Radial Head Dislocation with Ipsilateral Proximal Shaft of Radius Fracture: A Case Report.

Radial head dislocations are uncommon in adults. They are commonly seen in children and are generally associated with proximal ulna fracture. Radial head dislocation with associated proximal radial shaft fracture is rarer than isolated radial head dislocation in adults. Due to the rarity of this complex injury, in the absence of keen observation and meticulous attention, the correct diagnosis might be missed leading to unsatisfactory management and related complications. Here, a similar case of radial head dislocation with associated proximal radial shaft fracture has been presented.

Isolated Rupture of the Short Head of the Biceps at the Proximal Musculotendinous Junction: A Report of Two Cases.

CASE: Two men aged 19 and 21 years sustained wakeboarding injuries where the towrope was forced against the anterior region of the arm. Physical examination revealed bruising and tenderness over the biceps with weakness in elbow flexion. Magnetic resonance imaging demonstrated complete musculotendinous rupture of their short head of biceps. Surgical repair followed by cast immobilization led to an excellent outcome in both cases. CONCLUSIONS: This is a unique series of complete musculotendinous rupture of the short head of the biceps occurring in wakeboarding accidents, with the mechanism of …

Repair of Bado II Monteggia Fracture: Case Presentation and Surgical Technique.

INTRODUCTION: Monteggia fractures are rare, although complex elbow injuries. Bado II Monteggia fractures are characterized by posterior dislocation of the radial head and concurrent fracture of the proximal or middle third of the ulna. This video demonstrates the open reduction and internal fixation of a complex Bado II Monteggia fracture dislocation. METHODS: The patient is a 65-year-old woman with a Bado II Monteggia fracture after a low-energy fall from standing height. Given the extent of comminution and the instability of the elbow, the patient was indicated for surgical fixation. RESUL…

Complications and Outcomes After Medial Ulnar Collateral Ligament Reconstruction: A Meta-Regression and Systematic Review.

BACKGROUND: Evidence is limited regarding the complications and outcomes after medial ulnar collateral ligament (UCL) reconstruction. The published data regarding this procedure are composed predominantly of small Level-III and IV retrospective studies for which meta-analysis is not generally useful. Meta-regression is an alternative technique to identify variables across multiple publications that have an effect on published outcomes and complication rates. METHODS: We performed a systematic search of published literature for outcomes after UCL reconstruction. A random effects meta-regressio

Ultrasound demonstration of distal triceps tendon tears.

PURPOSE: Rupture of the distal triceps tendon is an uncommon injury that may be unrecognized on clinical examination. The purpose of the study is to describe the role of US in distal triceps tendon tears evaluation. MATERIALS AND METHODS: IRB approval was obtained and patients gave written informed consent. Of 77 consecutive US examinations of the elbow obtained over a five-year period, eight patients with correlative MR and surgery available were identified having partial or complete distal triceps tendon tear. RESULTS: N = 4 complete tears of the triceps tendon and n = 4 partial tears of t…

Distal humeral nonunion.

Nonunion is one of the most common and challenging complications of distal humeral fractures. Newer internal fixation principles and techniques have improved the ability of orthopaedic surgeons to achieve stable fixation of complex distal humeral fractures. However, some fractures will fail to unite, leaving the patient with an unstable, dysfunctional, and often painful upper extremity that requires additional surgery.

Terminal bifurcation of the biceps brachii muscle and tendon: anatomic considerations and clinical implications.

OBJECTIVE: The objective of our study was to describe the anatomic variation of a bifurcated distal biceps tendon with MRI, histology, and dissection in cadavers and to report the MR appearance of superimposed lesions in a patient population with this anatomic variant. MATERIALS AND METHODS: Visual and histologic examinations of the distal biceps brachii tendon in eight sectioned fresh-frozen elbow specimens were performed. Dissection of 17 elbow specimens was performed to describe the distal biceps brachii tendon. In addition, all elbow MRI reports over a 3-year period (n = 411) were retrosp…

Supracondylar humeral fractures in children.

Operative fixation is indicated for most type-II and III supracondylar humeral fractures in order to prevent malunion. Medial comminution is a subtle finding that, if treated nonoperatively, is likely to lead to unacceptable varus malunion. Angiography is not indicated for a pulseless limb, as it delays fracture reduction, which usually corrects the vascular problem. A high index of suspicion is necessary to avoid missing an impending compartment syndrome, especially when there is a concomitant forearm fracture or when there is a median nerve injury, which may mask symptoms of compartment synd

Intra-articular fractures of the upper extremity: new concepts in surgical treatment.

New techniques of internal fixation, postoperative rehabilitation, and emphasis on functional as well as radiographic outcome have refined the surgical treatment of complex fractures of the glenoid, humeral head, supracondylar and intracondylar humerus, olecranon, radial head, distal radius, and distal radioulnar joint over the past decade. Early stabilization and rehabilitation of these injuries leads to soft-tissue stabilization and facilitates the patient's ability to place the hand in three-dimensional space.

Local fasciocutaneous flaps for olecranon coverage.

Fasciocutaneous flaps around the elbow can be categorized as proximally or distally based. Proximally based flaps include the radial and ulnar forearm flaps as well as the posterior interosseous flap. Displaced fractures of the head and neck of the radius in children: open reduction and temporary transarticular internal fixation.

This report reviews the results of 36 children surgically treated for fractures of the head and neck of the radius. Indications for operation were: displacement over 2 mm and/or angulation over 45 degrees. Treatment included open reduction and temporary internal fixation with a K-wire transfixing the joint. Eighteen children had a good result, 8 had a fair result, and 10 had a poor result. It appears that when radial head fractures are associated with severe displacement, a high rate of fair and poor results can be expected, despite treatment by open reduction and temporary internal fixation.

[Fracture of condylus radialis humeri during skeletal growth (author's transl)].

A long term follow-up study of 54 fractures of the lateral condyle of the humerus was done to assess their special problems during the period of skeletal growth. It was demonstrated, that valgisation of the carrying angle will always be a result of an increasing displacement of the peripheral fragment. Furthermore it was shown that the most significant growth disturbance following this fracture is a partial stimulation of the radial part of the growth plate. This leads to radial overgrowth and thereby to varus deformity of the axis of the elbow joint. The extent of this varus deformity is sign…

Supporting upper limb movements with a soft robot after a brachial plexus injury.

Brachial plexus injury causes severe arm weakness, forcing patients to rely on compensatory movements for daily activities. Most robotic solutions target isolated elbow or hand function, whereas multi-joint assistance remains limited. Here we show that a soft wearable exosuit providing shoulder and elbow assistance through pneumatic actuation and inertial sensing improves arm function in 14 adults with chronic traumatic brachial plexus injury, including six with complete lesions and ten after nerve transfer surgery. In single-session testing, the device increases active range of motion ( + 80-…

Analysis of 5 cases of PICC removal difficulty caused by fibroblastic sleeve.

OBJECTIVE: To explore the difficulties and solutions encountered during the removal of PICC catheters in five patients with a fibroblastic sleeve formation. METHODS: This study includes five cases of PICC (peripherally inserted central catheter) removal difficulty caused by the formation of a fibroblastic sleeve. The patients were selected from a cohort with two cases involving the basilic vein in the right elbow fossa, two cases involving the brachial vein in the right elbow fossa, and one case involving the right cephalic vein. All patients underwent ultrasonography to exclude phlebitis and…
