Electrocochleography for Ménière's disease: is it reliable?

INTRODUCTION: Endolymphatic hydrops, the histopathological substrate of Ménière's disease, is an almost universal finding in postmortem studies of patients with this disease. The cause of hydrops is still unknown, as is the mechanism by which it causes progressive dysfunction of the sensory organs of inner ear. The fluctuating course of the disease complicates the interpretation of certain tests, such as electrocochleography; thus, for some authors its diagnostic value is questionable. OBJECTIVE: The aim of this study was to analyze the clinical applicability of electrocochleography in the di

Retinoic acid deficiency impairs the vestibular function.

The retinaldehyde dehydrogenase 3 (Raldh3) gene encodes a major retinoic acid synthesizing enzyme and is highly expressed in the inner ear during embryogenesis. We found that mice deficient in Raldh3 bear severe impairment in vestibular functions. These mutant mice exhibited spontaneous circling/tilted behaviors and performed poorly in several vestibular-motor function tests. In addition, video-oculography revealed a complete loss of the maculo-ocular reflex and a significant reduction in the horizontal angular vestibulo-ocular reflex, indicating that detection of both linear acceleration and

Vascular trauma during cochlear implantation: a contributor to residual hearing loss?

OBJECTIVE: This study was performed to investigate the vascular structures of the cochlea that are potentially vulnerable to mechanical trauma during cochlear implant surgery. BACKGROUND: Despite improvements in surgical technique and electrode design, residual hearing is lost in a significant percentage of cochlear implant patients. Although a variety of factors may contribute, it is widely believed that mechanical trauma plays an important role. This study focused on the vasculature of scala tympani and its potential susceptibility to injury during implant surgery. METHODS: Anatomic study

A brief review of Susac syndrome.

Susac syndrome was named after J.O. Susac who first described the syndrome in 1979. It is characterized by the clinical triad of encephalopathy, branch retinal artery occlusion, and sensorineural hearing loss. It mainly occurs in young women. This underdiagnosed disease needs to be considered in the differential diagnosis of a broad variety of disorders. In Susac syndrome, autoimmune processes leading to damage and inflammation-related occlusion of the microvessels in brain, retina, and inner ear are thought to play a causal role. The diagnosis is based primarily on the clinical presentation,

c-Ret-mediated hearing loss in mice with Hirschsprung disease.

A significantly increased risk for dominant sensorineural deafness in patients who have Hirschsprung disease (HSCR) caused by endothelin receptor type B and SOX10 has been reported. Despite the fact that c-RET is the most frequent causal gene of HSCR, it has not been determined whether impairments of c-Ret and c-RET cause congenital deafness in mice and humans. Here, we show that impaired phosphorylation of c-Ret at tyrosine 1062 causes HSCR-linked syndromic congenital deafness in c-Ret knockin (KI) mice. The deafness involves neurodegeneration of spiral ganglion neurons (SGNs) with not only i

Increased antisecretory factor reduces vertigo in patients with Ménière's disease: a pilot study.

It has been hypothesized that the symptoms of vertigo in patients with Ménière's disease somehow are related to impaired production and/or transport of endolymph. Antisecretory factor (AF) is a protein known to affect transport processes in the intestine and it has been shown that intake of specially processed cereals (SPC) can increase endogenous AF synthesis. In a prospective open pilot study, 24 patients with severe Ménière's disease (functional level scale 5-6 according to the criteria of AAO-HNS) received SPC for 14-30 days. AF levels in plasma increased by 83% in 20 of the 24 patients st

[Vestibular system in patients with sensorineural hearing loss].

A near localization of anatomical structures of an inner ear and possibility of damage caused by the same etiopathogenic factor implies that sensorineural hearing loss may be accompanied by vertigo and dizziness. The aim of a study was to evaluate a frequency of vestibular system dysfunction in patients with sensorineural hearing loss and to analyse it according to severity and type of the hearing loss. We evaluated a group of 126 persons which were treated in ENT Department during the last year from sensorineural hearing loss. In all subjects' audiological and ENG examinations were performed.

Postural control and glycerol test in Ménière's disease.

Ménière's disease is a clinical disorder, characterized by fluctuating hearing loss, recurrent spontaneous episodic vertigo, tinnitus and aural fullness, which may be defined as the idiopathic syndrome of endolymphatic hydrops. The most important test for diagnosis of Ménière's disease is the glycerol test. This is a simple and rapid method and several authors have confirmed its efficiency for identifying endolymphatic hydrops. This test provides information on the cochlear response to the osmotic changes produced by glycerol in the inner ear, whereas modifications in the vestibular labyrinth

Phase-dependent suppression of transient evoked and distortion product otoacoustic emissions by a low-frequency tone.

The subjective recording of the masked threshold of short acoustical stimuli with a loud tone of 30 Hz (phase audiogram) has been used for the clinical diagnosis of endolymphatic hydrops (EH). In normally-hearing subjects, a marked modulation of the threshold was found, depending on the phase of the low-frequency tone. A very small dependence was found in patients with Menière's disease, due to the micromechanical changes in the basilar membrane (BM). The same phase relationship becomes apparent in low-frequency suppression of otoacoustic emissions. The amplitudes of TEOAEs are controlled by t

[Enlarged middle fossa approach to the cerebello-pontine angle. Technique and indications].

