A goal-striving life event and the onset of hypomanic and depressive episodes and symptoms: perspective from the behavioral approach system (BAS) dysregulation theory.

On the basis of the behavioral approach system (BAS) dysregulation theory of bipolar disorder, this study examined the relation between occurrence of a BAS activation-relevant life event--goal striving--and onset of hypomanic and depressive episodes and symptoms. In particular, the authors examined the relation between preparing for and completing final exams (a goal-striving event) and onset of bipolar spectrum episodes and symptoms in college students with bipolar II disorder or cyclothymia (i.e., "soft" bipolar spectrum conditions). One hundred fifty-nine individuals with either a bipolar s…

Elevated nucleus accumbens structural connectivity associated with proneness to hypomania: a reward hypersensitivity perspective.

The Reward Hypersensitivity Model of bipolar disorder argues that hypersensitivity to reward-relevant cues characterizes risk for hypo/mania. This hypersensitivity leads to increased goal-directed motivation during reward-relevant life events that, in the extreme, is reflected in hypo/manic symptoms. In line with this perspective, individuals with bipolar disorder display elevated activation in a cortico-striatal reward circuit including the nucleus accumbens (NAcc) and medial orbitofrontal cortex (mOFC). To date, however, research on reward-related neural circuitry underlying bipolar symptoms…

Transmembrane protein 108 inhibits the proliferation and myelination of oligodendrocyte lineage cells in the corpus callosum.

BACKGROUND: Abnormal white matter is a common neurobiological change in bipolar disorder, and dysregulation of myelination in oligodendrocytes (OLs) is the cause. Transmembrane protein 108 (Tmem108), as a susceptible gene of bipolar disorder, is expressed higher in OL lineage cells than any other lineage cells in the central nervous system. Moreover, Tmem108 mutant mice exhibit mania-like behaviors, belonging to one of the signs of bipolar disorder. However, it is unknown whether Tmem108 regulates the myelination of the OLs. RESULTS: Tmem108 expression in the corpus callosum decreased with th…

Irritability in the Mix: An Exploratory Study of the Role of Irritability in Characterising Mood Disorders.

OBJECTIVES: To examine the relationship between irritability and mixed states in bipolar disorder to determine how subjective irritability and mixed symptoms of mania and depression manifest in patients with bipolar disorder over a period of 18 months. METHODS: This exploratory analysis examined longitudinal data from a randomised controlled trial of a psychological intervention for patients with bipolar disorder. Scores of subjective irritability and mood morbidity were analysed from data sampled over ten timepoints over the trial from 82 patients. RESULTS: Significant associations were fou…

[A comparative study of voxel-based morphometry in patients with paranoid schizophrenia and bipolar mania].

OBJECTIVE: To assess volumetric abnormalities of grey matter in the brains of patients with paranoid schizophrenia and bipolar disorder (mania). METHODS: 3D T1 weighted images were acquired by magnetic resonance imaging from 20 patients with paranoid schizophrenia, 20 patients with bipolar disorder (mania) and 20 control subjects. Regional deviation in gray matter volume was assessed using optimized volumetric voxel-based morphometry. ANOVA was performed to test the difference of the gray matter volume (GMV). RESULTS: Compared with controls, the patients with paranoid schizophrenia showed de…

Alcoholism and anxiety in bipolar illness: differential lifetime anxiety comorbidity in bipolar I women with and without alcoholism.

INTRODUCTION: This study was undertaken to evaluate the prevalence rate of anxiety comorbidity in bipolar subjects with and without alcohol use disorders (AUD). METHODS: Bipolar men and women who entered the Stanley Foundation Bipolar Network (SFBN) underwent a Structured Clinical Interview for DSM-IV (SCID-IV) and were divided into those subjects meeting current or lifetime criteria for an alcohol use disorder (AUD=213) vs. those subjects who did not (non-AUD=137). Lifetime rates of comorbid anxiety disorder were evaluated between groups. RESULTS: Of 350 subjects, 163 (46.5%) met criteria f

Drug interactions of lithium and other antimanic/mood-stabilizing medications.

Treatments for bipolar disorder often consist of a combination of 2 or more medications that have mood-stabilizing properties. Lithium is the only agent that has shown efficacy for the treatment of acute mania and acute depression as well as for the prevention of recurrence of mania and depression in patients with bipolar disorder. Other agents frequently used as mood stabilizers in monotherapy and in combination with lithium are valproate and carbamazepine. Several other drugs that have mood-stabilizing properties are used in many combinations to effectively manage the spectrum of symptoms of…

The high prevalence of "soft" bipolar (II) features in atypical depression.

Seventy-two percent of 86 major depressive patients with atypical features as defined by the DSM-IV and evaluated systematically were found to meet our criteria for bipolar II and related "soft" bipolar disorders; nearly 60% had antecedent cyclothymic or hyperthymic temperaments. The family history for bipolar disorder validated these clinical findings. Even if we limit the diagnosis of bipolar II to the official DSM-IV threshold of 4 days of hypomania, 32.6% of atypical depressives in our sample would meet this conservative threshold, a rate that is three times higher than the estimates of bi…

[Inbreeding and serious mental illness in the first Spanish Bourbons].

