Predictors of reduced coronary flow reserve in heart transplant recipients without angiographically significant coronary artery disease.

BACKGROUND: Determination of coronary flow reserve (CFR) is increasingly used to assess the functional significance of cardiac allograft vasculopathy. Although the relation between CFR and angiographically defined vasculopathy has been studied extensively, little is known about other factors determining CFR in heart transplant recipients without significant lesions by coronary angiography. METHODS: Sixty consecutive patients were studied 0.5 to 148 months after heart transplantation with intracoronary Doppler and intravascular ultrasound. An endothelium-independent CFR< or =2.5 was defined as

Clinical significance of reduced regional myocardial glucose uptake in regions with normal blood flow in patients with chronic coronary artery disease.

OBJECTIVES: The objective of this study was to assess the clinical significance of reduced regional fluorine-18 (18F) fluorodeoxyglucose uptake with normal flow in patients with chronic coronary artery disease. BACKGROUND: In patients with ischemic left ventricular dysfunction, 18F-fluorodeoxyglucose uptake may be reduced in some myocardial regions despite normal flow. The significance of this finding is unclear and has not been investigated systematically. METHODS: Twenty-three patients with coronary artery disease and impaired ventricular function (mean ejection fraction [+/- 1 SD] 28 +/-

The reduction of infarct size--forty years of research.

Advances in electrocardiography and enzymology in the 1940s and 1950s have provided better knowledge of the clinical evolution of myocardial infarction and recognition of the prognostic relevance of acute phase arrhythmias. This prompted the creation of intensive coronary care units in the subsequent decade. After the successful resolution of acute phase arrhythmias, it became clear that the myocardium necrotic area size was a determining factor in the long-term prognosis. The Killip-Kimball clinical classification in the 60s helped to clarify the role of infarct size on left ventricle (LV) dy

Assessment of the impact of post-processing approaches on absolute myocardial blood flow quantification using cardiac-dedicated CZT-SPECT: A retrospective observational study.

Quantitative myocardial blood flow (MBF) assessment using cardiac-dedicated cadmium-zinc-telluride (CZT) single photon emission computed tomography (SPECT) has emerged as a promising noninvasive approach for evaluating both macrovascular and microvascular coronary function. However, the reliability and comparability of MBF measurements are substantially influenced by variations in post-processing methodologies, including the application of attenuation correction (AC) and scatter correction (SC) techniques. Despite the increasing clinical adoption of CZT-SPECT for absolute perfusion quantificat

Percutaneous Atriotomy for Levoatrial-to-Coronary Sinus Shunting in Symptomatic Heart Failure: First-in-Human Experience.

OBJECTIVES: Our study reports the first-in-human experience of a novel approach for left atrial access via the coronary sinus-the percutaneous atriotomy-which is used for left atrial decompression with a dedicated left atrial (LA)-to-coronary sinus (CS) shunt. BACKGROUND: Elevated LA pressures contribute to heart failure symptoms, and targeted therapy with atrial shunt devices for LA decompression is an emerging strategy. Current devices reside in the interatrial septum, with risk for right-to-left shunting and systemic embolization. Moreover, preservation of the interatrial septum is imperat

Effect of Left and Right Coronary Flow Waveforms on Aortic Sinus Hemodynamics and Leaflet Shear Stress: Correlation with Calcification Locations.

Coronary flow induces hemodynamic alterations in the aortic sinus region. The objectives of this study are to: (1) investigate the differences among sinus hemodynamics and leaflet wall shear stresses engendered by the left versus right versus non-coronary flow and (2) correlate respective wall shear stresses with leaflet calcification in patients. A left heart simulator flow loop with a tunable coronary circuit provided physiological coronary flow waveforms corresponding to the left coronary cusp case (LCC), right coronary cusp case (RCC), and non-coronary cusp case (NCC). High spatio-temporal

Optimal Use of Vasodilators for Diagnosis of Microvascular Angina in the Cardiac Catheterization Laboratory.

BACKGROUND: Among patients with angina and nonobstructive coronary artery disease, those with coronary microvascular dysfunction have a poor outcome. Coronary microvascular dysfunction is usually diagnosed by assessing flow reserve with an endothelium-independent vasodilator like adenosine, but the optimal diagnostic threshold is unclear. Furthermore, the incremental value of testing endothelial function has never been assessed before. We sought to determine what pharmacological thresholds correspond to exercise pathophysiology and myocardial ischemia in patients with coronary microvascular dy

Decreased inspired oxygen stimulates de novo formation of coronary collaterals in adult heart.

RATIONALE: Collateral vessels lessen myocardial ischemia when acute or chronic coronary obstruction occurs. It has long been assumed that although native (pre-existing) collaterals enlarge in obstructive disease, new collaterals do not form in the adult. However, the latter was recently shown to occur after coronary artery ligation. Understanding the signals that drive this process is challenged by the difficulty in studying collateral vessels directly and the complex milieu of signaling pathways, including cell death, induced by ligation. Herein we show that hypoxemia alone is capable of indu

Analysis of cavopulmonary and cardiac flow characteristics in fontan Patients: Comparison with healthy volunteers.

