a digital goniometer. Then, each athlete performed three CMJs with no arm swing while standing on a uni-axial force plate system. Pearson product-moment correlation coefficients were used to measure the strengt…

Endoscopic excision for posterior ankle impingement secondary to chronic fragmented os trigonum fracture: a case report.

Chronic fragmented os trigonum fracture is an uncommon but clinically important cause of posterior ankle impingement that may be overlooked in active patients with persistent plantar-flexion-related posterior ankle pain. We report the case of a 31-year-old female physical education teacher with progressive right posterior ankle pain for over 1 year despite 3 months of conservative treatment. Radiographs and magnetic resonance imaging demonstrated a fragmented os trigonum with bone marrow edema and reactive inflammation. Posterior ankle endoscopy confirmed three osseous fragments, which were ex…
