Mutations of the calcium-sensing receptor (CASR) in familial hypocalciuric hypercalcemia, neonatal severe hyperparathyroidism, and autosomal dominant hypocalcemia.

The calcium-sensing receptor (CASR) is a plasma membrane G protein coupled receptor that is expressed in the parathyroid hormone (PTH) produ … ifelong hypercalcemia is asymptomatic. The homozygous condition manifests itself as neonatal severe hyperparathyroidism (NSHPT), a rare disorder characterized by extreme hypercalcemia and the bony changes of hyperparathyroidism which occur in infancy. The disorder autosomal dominant hypocalcemia (ADH) is due to gain-of-function mutations in the CASR gene. ADH may be …

The vitamin D receptor (VDR) start codon polymorphism in primary hyperparathyroidism and parathyroid VDR messenger ribonucleic acid levels.

Vitamin D regulates parathyroid cell proliferation and secretion of PTH. Increased prevalence of the polymorphic vitamin D receptor (VDR) alleles b, a, and T has been reported in sporadic primary hyperparathyroidism (PHPT), suggesting that these genetic variants may predispose to the disease. Recently, another polymorphism in the VDR gene was related to bone mineral density, and this VDR-FokI polymorphism causes different lengths of the VDR, implying possible functional consequences. The VDR-FokI polymorphism was studied in 182 postmenopausal women with sporadic PHPT and in matched controls. N…

Primary hyperparathyroidism: whole-body bone mineral density in surgically treated Danish patients: a three-year follow-up study.

Whole-body bone mineral density (BMD) and body composition were measured before surgery in 25 patients (20 women and 5 men, aged 53 +/- 13 years, range 26-73 years) with mild to moderate primary hyperparathyroidism (PHPT) and compared with 25 controls exactly matched with respect to age, gender, and menopausal status. Fifteen pairs of matched patients and controls were reexamined 3 years later (5 men and 10 women, aged 53 +/- 12 years in both groups). In the untreated PHPT patients, whole-body BMD was 95.4% +/- 10.5% (SD) of control BMD (p < 0.05). Body weight and height, body mass index, whol…

Preoperative imaging of parathyroid glands with technetium-99m-labelled sestamibi and iodine-123 subtraction scanning in secondary hyperparathyroidism.

BACKGROUND: Parathyroidectomy is unsuccessful in 10-30% of uraemic patients operated on for secondary hyperparathyroidism. We investigated the usefulness of preoperative radionuclide imaging, with simultaneous recording of the distribution images of iodine-123 and technetium-99m-labelled sestamibi. METHODS: 11 patients with secondary hyperparathyroidism underwent prospective imaging and parathyroidectomy. Plasma concentrations of intact parathyroid hormone (PTH) were measured in all patients before and 6 months after subtotal parathyroidectomy. FINDINGS: Preoperative scanning showed 42 hot-s…

Secondary hyperparathyroidism and vitamin D receptor binding to vitamin D response elements in rats with incipient renal failure.

The pathogenesis of secondary hyperparathyroidism in early renal failure is poorly understood. In the study presented here, parathyroid hormone and GFR in rats with mild renal failure of various durations are evaluated. Parathyroid hormone increased significantly 3 days after nephrectomy and peaked at 2 wk, despite reduction in GFR of < 50%. Parathyroid hormone remained elevated, but there was no difference in serum levels of calcium, phosphorus, and calcitriol between the nephrectomized and sham-operated rats. There were also no differences in both intestinal and kidney vitamin D receptor con…

Double-phase Tc-99m MIBI scintigraphy in secondary hyperparathyroidism relapsing after parathyroidectomy and removal of a parathyroid autograft.

