A Practical Approach to Chronic Hand Eczema.

Chronic hand eczema (CHE) is a multifactorial condition with significant physical, psychological, and socioeconomic burdens. Its complex pathogenesis often does not align with clinical presentation, leading to overlapping etiologic subtypes, including allergic contact dermatitis (ACD), irritant contact dermatitis (ICD), and atopic hand eczema (AHE). While patch testing remains the gold standard for confirming ACD, its clinical utility can be limited owing to frequent subtype overlap, impracticality of allergen avoidance, and practical considerations relating to the process and availability of

Analysis of patch testing results in patients with contact dermatitis in Istanbul, Turkey, from 2012 to 2022.

BACKGROUND: Allergens responsible for allergic contact dermatitis (ACD) differ between populations. They can even change over the years with the effect of environmental factors. AIMS: To evaluate the results of patch testing performed in our center. METHODS: In this study, the thin-layer rapid-use epicutaneous (T.R.U.E.) test results of patients with a diagnosis of ACD between 2012 and 2022 were retrospectively evaluated. RESULTS: In 431 (42.5%) of the total 1012 patients, a positive reaction to at least one allergen was detected in the patch test. Allergen positivity was most detected for

Frequency of occupational contact dermatitis in an ambulatory of dermatologic allergy.

BACKGROUND: Occupational contact dermatitis (OCD) corresponds to 80% of the cases of skin diseases related to work. OBJECTIVES: To determine: the frequency of OCD in an ambulatory of allergy not specific for occupational dermatosis; the profile of these patients according to age, gender, color, profession, atopic history, duration and localization of the dermatosis; the diagnosed types of contact dermatitis; the main allergens and to compare the frequency of the main antigens with the results obtained from patients examined in the same period of time, without DCO. METHODS: During the three-y

The status of corticosteroid therapy in dermatology.

Therapy with systemic corticosteroids, despite attendant serious risks, is mandatory in diseases such as pemphigus, acute disseminated lupus erythematosus and some cases of exfoliative dermatitis that are ordinarily fatal, for in such cases life may be prolonged and the patients made comfortable. If no contraindications exist, therapy with corticosteroids is desirable, for diseases of short duration-contact dermatitis, serum sickness reactions and drug eruptions of all kinds-provided the causative factors have been removed and the reactions are causing severe distress. On the basis of encouragi

Contact Allergy to Shellac. Retrospective Cross-Sectional Study With Data From the Spanish Registry of Research in Contact Dermatitis and Cutaneous Allergy (REIDAC).

INTRODUCTION: Shellac is a known allergen present mainly in cosmetics used on the eyelids and lips, although new sources of exposure have recently been described. Our objective was to assess the use of shellac as a contact allergen in Spain and the clinical profile of patients allergic to shellac. METHODS: This retrospective cross-sectional study included patients patch tested for shellac between 2018 and 2021 from the Spanish Registry of Contact Dermatitis and Cutaneous Allergy (REIDAC). RESULTS: A total of 980 patients were patch tested for shellac (20% in ethanol), and 37 (3.77%, 95% conf

National survey of patch testing in the United Kingdom and Republic of Ireland: what proportion of the population is and should be tested?

BACKGROUND: An initial study of patch testing practices across Wales, United Kingdom, showed wide variation in practice against a standardized set of guidelines and best practice recommendations. OBJECTIVES: To evaluate (i) current patch testing practice on a national level in the United Kingdom and Republic of Ireland (ROI), (ii) compliance with the current published guidelines, and (iii) the proportion of the catchment population tested by patch testing centres. METHODS: In consultation with the British Association of Dermatologists and British Society for Cutaneous Allergy, proformas addr

Contact sensitization to disperse dyes in children.

Between 2007 and 2010, the Netherlands experienced one of the largest outbreaks of Q fever. Since asymptomatic Coxiella burnetii infection has been associated with maternal and obstetric complications, evidence about the effectiveness of routine screening during pregnancy in outbreak areas is needed. We performed a clustered randomised controlled trial during the Dutch outbreak, in which 55 midwife centres were randomised to recruit pregnant women for an intervention or control strategy. In both groups a serum sample was taken between 20 and 32 weeks of gestation. In the intervention group (n=

Secondary bacterial infections complicating skin lesions.

Secondary bacterial infection in skin lesions is a common problem. This review summarises a series of studies of the microbiology of several of these infections: scabies, psoriasis, poison ivy, atopic dermatitis, eczema herpeticum and kerion. Staphylococcus aureus and group A beta-haemolytic streptococci were the most prevalent aerobes and were isolated from all body sites. In contrast, organisms that reside in the mucous membranes close to the lesions predominated in infections next to these membranes. In this fashion, enteric gram-negative bacilli and Bacteroides spp. were found most often i

[The skin and the environment].

The skin is in such continuous and intimate interaction with environmental influences as no other organ. Environment comes in contact with the organism in the form of substances or forces (physical, chemical, biological) and via different routes: irradiation (electromagnetic irradiation, often referred to in a simplified way as "light"), air, food, direct contact with the skin, injection, or psychosocial interactions. All these influences can lead to disease under certain conditions and in certain individuals, and especially to skin disease. The critical factors are the dose, the mode and time

Allergy. Conventional and alternative concepts. A report of the Royal College of Physicians Committee on Clinical Immunology and Allergy.

Allergy is an exaggerated response of the immune system to external substances. It plays a role in a wide range of diseases. In some, such as summer hayfever, the symptoms are due entirely to allergy. In other conditions, particularly asthma, eczema and urticaria, allergy plays a part in some patients but not all. In these situations, allergy may have either a major role or provide just one of many triggers. In an individual patient's illness, the importance of allergy may change with time. The most common allergens (substances causing allergy) are grass and tree pollens, the house dust mite,

Contact urticaria to cow milk.

A patient with contact urticaria to cow milk is presented. Contact sensitivity was documented to whole cow milk, skim milk, commercial condensed milk, and milk heated to 80 degrees C for 30 min. Contact sensitivity was also documented to casein and lactalbumin. Unlike a previous report our patient's sensitivity to heated milk precluded its use in the diet. Thus, heating milk does not necessarily reduce allergenicity to its component proteins.

[Netherton syndrome].

The case of a 8 years old girl, who since the delivery had suffered from a generalized, erythematous and scaling eruption of the skin is reported. She also had a sparse, short and brittle hair. Abnormalities such as trichorrhexis nodosa and pilo torti were observed. The finding of a generalized increase of the urinary aminoacids, excretion especially for Glycine and Serine, is added to this features.

Asthma and atopy in workers with an epoxy adhesive.

Seven workers using an epoxy adhesive cured with pyromellitic dianhydride were studied. Workers both mixed and used the adhesive. Each completed a questionnaire about previous respiratory symptoms, family or personal history of atopy, smoking history and symptoms using the adhesive. FEV1 was measured as a base-line and after working with the adhesive. Skin tests to common allergens were performed on all subjects. Two subjects who stated they wheezed with adhesive had falls in FEV1 of 15% and 18% from base-line. Totally symptom-free subjects had falls of less than 4% from base-line or a rise in
