Clinical experience of eclampsia at the University Teaching Hospital, Lusaka. An analysis of 50 cases.

A clinical analysis of 50 cases of eclampsia for the period June 1971-June 1973 is presented. During the same period there were 32 011 deliveries--an incidence of 0,156%. Of these 50 cases, only 8 were booked. There were 40 primigravidas. The highest number of fits recorded in any one patient was 6, and 13 patients has had fits before admission. Thirty-four patients were in established labour on admission. Only 12 patients had postpartum fits. The basis of management was heavy sedation, anticonvulsant therapy and emptying of the uterus as soon as it could be achieved. Biochemical studies and u

Exploring family, community and healthcare provider perceptions and acceptability for minimal invasive tissue sampling to identify the cause of death in under-five deaths and stillbirths in North India: a qualitative study protocol.

BACKGROUND: Around 5.4 million under-five deaths occur globally annually. Over 2.5 million neonatal deaths and an equivalent stillbirths also occur annually worldwide. India is largest contributor to these under-five deaths and stillbirths. To meet the National Health Policy goals aligned with sustainable development targets, adoption of specific strategy and interventions based on exact causes of death and stillbirths are essential. The current cause of death (CoD) labelling process is verbal autopsy based and subject to related limitations. In view of rare diagnostic autopsies, the minimally

Frontier nurse-midwives and antepartum emergencies, 1925 to 1939.

This article examines how the Frontier Nursing Service (FNS) utilized nurse-midwives to respond to antepartum emergencies such as preterm birth, eclampsia, malpresentation, and hemorrhage in the women of Appalachia in the years 1925 to 1939. Particular attention is given to the preparation that nurse-midwives received during their midwifery education to prevent and respond to emergencies. Using traditional historical research methods and primary source material from the FNS papers in the Special Collections, University of Kentucky Libraries, Lexington, Kentucky, this article describes the nurs

Using cell phones to collect postpartum hemorrhage outcome data in rural Ghana.

OBJECTIVE: To evaluate the use of cell phones by professional and traditional birth attendants in rural Africa for reporting postpartum hemorrhage (PPH) data. METHODS: Ten birth attendants from the remote Sene District of Ghana participated in the study. Subjects were trained to send Short Message Service text messages from cell phones using a simple numeric protocol to report data regarding PPH: maternal age; PPH; use of bimanual uterine compression; maternal and neonatal mortality; and prenatal care. Participants sent texts to a pre-programmed number to report data for all births they atten

Effect of a participatory intervention with women's groups on birth outcomes in Nepal: cluster-randomised controlled trial.

BACKGROUND: Neonatal deaths in developing countries make the largest contribution to global mortality in children younger than 5 years. 90% of deliveries in the poorest quintile of households happen at home. We postulated that a community-based participatory intervention could significantly reduce neonatal mortality rates. METHODS: We pair-matched 42 geopolitical clusters in Makwanpur district, Nepal, selected 12 pairs randomly, and randomly assigned one of each pair to intervention or control. In each intervention cluster (average population 7000), a female facilitator convened nine women's

National health objectives for the year 2000: the demographic impact of health promotion and disease prevention.

BACKGROUND: The national objectives in Healthy People 2000, drafted by health professionals aware of currently available public health interventions, represent a wealth of information about near-term future mortality and morbidity. METHODS: Life table methods were used to calculate the impact of projected changes in mortality and activity limitation rates on life expectancy and expected disability years. RESULTS: Meeting the mortality objectives would increase life expectancy at birth by 1.5 to 2.1 years, raising life expectancy to 76.6 to 77.2 years. In addition, meeting the target for disa

Maternal and perinatal outcome of expectant management of premature rupture of membranes in the midtrimester.

The purpose of this clinical investigation was to determine the maternal and perinatal results of continuing pregnancy in 118 consecutive patients with premature rupture of the membranes at 16 to 26 weeks. The mean gestational age at diagnosis of premature rupture of the membranes was 23.1 +/- 2.7 weeks, with a median of 23.5. The interval from rupture to delivery ranged from 1 to 152 days, with a mean of 13. There was no correlation between gestational age at the time of rupture and the latency period. Thirty-five patients received tocolytic agents and 24 received steroids. Forty-eight percen

Antiretroviral therapy (ART) for treating HIV infection in ART-eligible pregnant women.

BACKGROUND:: This systematic review focuses on antiretroviral therapy (ART) for treating human immunodeficiency virus (HIV) infection in ART … nfection, ART is a safe and effective means of providing maternal virologic suppression, decreasing infant mortality, and reducing MTCT. Specifically, AZT/3TC/NVP, AZT/3TC/LPV-r, and AZT/3TC/ABC have been shown to decrease MTCT. More research is needed regarding the use of specific regimens and their maternal and infant side-effect profiles.

