BACKGROUND: Supracondylar fractures of the humerus are common pediatric elbow injuries. Most displaced or angulated fractures are treated by closed reduction and percutaneous pinning, with either a crossed pin or lateral pin configuration. The purpose of this study was to conduct a systematic review to determine if there is an increased risk of iatrogenic nerve injury associated with the crossed pin configuration.
METHODS: Relevant articles were identified by searching electronic databases and hand searching-related journal and conference proceedings. Within each trial, the risk of iatrogenic

Surgical reconstruction for limb salvage is an acceptable and often preferred treatment for patients who are facing potential amputation. Because of the invasive nature of these surgeries, complications such as infection are not uncommon. Osseous infection at the site of implantable hardware may result in a tremendous consequence to the patient. In an attempt to minimize infection with implantable hardware, a local delivery system involving the use of antimicrobial coating on medical devices has been developed. This system optimizes local administration of antibiotics to the sources of infecti

INTRODUCTION: The demand for revision and salvage procedures after knee arthroplasty is increasing as the number of primary procedures increases. Surgical salvage techniques when revision arthroplasty is contra-indicated include above knee amputation and arthrodesis. The results of arthrodesis are functionally superior to those of amputation but not all techniques of arthrodesis are associated with good results.
MATERIALS AND METHODS: We present a single surgeon series of 14 consecutive patients who underwent arthrodesis of the knee with a customised intramedullary coupled nail (Mayday arthro

INTRODUCTION: The advantages of intramedullary fixation of pertrochanteric hip fractures over dynamic screw-plate devices in the clinical setting are still a matter of debate. We performed a case series study in a teaching hospital to analyse the results of the recent AO-ASIF proximal femoral nail (PFN).
MATERIALS AND METHODS: Between July 1998 and May 2001, 46 consecutive patients (47 fractures) were included in the investigation. The average patient age was 76.4 years (range 50-93 years), and most fractures (74.5%) were unstable. Operative details, adequacy of fracture reduction, screw posi

The results of a "selective treatment" policy applied to 176 patients presenting with supracondylar fracture of the humerus are analyzed. As initial treatment 148 patients underwent closed reduction and casting, seven closed reduction and pinning, 17 olecranon traction, and four open reduction and pinning. Analysis of patients by fracture type and initial treatment demonstrated a satisfactory outcome in 46 of 75 type III fractures and in 37 of 48 patients with type II fractures treated by closed reduction and casting. Thus a policy of initially pinning all such type III fractures in our unit w

INTRODUCTION: Flexible intramedullary nailing (FIN) is a common treatment method for paediatric forearm fractures. They are routinely removed after fracture healing. This study aims to assess the 15-years' experience of managing paediatric forearm fractures with FIN, with comparison between the outcomes of implant retention versus removal and cost analysis. This is the first study on FIN retention in paediatric forearm fractures.
METHODS: This was retrospective cohort study for paediatric forearm fractures treated with FIN between 2009 and 2024 at Leeds General Infirmary, a level one trauma c

PURPOSE: This article aims to present a technique for thumb tip amputations using a homodigital soft-tissue/periosteum flap and a portion of the amputated digit.
METHODS: Eight patients (aged 21-53 years) with avulsion thumb tip amputations were reviewed. We report a new technique in which a bone and nailbed composite graft was taken from the amputated portion of the thumb and a dorsoulnar flap combined with periosteum was harvested from the first metacarpal and designed to cover the volar bone. Patients were evaluated 6 months after surgery for functional and cosmetic outcomes.
RESULTS: The

OBJECTIVES: We examined the impact of the bioresorbable osteosynthesis sternal pin (Super Fixsorb 30) on sternal healing after median sternotomy.
METHODS: Sixty-three patients who underwent aortic surgery through median sternotomy between January 2006 and March 2009 were analysed. Sternal pins were utilized in 36 patients in addition to the standard closure of the sternum with Ethibond sutures (Group A), and 27 patients received no pins with the standard Ethibond sternal closure (Group B). The occurrence of transverse sternal dehiscence, anterior-posterior displacement and complete fusion of

BACKGROUND: Elongating intramedullary rods have been used in the management of osteogenesis imperfecta for the past fifty years. The complication rates reported in many reviews of the available techniques have been high. This study reviews the long-term functional outcomes and complications following the use of the Sheffield system of telescopic intramedullary rods.
METHODS: We conducted a retrospective analysis of patients with osteogenesis imperfecta who were at least eighteen years of age and who had at least thirteen years of follow-up. Complications, reoperations, and data from a disease

The intramedullary nailing of humerus has emerged as one of the treatments of reference for proximal fractures. We have reviewed 38 patients aged 64.2 on average with a mean follow of 18 months. The functional scores used were the Constant and Murley's score as well as the DASH self-administered quality of life questionnaire. Radiological criteria have been analyzed, namely the cephalic angle noted alphaF and the presence of any osteolysis of the major tuberosity. Patients were grouped by type of fracture, according to Neer's classification, with nine cases in Neer 2 group, 19 in Neer 3 group,

Subclavian vein catheterization is associated with failure and complications because of injury to the nearby lung and subclavian artery. Its position, sandwiched between the clavicle and the first rib, makes sonographic imaging difficult. The medially pointed sonography probe makes it difficult to align the needle as well as image the entire needle. The axillary vein lies outside of the thoracic cage and can be easily imaged in its longitudinal view along with the entire needle, guidewire, dilator, and catheter in real-time. All described techniques of venous access using sonography have used

Forty-five Russell-Taylor Type 1B subtrochanteric femoral fractures were stabilized using an interlocked cephalomedullary nail. The intraoperative complication rate was 13.5%; the most frequent complication was a varus malreduction. The union rate was 100% at an average of 13.5 weeks after surgery; there were no implant failures. Forty-three of 45 (96%) patients regained greater than 120 degrees knee motion. Based on these results it is thought that an interlocked cephalomedullary nail may be the implant of choice for stabilization of Russell-Taylor Type 1B fractures; however, its proper use r

AIMS: Pin-site infection remains a significant problem for patients treated by external fixation. A randomized trial was undertaken to compare the weekly use of alcoholic chlorhexidine (CHX) for pin-site care with an emollient skin preparation in patients with a tibial fracture treated with a circular frame.
METHODS: Patients were randomized to use either 0.5% CHX or Dermol (DML) 500 emollient pin-site care. A skin biopsy was taken from the tibia during surgery to measure the dermal and epidermal thickness and capillary, macrophage, and T-cell counts per high-powered field. The pH and hydrati

BACKGROUND: In situ pinning is the conventional treatment for a stable slipped capital femoral epiphysis (SCFE). However, with a severe stable SCFE the residual deformity may lead to femoroacetabular impingement and articular cartilage damage. A modified Dunn subcapital realignment procedure has been developed to allow for correction at the level of the deformity while preserving the blood supply to the femoral head.
QUESTIONS/PURPOSES: We compared children with severe stable SCFE treated with the modified Dunn procedure or in situ pinning in terms of (1) proximal femoral radiographic deformi
