Severe cholestatic jaundice induced by Epstein-Barr virus infection in the elderly.

Infectious mononucleosis due to Epstein-Barr virus (EBV) is almost always a self-limited disease, most commonly seen in young adults. Hepatitis is a well-recognized complication of EBV infection that usually resolves spontaneously. Jaundice occasionally results from the unusual complication of autoimmune haemolytic anaemia rather than hepatitis. We report a 60-year-old man with severe cholestatic jaundice whose history, liver histology and laboratory findings suggested EBV infection. He also developed significant jaundice related to his hepatitis, but not to autoimmune haemolysis, a situation

Operative treatment of congenital stenoses of the intrahepatic bile ducts in patients with choledochal cysts.

BACKGROUND: Postoperative complications including intrahepatic calculi may develop after the complete excision of a choledochal cyst. Since congenital stenoses of the intrahepatic bile ducts are more likely the cause of intrahepatic calculi, operative procedures for intrahepatic stenoses are reported. METHODS: There were 16 patients with choledochal cysts who underwent surgery for stenoses of intrahepatic bile ducts. The stenoses were excised at the opening of the common hepatic duct. RESULTS: In the 16 patients, 25 of the 26 stenoses that involved an intraluminal membrane or septum could be

[Transcutaneous radiotherapy combined with low dose intraluminal brachytherapy in the treatment of non-operable neoplastic stenoses of the bile ducts].

Percutaneous biliary drainage is an excellent method to relieve the acute symptoms related to neoplastic stenoses of extrahepatic bile ducts. However, survival rates are low and the quality of life of these patients is poor. High dose irradiation--combined with external beams (ERT) and intraluminal brachytherapy (BRT) through percutaneous drainage--allows effective disease control and, therefore, not only higher survival rates but also a much better quality of life when drainage can be removed after bile duct stenosis resolution. March, 1990, through March, 1993, eleven patients (8 with extrah

The applicability of micro- and minilaparotomy in the management of obstructive cholecystitis.

There are conflicting opinions concerning the management of acute cholecystitis, with particular reference to the optimal time for surgical intervention in the course of minimally access surgery. The present study was undertaken to elucidate the applicability of micro- and modern minilaparotomy in the management of obstructive cholecystitis and to ascertain the optimum time for surgical intervention. A total of 607 consecutive cholecystectomies were performed by micro- and modern minilaparotomy between 11 December, 1990 and 11 December, 1993 at our department: 202 (33.3%) with obstructive chol

An uncommon cause of biliary obstruction (Mirizzi syndrome): report of five cases.

Mirizzi syndrome is a rare form of common hepatic duct obstruction resulting from an inflammatory response secondary to a gallstone impacted in the cystic duct or neck of the gallbladder. Herein, we report five patients with this syndrome. Clinically, all patients had prominent jaundice. Ultrasound examination showed a large stone in the neck of the gallbladder. Endoscopic retrograde cholangiopancreaticography demonstrated a filling defect in the biliary tract the cystic duct level. Four patients possessed cholecystobiliary fistulas. Two patients also had common bile duct stones. Operations in

[Hemangiosarcoma of the liver. The diagnostic difficulties and therapeutic possibilities].

A 65-year-old man complained of decreasing physical capacity and weakness over the preceding six months, associated with marked painless jaundice and subsequently ascites. Despite extensive tests, some invasive, in three different hospitals no cause was found of the patient's symptoms and the marked though nonspecific abnormalities of various biochemical values (raised bilirubin concentration; increased alkaline phosphatase activity). Liver transplantation was performed because of progressive liver failure, without a firm diagnosis being established. At operation the liver was found to contain

Stones in a long pancreaticobiliary common channel: a rare cause of obstructive jaundice and pancreatitis.

A case of obstructive jaundice associated with acute pancreatitis is reported. The underlying cause was a dilated long pancreaticobiliary common channel impacted with stones and bile debris. There was no stone in the rest of the biliary system except for one in the lowest part of the common bile duct and the overall appearance of the stones suggested that they originated within the common channel itself. A long common channel may be an underlying cause of various pathological conditions in the pancreaticobiliary system. Early operative intervention is recommended when such a diagnosis is made.

Copper and liver disease.

There has been some limited progress in the understanding of the basic defect in Wilson disease and the gene concerned has been located to the chromosome region 13q14. Treatment with zinc has emerged as a definite alternative to penicillamine administration and some shortcomings and/or hazards of both forms of therapy have emerged as their modes of action have been studied more carefully. Tetrathimolybdate may have a place in treatment, especially when rapid complexing of copper is important. Hepatic copper accumulation occurs in a number of cholestatic diseases and they play an important part

[Jaundices in pregnancy].

There are three types of disorders which may produce jaundice during pregnancy: 1) acute or chronic coexistent or pre-existent liver disease (acute and chronic hepatitis, liver cirrhosis etc.), 2) acute and chronic hemolytic disorders, 3) jaundice directly or indirectly related to pregnancy. Among the later group, intrahepatic cholostasis, gravidic hyperemesis with jaundice represent exacerbations of physiologic gravidic cholostasis. Hepatic steatoses of pregnancy are the result of toxic metabolites or drugs on the liver parenchyma. Severe forms of eclampsia may induce jaundice of hemolytic an

[Repeat surgery in focal lesions of the liver].

The authors had 302 patients with focal lesions of the liver, 232 of them were operated on. Radical operations were carried out on 96 and palliative operations on 136 patients. Operations were performed for a second time in 35 patients, radically in 24 of them. A previously conducted exploratory laparotomy is still no evidence of inoperability of the process. In alveococcosis of the liver with no clinical signs of biliary stasis, it is advisable to consider resection of the liver after a previously conducted bile-draining operation. Patients who had been subjected to cavernotomy may also be op

[Cholelithiasis in patients with primary extrahepatic portal hypertension].

The results of treatment of 17 patients who had primary extrahepatic portal hypertension in combination with cholelithiasis are analysed. Of them 13, were operated on. In presence of the extended strictures of the bile ducts, the performance of hepaticojejunostomy is mandatory. In dominating involvement of the extrahepatic bile ducts, the endoscopic papillotomy as a definitive intervention is expedient. In asymptomatic course of cholelithiasis, the operation is not expedient.

[Plasma levels of various gastrointestinal polypeptides in patients with cholelithiasis and different degree of functional disorders of the major duodenal papilla].

Findings of dynamic cholangiomanometry with the analysis of the tension curves are overviewed. This technique helped reveal different functional ailments of the bile papilla in major variants of the cholelithiasis course (acute obstructive++ cholecystitis, recurrent pancreatitis, and choledocholythiasis with obstructive jaundice). Parallel radioimmunoassay-based studies of a series of gastrointestinal polypeptides (insulin, glucagon, gastrin, vasoactive peptide, bombesin , and somatostatin) were conducted to determine the importance of these polypeptides in the pathogenesis of cholelithiasis c