Cardiorespiratory coupling during high-frequency oscillatory ventilation with volume guarantee in extremely preterm infants.

UNLABELLED: This study aims to evaluate associations between PCA-derived cardiovascular components and respiratory burden during high-frequency oscillatory ventilation with volume guarantee (HFOV-VG). This retrospective cohort study included infants born before 30 weeks of gestation who received HFOV-VG and underwent targeted neonatal echocardiography during respiratory support. Echocardiography assessed ventricular systolic function, cardiac output, myocardial deformation, and pulmonary vascular loading. Principal component analysis (PCA) derived integrated cardiovascular components. Multivar

Post-mortem characterisation of HIV-associated under-5 deaths in the CHAMPS network: population-based mortality surveillance.

BACKGROUND: Despite widespread implementation of vertical transmission prevention and paediatric HIV programmes, HIV remains a leading cause of child mortality. We analysed HIV-attributed deaths in the Child Health and Mortality Prevention Surveillance (CHAMPS) network, focusing on comorbidities, co-infections, and care gaps. METHODS: We conducted a prospective, descriptive analysis of deaths in children younger than 5 years (under-5) recorded from Dec 3, 2016, to Dec 31, 2024, across seven CHAMPS sites in Ethiopia, Kenya, Mali, Mozambique, Sierra Leone, South Africa, and Bangladesh. Clinical characteristics and respiratory management of bronchopulmonary dysplasia in preterm infants: a retrospective case-control study.

OBJECTIVE: Bronchopulmonary dysplasia (BPD) is a common complication in preterm infants, and early recognition of clinical risk factors facilitates risk prediction. METHODS: This retrospective case-control study included preterm infants admitted with BPD diagnosed and classified as mild, moderate or severe. Data were collected on perinatal characteristics, maternal factors, respiratory support, early arterial blood gas and haematologic markers and hospitalisation complications. RESULTS: Among 410 preterm infants, 210 developed BPD and 200 did not. Infants with BPD had lower gestational age (

Long-term developmental outcomes of very-low-birth-weight infants born to non-Korean mothers: an analysis using the Korean neonatal network (KNN) database.

BACKGROUND: Preterm infants born to multicultural families, particularly very-low-birth-weight infants (VLBWIs; <1,500 g), may experience disparities in neurodevelopmental outcomes. This study examined long-term developmental outcomes among VLBWIs born to non-Korean mothers using data from the nationwide Korean Neonatal Network (KNN) database. METHODS: A total of 20,582 VLBWIs born between 2013 and 2023 were analysed after excluding infants with major congenital anomalies. Participants were categorized into two cohorts: Period I (2013–2018) and Period II (2019–2023). Developmental outcomes at

Coverage and factors associated with receipt of early newborn postnatal care components in Afghanistan: findings from the 2022-2023 multiple indicator cluster survey.

BACKGROUND: Neonatal mortality remains high in low‑ and middle‑income countries, where early postnatal care (PNC) within 48 hours can avert many deaths. Yet neither the national coverage of newborn PNC nor the factors associated with its receipt have been assessed. OBJECTIVES: We assessed the coverage of newborn PNC components and the factors associated with their receipt in Afghanistan. METHODS: We conducted a cross‑sectional analysis of 11,965 mother-infant dyads from the 2022-2023 Afghanistan Multiple Indicator Cluster Survey. The outcome was coded 'yes' if the infant received at least tw

Evaluating a blood warming device for packed red blood cell transfusions to decrease hypothermia in very preterm infants: A randomised control trial protocol.

BACKGROUND: This protocol describes a study to test a commercial blood warming device in warming blood transfusions to prevent hypothermia associated with packed red blood cell (PRBC) transfusions in very preterm infants. OBJECTIVES: Very preterm infants receiving blood transfusions warmed by a commercial blood warmer will have less central body hypothermia post transfusion (abdominal temperatures <36.5°C) and/or a higher mean abdominal temperature for the hour after the transfusion is complete compared to those receiving blood transfusions by standard of care. This trial has been registered

Congenital diaphragmatic hernia: Exclusion criteria for repair or ECMO?

BACKGROUND: Mortality in congenital diaphragmatic hernia (CDH) approximates 30 %. Both severe lung hypoplasia and other anomalies contribute to demise regardless of repair and/or extracorporeal membrane oxygenation (ECMO). We report clinical and physiological parameters for CDH infants not offered repair or ECMO (NoR/ECMO). METHODS: A single center retrospective analysis of 364 CDH infants managed from 01/2003 - 12/2024. We analyzed reasonings to not offer repair or ECMO in 49 infants (13 %) across two time epochs. We compared the outcomes of infants that did or did not meet 3 possible lethal
