Mortality rates among adult critical care patients with unusual or extreme values of vital signs and other physiological parameters: a retrospective study.

BACKGROUND: We evaluated relationships of vital signs and laboratory-tested physiological parameters with in-hospital mortality, focusing on values that are unusual or extreme even in critical care settings. METHODS: We retrospectively studied Philips Healthcare-MIT eICU data (207 U.S. hospitals, 20142015), including 166,959 adult-patient critical care admissions. Analyzing most-deranged (worst) value measured in the first admission day, we investigated vital signs (body temperature, heart rate, mean arterial pressure, and respiratory rate) as well as albumin, bilirubin, blood pH via arterial…

Advancing Quality Metrics for Durable Left Ventricular Assist Device Implant: Analysis of the Society of Thoracic Surgeons Intermacs Database.

BACKGROUND: Patients undergoing left ventricular assist device (LVAD) implantation are at risk for death and postoperative adverse outcomes. Interhospital variability and concordance of quality metrics were assessed using the Society of Thoracic Surgeons Interagency Registry for Mechanically Assisted Circulatory Support (Intermacs). METHODS: A total of 22 173 patients underwent primary, durable LVAD implantation across 160 hospitals from 2012 to 2020, excluding hospitals performing <10 implant procedures. Observed and risk-adjusted operative mortality rates were calculated for each hospital.

[Current outcome quality in the care of preterm infants with birth weight less than 1 500 g as a basis for regionalisation of risk pregnancies].

BACKGROUND: For preterm infants an association between patient volume and mortality has been described. METHODS: Outcome variables were evaluated for 28 hospitals in Baden-Württemberg for the years 2004-2008. Hospitals with high patient volume were compared to hospitals with a lower patient volume. RESULTS: Outcomes for 1 164 infants in 2008 and for 4 775 infants in 2004-2008 were analysed. In 2008, mortality of preterm infants less than 32 weeks gestational age (GA) was 9.2% (n=402) in the 5 major hospitals compared to 6.5% (n=520) in the other hospitals (combined mortality 7.7%, n. s., chi

Survival benefit of IABP in pre- versus post-primary percutaneous coronary intervention in patients with cardiogenic shock.

BACKGROUND: Cardiogenic shock (CS) remains a major cause of in-hospital mortality in the setting of acute myocardial infarction (AMI). However, little evidence is available regarding the optimal order of intra-aortic balloon counter-pulsation (IABP) insertion and primary percutaneous coronary intervention (PPCI). The aim of this study was to assess the hospital and short-term survival benefits of two different IABP insertion approaches, before versus after PPCI in patients with acute myocardial infarction and cardiogenic shock. RESULTS: Total mortality was 80 patients representing 48.4% of th…

External Validation of the Comorbid Operative Risk Evaluation Score Among Medicare Beneficiaries Undergoing Major Gastrointestinal Surgery.

BACKGROUND: Accurate risk adjustment is essential in gastrointestinal surgery, particularly among older adults. The surgery-specific Comorbid Operative Risk Evaluation (CORE) score demonstrated greater in-hospital mortality discrimination than conventional comorbidity indices in its derivation study, but its performance has not been independently evaluated among Medicare beneficiaries undergoing major gastrointestinal surgery. METHODS: This retrospective cohort study used Medicare claims to identify beneficiaries aged 66 years or older undergoing major gastrointestinal surgery from 2016 to 20…