A 53-year-old woman with chronic renal failure had secondary hyperparathyroidism relapsing after undergoing a second parathyroidectomy and excision of an autologous parathyroid graft that had been implanted after her first parathyroidectomy. Double-phase scintigraphy with Tc-99m MIBI disclosed pathologic MIBI uptake in the right anterior thorax where the parathyroid transplant had been placed. Histopathologic examination of three nodules excised from this region yielded hyperplastic parathyroid tissue. This case report further illustrates the value of double-phase Tc-99m MIBI scintigraphy in a…

Hypercalcemia due to constitutive activity of the parathyroid hormone (PTH)/PTH-related peptide receptor: comparison with primary hyperparathyroidism.

In Jansen's disease (JD), the hypercalcemia found in about half the cases is the result of a mutant, constitutively overactive, form of the PTH/PTHrP receptor, which in these cases also causes the skeletal dysplasia. The subject of the present report was first seen in 1956 and is still under treatment at the same medical center. We report the clinical course and a detailed study of calcium and bone metabolism carried out in 1976 and compare the results with those of six typical patients with mild primary hyperparathyroidism in whom exactly the same studies were carried out. In the patient with…

Primary hyperparathyroidism in patients who have received radiation therapy and in patients who have not received radiation therapy.

BACKGROUND: Primary hyperparathyroidism (HPT) occurs more frequently in persons who have been exposed to low-dose therapeutic radiation. Little information is available concerning whether or not the clinical, metabolic, and pathologic manifestations, as well as outcome, differ in these patients when compared with patients with sporadic HPT. STUDY DESIGN: Records from 438 unselected patients with primary HPT, who were treated from 1982 to 1993 were retrospectively analyzed. All patients prospectively received a standard questionnaire preoperatively and postoperatively concerning clinical manif…

[Surgery of primary hyperparathyroidism by unilateral approach under local anesthesia and peroperative assay of 1-84 parathyroid hormone].

By combining echography and per-operative hormone assay, surgery for primary hyperparathyroidism due to a single adenoma can be performed via a limited unilateral access route with local anaesthesia. In a recent series of 204 patients, this technique was not contraindicated in 88 (43%). The operation was successful in 81 of these (92%). In 8 cases, the operation had to be converted into bilateral cervicotomy under general anaesthesia. Six of them were successful. The overall cure rate was 98%. Peroperative assay of 1-84 parathyroid hormone is an essential element in this technique.

Comparison of intestinal calcium absorption and circulating 1,25-dihydroxyvitamin D levels in malignancy-associated hypercalcaemia and primary hyperparathyroidism.

The relation between circulating 1,25-dihydroxyvitamin D (1,25(OH)2D) levels and intestinal calcium absorption--as determined by an oral calcium load test--was studied in 16 patients with hypercalcaemia of malignancy (HM) and 16 with hypercalcaemic primary parathyroidism (HPT). In the HPT group serum calcium rose significantly after the oral calcium load and the increment correlated significantly with 1,25(OH)2D levels. While 1,25(OH)2D levels were raised to within the hyperparathyroid range in a number of HM patients, there was no correlation between change in serum calcium and 1,25(OH)2D lev

[Mid C regional parathyroid hormone in the clinical workup: diagnostic value in extrarenal (primary) and renal (secondary) hyperparathyroidism].

The selective determination of mid-C-regional parathyroid hormone (mid-C-PTH) in combination with other laboratory parameters is a reliable tool for diagnosis and treatment of extra-renal (primary) and renal (secondary) hyperparathyroidism. Early stages, which show either high-to-normal serum calcium and elevated mid-C-PTH or increased serum calcium but normal mid-C-PTH, can be distinguished from overt hyperparathyroidism. Alkaline phosphatase (AP) activity and mid-C-regional PTH provide biochemical confirmation of histologically classified renal osteodystrophy. Since the index AP X PTH signif…

Reoperative parathyroid surgery for primary hyperparathyroidism caused by multiple-gland disease: total parathyroidectomy and autotransplantation with cryopreserved tissue.