INTRODUCTION: There is an argument about inbreeding as a risk factor in the development of major mental illnesses like bipolar disorder or schizophrenia. The genealogical trees of European royal dynasties represent an opportunity to study this relationship. Felipe V was the first Bourbon king to rule in Spain, who was thought to have possibly suffered a severe mental disorder. Felipe's son Fernando VI, also had a mental disease. Nevertheless Carlos III, Felipe V's son born from a different, unrelated mother, apparently showed no trace of such disorder. We hypothesize that consanguinity or inbr…

Validity of the parent young mania rating scale in a community mental health setting.

The objective of this study was to examine the potential of using the Parent Young Mania Rating Scale (P-YMRS) to distinguish pediatric bipolar disorder from other psychiatric conditions. The design of the study was a retrospective chart review. The setting of the study was community mental health. Participants included 130 children and adolescents. Measurements were based on P-YMRS scores. An ANOVA revealed a significant difference between mean scores of patients with and without bipolar disorder. A receiver operating characteristic (ROC) analysis revealed that the P-YMRS is very good at pred…

Clinical characteristics of bipolar vs. unipolar depression in preschool children: an empirical investigation.

BACKGROUND: Despite retrospective reports of the onset of childhood bipolar disorder during the preschool period, few studies have investigated whether mania symptoms can be identified in preschoolers. A group of children with a cluster of mania symptoms that showed discriminant validity from other disruptive disorders was identified in a large preschool sample. These empirical data add to descriptive studies of mania in clinical preschool populations. An investigation of the characteristics of depression among putative bipolar preschoolers may inform the controversial nosologic questions that…

Does Decreased Vitamin D Level Trigger Bipolar Manic Attacks?

BACKGROUND: Bipolar disorder is a chronic psychiatric disorder with depression and manic episodes. It is one of the leading causes of disease-related disability worldwide. Despite the presence of various alternative drug options for bipolar disorder, some patients do not adequately benefit from the treatment. Therefore, possible underlying mechanisms need to be clarified. Recently, studies on the relationship between bipolar disorder and vitamin D (Vit D) have attracted attention. Although many studies have found an association between depression and Vit D deficiency, little is known about the…

Impulsivity in first-degree relatives at risk of psychosis and mania: a systematic review and meta-analysis.

Impulsivity is elevated in psychosis and during mania in bipolar disorder. Studies in unaffected relatives may help establish whether impulsivity is a heritable, state independent endophenotype. The aim of this systematic review and meta-analysis was to examine whether impulsivity is elevated in unaffected relatives of those with bipolar disorder, schizophrenia, and schizoaffective disorder, compared to controls. Databases were systematically searched up until March 2023 for articles reporting data on a behavioral or self-report measure of impulsivity in first-degree relatives and controls. Ni…

Preliminary results of a fine-grain analysis of mood swings and treatment modalities of bipolar I and II patients using the daily prospective life-chart-methodology.

OBJECTIVE: The study aimed to increase the knowledge about the detailed course differences between different forms of bipolar disorder. METHOD: Using the prospective life-chart-clinician version, we compared the fine-grain analysis of mood swings and treatment modalities of 18 bipolar II with 31 bipolar I patients. RESULTS: During an observational period of a mean of 26 months we observed an increase of euthymic days, and a decrease of (sub)depressive and (hypo)manic days. Days in a (sub)depressed state were more frequent than days of (hypo)mania as well as days of subdepression or hypomania…

Seasonal mood disorders. Patterns of seasonal recurrence in mania and depression.

DSM-III-R criteria, applied retrospectively in a research-oriented psychiatric clinic, identified patients (N = 146) with a mood disorder and a seasonal pattern of recurrence (seasonal mood disorder). The seasonal mood disorder syndrome was not rare (10% of all mood disorders); diagnostic distribution was as follows: recurrent depression, 51%, and bipolar disorder, 49%, with 30% of the latter having mania (bipolar disorder type I) and 19% having hypomania (bipolar disorder type II). Most patients were women (71%); onset age averaged 29 years, with a mean of eight cycles in 12 years of illness;…

Management of bipolar depression with lamotrigine: an antiepileptic mood stabilizer.

The efficacy of lamotrigine in the treatment of focal epilepsies have already been reported in several case reports and open studies, which is thought to act by inhibiting glutamate release through voltage-sensitive sodium channels blockade and neuronal membrane stabilization. However, recent findings have also illustrated the importance of lamotrigine in alleviating the depressive symptoms of bipolar disorder, without causing mood destabilization or precipitating mania. Currently, no mood stabilizers are available having equal efficacy in the treatment of both mania and depression, two of whi…

Social rhythm regularity and the onset of affective episodes in bipolar spectrum individuals.

OBJECTIVES: Research suggests that bipolar disorder individuals may have less social rhythm regularity than normal controls and that this may contribute to their affective symptoms and episodes. This study examined whether regularity prospectively predicted time to onset of major depressive, hypomanic and manic episodes in a sample with bipolar spectrum disorders. METHODS: We recruited 414 undergraduate students from Temple University and University of Wisconsin diagnosed with cyclothymia, bipolar II disorder, or with no affective disorder (normal controls). Participants completed the Social …