Our enlarged middle fossa approach, first published in 1982, aims to expose the internal auditory meatus and cerebello-pontine angle from above, with preservation of the cochlea and labyrinth. The steps of the operation are described, especially certain technical details: the skin incision, resection of the squamous temporal bone, extradural exposure of the petrous temporal bone, subsequent resection of the middle meningeal artery and superior petrosal sinus, extensive bone removal behind and in front of the internal auditory meatus, opening of the dura mater to expose the brain stem, and plas

Modulation of outer hair cell compliance and force by agents that affect hearing.

Force generated by outer hair cells is thought to be an essential source of mechanical input to the normal cochlea. Many disease processes in the inner ear act via outer hair cells. It is therefore plausible that such disease processes modulate outer hair cell force generation. The force generated by an isolated, electrically stimulated outer hair cell against a load may be represented by an intrinsic motor and a passive axial stiffness in series. Thus modulation of outer hair cell force generation may occur either by action on the motor or indirectly by an action on cell stiffness. In this st

Extra inner hair cells: prevalence and noise susceptibility.

A total of 39 cochleae, 10 non-exposed and 29 noise exposed cochleae, were analyzed in a scanning electronmicroscope. Inner hair cells (IHCs) localized on the modiolar side of the ordinary row of IHCs were described. The mean number of such extra IHCs was 11 per cochlea both for non-exposed and exposed ears, ranging from 0-37. They showed a bimodal distribution with a maximum number at apex and 6-9 mm from apex. The dimension of the cuticular plate was slightly but significantly larger than for the ordinary IHCs. The extra IHCs suffered significantly less damage to their stereocilia than the o

Immunological pathogenesis of endolymphatic hydrops and its relation to Menière's disease.

This study was designed to investigate an immunologically induced endolymphatic hydrops (ELH) and to focus on the issue of its pathogenesis in relation to Menière's disease. The time course of ELH was evaluated by light microscopy in a 2-hour to 7-month period following direct antigen challenge to the endolymphatic sac (ELS) in systemically pre-sensitized guinea pigs. ELH began to appear in the vestibule and the basal turn 5-7 hours after inner ear challenge and developed gradually. During the interval from the second day to the first week, ELH rapidly developed in all the cochlear turns and r

Ultrastructural evidence of a merocrine secretion in the human endolymphatic sac.

The results of a light and transmission electron microscopic analysis of an endolymphatic sac (ES) from a patient suffering from episodic vertigo, tinnitus, and hearing loss are presented. A biopsy of the intraosseous portion of the ES was obtained during a translabyrinthine approach to section the vestibular nerve in the internal acoustic meatus. The material consisted mainly of tubular epithelial structures filled with heavily stained material. Pathologically dilated and degranulated rough endoplasmic reticuli and disaggregation of polyribosomes with accumulation of solitary ribosomes in the

Histopathologic correlation of spiral ganglion cell count and new bone formation in the cochlea following meningogenic labyrinthitis and deafness.

Bacterial meningitis is a common cause of profound deafness and, hence, a common cause of deafness in published series of patients treated with a cochlear prosthesis. Labyrinthitis ossificans is a common finding in meningogenic labyrinthitis and has been considered a relative contraindication to cochlear implantation. In the present study, the numbers of remaining spiral ganglion cells in cases of meningogenic labyrinthitis were correlated with the severity of new bone formation within the inner ear. Six temporal bones in which profound sensorineural hearing loss occurred in life secondary to

Criteria for preservation of vestibulocochlear nerve function during microsurgical removal of acoustic neurinomas.

A careful examination of the shape, location and course of the 8th cranial nerve in medium-sized and large tumours exhibits three distinct variants. In all cases where cochlear function was preserved, the type III variant (12%) in the cranial nerve tumour relationship was observed. The author's experience clearly indicates that, if a tumour is resected in toto, anatomical nerve continuity cannot be preserved in the type I and II variants (48%, respect. 40%). However, in the type I and II variants the patients invariably had preoperative hearing loss. The important criteria which must be consid

A functional and histological study of the combined effects of gentamicin and aminooxyacetic acid on the organ of Corti of the guinea pig.

Aminooxyacetic acid (AOAA) is a transaminase inhibitor that has been shown to protect the inner ear from loud noises. This study was done to determine if it can also protect against the cochleotoxic action of gentamicin. Four groups of guinea pigs were injected with gentamicin in doses approximating a clinical therapeutic dose and then in ototoxic doses. Thereafter animals were treated with parenteral AOAA. The effect on hearing was investigated using Preyer's reflex measurements. All animals were sacrificed and their cochleas were examined histologically using the surface preparation techniqu

[Categorizing hearing disorders based on latency deviations in the BERA (brainstem electric response audiometry)].

The dependence of latencies of EAEPs (Early Auditory Evoked Potentials) on stimulus intensity has been determined from measurements at 15 normal hearing subjects. The results are in good agreement with an exponential function, which has been empirically found and fitted to the data. The deviation from this function in case of hearing disorders can be classified in terms of a "latency deviation diagram". In the simplest case of a pure conductive hearing loss, the effect on latencies is a mere shift of all values to higher levels. The resultant latency deviation lines can be computed and the res