BACKGROUND: Characterizing the flow of the Fontan circuit, and correlating flow characteristics with the development of complications, is an important clinical challenge. Past work has analyzed the flow characteristics of Fontan circulation on a component-by-component basis. 4D flow MRI with radial projections allows for large volumetric coverage, and therefore can be used to analyze the flow through many codependent cardiovascular components in a single imaging session. PURPOSE: To describe flow characteristics across the entire Fontan circuit and to compare these with the flow characteristi

Surgical results of unifocalization revision.

OBJECTIVE: Midline unifocalization has been developed for the surgical treatment of pulmonary atresia with ventricular septal defect and major aortopulmonary collateral arteries. All patients will eventually require reoperation due to the presence of a conduit, and some may also require revision of the distal unifocalized bed. The purpose of this study was to evaluate the surgical results of unifocalization revision. METHODS: This was a retrospective review of 254 patients who underwent midline unifocalization for treatment of pulmonary atresia with ventricular septal defect and major aortopu

Left Recumbent Position Decreases Heart Rate without Alterations in Cardiac Autonomic Nervous System Activity in Healthy Young Adults.

Some studies have reported that recumbent position may have advantages in patients with heart disease and in pregnancy. However, it remains controversial whether recumbent position affects autonomic nervous system activity and hemodynamics in healthy adults. The aim of this study was to evaluate alterations in heart rate variability (HRV) and hemodynamics in the supine, left recumbent and right recumbent positions in healthy young adults. A total of 80 participants aged 22.8 ± 3.1 years were enrolled in this observational study. Fifty-eight volunteers (29 men and 29 women) maintained the supin

[Infiltration of Cardiac Vessels by Lung Cancer: Incidence, Classification, Operative Technique with Heart Lung Bypass, and Results].

Carcinomas of the lung that infiltrate the blood vessels close to the heart (left atrium, pulmonary artery and aorta) without spreading to mediastinal lymph nodes or developing distant metastases are rare overall. Such situations are often classified as primarily inoperable by interdisciplinary tumour boards. This is only the case if, for technical reasons, an experienced thoracic surgeon does not feel able to perform a resection with a surrounding margin of healthy tissue. The surgical strategy to be employed must be chosen individually depending on the infiltrated structure. Complete tumour

High-Sensitivity and High-Resolution SPECT/CT Systems Provide Substantial Dose Reduction Without Compromising Quantitative Precision for Assessment of Myocardial Perfusion and Function.

UNLABELLED: There is increasing concern about radiation exposure from myocardial perfusion SPECT (MPS). γ-cameras with solid-state cadmium-zinc-telluride (CZT) detectors have better count sensitivity and spatial resolution than conventional sodium iodine detectors, allowing for significant reductions in radiotracer dose or acquisition time. This study aimed to demonstrate the capability of a hybrid CZT SPECT/64-slice CT system for dose reduction and to determine the maximal reduction possible without compromising image quality or the quantification precision of clinical MPS. METHODS: The imag

Coronary microcirculation changes in non-ischemic dilated cardiomyopathy identified by novel perfusion CT.

Intramyocardial microvessels demonstrate functional changes in cardiomyopathies. However, clinical computed tomography (CT) does not have adequate spatial resolution to assess the microvessels. Our hypothesis is that these functional changes manifest as altered heterogeneity of the spatial distribution of arteriolar perfusion territories. Our goal was to determine whether the spatial analysis of perfusion CT could clinically detect changes in the function and structure of the intramyocardial microcirculation in a non-ischemic dilated cardiomyopathy (DCM). Two groups were studied: (1) a Control

The role of venous return in critical illness and shock-part I: physiology.

OBJECTIVE: To provide a conceptual and clinical review of the physiology of the venous system as it is relates to cardiac function in health and disease. DATA: An integration of venous and cardiac physiology under normal conditions, critical illness, and resuscitation. SUMMARY: The usual teaching of cardiac physiology focuses on left ventricular function. As a result of the wide array of shock states with which intensivists contend, an approach that takes into account the function of the venous system and its interaction with the right and left heart may be more useful. This two-part review

Predictors of 30-day mortality and outcome in cases of myocardial infarction with cardiogenic shock treated by extracorporeal life support.

OBJECTIVES: The twin aims of this study were to identify the independent predictors of 30-day mortality and to analyse the outcomes of patients with cardiogenic shock (CS) associated with acute myocardial infarction (AMI) and necessitating extracorporeal life support (ECLS). METHODS: The investigation was a single-centre, retrospective study of 77 patients who required ECLS for AMI with CS. A logistic regression analysis was performed to identify the independent variables associated with 30-day mortality. RESULTS: Between February 2006 and November 2009, 745 patients in our establishment rec
