Relative survival in elderly European cancer patients: evidence for health care inequalities. The EUROCARE Working Group.

This paper examines the survival of elderly European cancer patients, on the basis of the EUROCARE II results. Using Hakulinen and Abeywickrama's method, the relative survival rates at 1 and 5 years from diagnosis were computed by sex and quinquennial age group for the elderly (65-99 years old). Age-standardised rates for the whole elderly group were also calculated. The analysis covered: all malignancies combined, stomach, colon, rectum, pancreas, lung, melanoma, bladder, kidney and non-Hodgkin's lymphomas for both sexes; prostate and larynx for men; and breast, ovary, uterine cervix and corp

The impact of comorbidity on mortality following in-hospital diagnosis of tuberculosis.

STUDY OBJECTIVES: Despite the availability of curative chemotherapy, mortality remains high among patients hospitalized for tuberculosis. Although the elevated mortality rate is often attributed to the presence of multidrug resistant tuberculosis (MDRTB) or concomitant infection with the HIV, other factors must be contributory, especially among the HIV-negative population. Therefore, we performed a study to define the factors associated with mortality following the in-hospital diagnosis of tuberculosis in a region with low levels of MDRTB and coinfection with HIV. DESIGN: Retrospective cohort

Long-term mortality trends of COPD and respiratory failure among adults aged ≥25 years in the United States: an analysis of CDC WONDER data (1999-2023).

Chronic obstructive pulmonary disease (COPD) is a leading cause of morbidity and mortality in the United States and frequently associated with advanced stages complicated by respiratory failure (RF), a major terminal event that has been epidemiologically associated with significantly higher mortality risk among COPD patients. Despite this, long-term population-level trends in COPD-related mortality complicated by RF remain insufficiently characterized. We aimed to quantify long-term mortality trends of COPD and RF among adults aged 25 years or older in the United States from 1999 to 2023 by an

Attitudes Toward Video Consultations From the Perspective of Physicians and Psychotherapists in German Outpatient Care After the COVID-19 Pandemic: Survey Study.

BACKGROUND: Although video consultations (VCs) are permitted in German outpatient care and have seen a notable rise during the COVID-19 pandemic, their use still does not seem to have become established in Germany. OBJECTIVE: This survey aims to evaluate the attitudes of physicians and psychotherapists with regard to the use of VC after the COVID-19 pandemic, in particular in the context of types of treatment and suitable medical fields. METHODS: A standardized questionnaire was sent out to all 34,095 physicians and psychotherapists in 4 German regions. The analysis consisted of both descrip

Change in self-reported tobacco cessation support practices among HIV providers in Kisumu, Kenya: a pre-post evaluation of a web-based training programme nested within the IN-ToUCH cluster randomised trial.

INTRODUCTION: HIV care providers are in frequent contact with their patients and are well placed to provide tobacco cessation support. Yet they rarely do so in Kenya, despite National Guidelines calling for cessation interventions to be integrated into HIV care. Increasing healthcare providers' (HCPs) knowledge in tobacco cessation interventions could improve quit rates and contribute to better health outcomes. We evaluated whether a web-based training programme improved HIV HCPs' self-reported delivery of tobacco cessation support. METHODS: This pre-post evaluation was conducted within 19 HI

Menopausal Hormone Therapy and Cardiovascular Risk in Midlife Women With Vasomotor Symptoms.

IMPORTANCE: Vasomotor symptoms in perimenopause are associated with increased future cardiovascular (CVD) risk. Most clinical trials on menopausal hormone therapy (MHT) and CVD risk have focused on postmenopausal women, with limited study of the perimenopausal period when hormonal fluctuations and symptoms are greatest. Moreover, these trials were not designed to evaluate CVD risk with MHT among women with vasomotor symptoms. OBJECTIVE: To estimate the effect of MHT on CVD risk among perimenopausal and recently postmenopausal women with vasomotor symptoms and assess the extent to which timing

Cognitive Sarcopenia: Prevalence and the Risk for Mortality and Healthy Aging in the KORA-Age Study.

BACKGROUND: Cognitive sarcopenia, defined by this study as the co-existence of sarcopenia and cognitive impairment, has been frequently reported in older adults, while we hypothesize that the co-existence increases the risk for adverse outcomes in the older general population. METHODS: This study included 1055 participants aged 65-93 years from the population-based cohort Cooperative Health Research in the Region Augsburg (KORA)-Age (2008/9). At baseline, probable sarcopenia (i.e., low grip strength) and confirmed sarcopenia (i.e., probable sarcopenia plus low muscle mass) were defined accord

From Approval to Evidence: Device Regulation, Long-Term Outcomes, and Conflicts of Interest in Metabolic Bariatric Surgery.

BACKGROUND: Device-based innovation in metabolic bariatric surgery (MBS) has proliferated under evidentiary standards that differ substantially from those for pharmaceuticals. This review examines the lifecycle of MBS devices, from regulatory approval to long-term outcomes. METHODS: We conducted a structured literature search of PubMed/MEDLINE, Embase, and Scopus for English-language publications from January 1990 through May 2026, supplemented by regulatory database searches (FDA, EUDAMED) updated to manuscript submission. Five device categories representing distinct regulatory pathways were

Severe Maternal Morbidity by Race and Ethnicity and Birth Mode Among Individuals With a Prior Cesarean Birth.

IMPORTANCE: Given that nearly one-third of US births are cesarean deliveries, subsequent births after a cesarean delivery are common. Racial and ethnic disparities in severe maternal morbidity (SMM) have been well-documented, and prior studies have identified differences in birth mode after prior cesarean delivery by race and ethnicity. OBJECTIVE: To examine variation by race and ethnicity in the association between SMM and birth mode for individuals with a prior cesarean delivery. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used Massachusetts linked birth certificate and h

Association of multiple pregnancies with risk of preterm birth in the United States: a retrospective cohort study.

BACKGROUND: The elevated risks of adverse fetal and neonatal outcomes in multiple pregnancies (MPs) are predominantly driven by complications linked to preterm birth (PTB). However, studies on the association between MPs and PTB are limited. Furthermore, the impact of varying degrees of MPs on the likelihood of PTB is unclear, making it challenging to assess the PTB risk degree associated with different types of MPs. This uncertainty can consequently affect prenatal and perinatal management planning and fetal health. In this study, we aimed at examining the association between MPs and the risk

Effects of atmospheric pressure change during flight on insulin pump delivery and glycaemic control of pilots with insulin-treated diabetes: an in vitro simulation and a retrospective observational real-world study.

AIMS/HYPOTHESIS: Glycaemic control and clinical outcomes in diabetes are improved by continuous subcutaneous insulin infusion (CSII). Atmospheric pressure changes during flights may affect insulin delivery from pumps and cause unintended metabolic consequences, including hypoglycaemia, in people with type 1 diabetes. The present report evaluates both hypobaric flight simulation and real-world data in pilots using insulin pumps while flying. METHODS: In the flight simulation part of this study, an in vitro study of insulin pumps was conducted in a hypobaric chamber, de-pressurised to 550 mmHg