Between September 1975 and June 1980, 27 patients with primary hyperparathyroidism caused by multiple-gland disease underwent reoperative parathyroid surgery at the National Cancer Institute. In an effort to minimize persistence and recurrence we adopted a strategy of attempting total parathyroidectomy. Immediate autotransplantation was not performed, but rather tissue was assessed in vitro for suppressibility and was cryopreserved. Patients with prolonged postoperative hypoparathyroidism underwent autotransplantation with cryopreserved tissue. Of 26 surviving patients, 23 (88%) were cured of …

Etiology of hyperparathyroidism and bone disease during chronic hemodialysis. I. Association of bone disease with potentially etiologic factors.

The present study was prompted by the observation that, in patients with chronic renal failure being followed at this center, renal osteodystrophy developed almost exclusively in those who were treated by chronic hemodialysis at home rather than in our center. A systematic comparison was made between the 10 patients with roentgenographic evidence of the bone disease and 18 patients without demonstrable bone disease. The two groups were similar in age, sex, nature of renal disease, and duration of dialysis. The mean duration of kidney disease was almost 2 yr longer in the patients without bone

Persistent hyperparathyroidism and its predictive factors in end-stage renal disease patients after kidney transplantation: A prospective study.

Hyperparathyroidism (HPT) may persist in patients after kidney transplantation (KT). A better understanding of which patients are likely to have persistent HPT after KT would help in the development of optimal management strategies. Our aim was to examine the prevalence of persistent HPT after KT in a contemporary cohort and to identify risk factors associated with persistent HPT at 6 months. We followed 104 KT subjects in a longitudinal prospective cohort at Cho Ray Hospital, Vietnam, from October 2023 to May 2025. Persistent HPT was defined as a parathyroid hormone (PTH) level of 150 pg/mL a…

Elevated parathyroid hormone levels in normocalcemic patients after parathyroidectomy for primary hyperparathyroidism: what risk factors should be considered?

OBJECTIVE: Normocalcemia with elevated parathyroid hormone levels (NCePTH) after successful parathyroidectomy in patients with primary hyperparathyroidism (PHPT) has drawn attention during the postoperative period and remain a clinical conundrum. PATIENTS AND METHODS: We retrospectively studied 539 consecutive patients with PHPT who underwent parathyroid surgery from August 2017 to December 2023. RESULTS: The mean age of the patients was 53.3 years, and 66.3% of the patients were female. After a median follow-up of 12 months, 9.9% of the patients were diagnosed with NCePTH. Compared with patie…

Future Directions in Hypercalcemic and Normocalcemic Primary Hyperparathyroidism: FRAXplus for 10-Year Fracture Risk Assessment (A Retrospective Study).

Background: Osteoporosis/osteoporotic fractures are identified in both hypercalcemic (HC-HPT) and normocalcemic variant (NC-HPT) of primary hyperparathyroidism (HPT) at various rates. Objective: Noting the need of modern society to easily assess the osteoporotic fracture risk amid the diagnosis of HPT, we aimed to address this gap by analyzing the 10-year fracture risk assessment based on traditional FRAX (Fracture Risk Assessment Tool) model in comparison to the novel algorithm (FRAXplus), according to the adjustment for the presence of HPT, as well as for the use of lumbar bone mineral densi…

Diagnostic Contribution of SPECT/CT to Dual-Phase 99mTc-MIBI Parathyroid Scintigraphy for Preoperative Localization in Primary Hyperparathyroidism.

Background: Accurate preoperative localization of hyperfunctioning parathyroid glands is essential for minimally invasive surgery in primary hyperparathyroidism (PHPT). Although dual-phase 99mTc-MIBI planar scintigraphy is widely used, its diagnostic performance may be limited in small or ectopic lesions. Hybrid SPECT/CT imaging combines functional and anatomical information and may improve localization accuracy. The aim of this study is to evaluate the diagnostic contribution of SPECT/CT to dual-phase 99mTc-MIBI planar scintigraphy for preoperative localization in patients with PHPT. Methods:…
