IL-2 consumption by highly activated CD8 T cells induces regulatory T-cell dysfunction in patients with hemophagocytic lymphohistiocytosis.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a severe inflammatory condition driven by excessive CD8(+) T-cell activation. HLH occurs as both acquired and familial hemophagocytic lymphohistiocytosis (FHL) forms. In both conditions, a sterile or infectious trigger is required for disease initiation, which then becomes self-sustaining and life-threatening. Recent studies have attributed the key distal event to excessive IFN-γ production; however, the proximal events driving immune dysregulation have remained undefined. OBJECTIVE: We sought to investigate the role of regulatory T (Tre

Persistence of uncontrolled cardiovascular risk factors in patients treated with percutaneous interventions for stable coronary artery disease not receiving cardiac rehabilitation.

BACKGROUND: Cardiac rehabilitation programmes are strongly recommended for all forms of coronary artery disease to reduce recurrent events and mortality. Few patients seem to participate in these programmes after elective percutaneous coronary intervention (PCI). The aim of this study was to assess changes in lifestyle and risk factors after PCI for patients with stable disease and not included in cardiac rehabilitation programmes. METHODS: We prospectively enrolled 207 consecutive patients with stable disease who underwent a first elective PCI. Follow up was performed at 13.0 ± 3.2 months fo

Intradiscal Mesenchymal Stromal Cell Therapy for the Treatment of Low Back Pain Due to Moderate-to-Advanced Multilevel Disc Degeneration: A Preliminary Report of a Double-Blind, Phase IIB Randomized Clinical Trial (DREAM Study).

BACKGROUND: Low back pain (LBP) is a leading cause of disability worldwide, often associated with intervertebral disc degeneration (IDD). Mesenchymal stromal cells (MSCs) have emerged as a promising regenerative therapy for IDD due to their ability to promote tissue repair. This phase IIB randomized controlled trial aimed to evaluate the safety and efficacy of autologous bone marrow-derived MSC (BM-MSC) intradiscal injections in patients with chronic LBP due to moderate-to-advanced multilevel IDD. METHODS: Fifty-two patients with chronic LBP unresponsive to conservative treatments with modera

Botulinum toxin type A versus botulinum toxin type B for cervical dystonia.

BACKGROUND: Cervical dystonia is the most common form of focal dystonia. It is characterized by involuntary posturing of the head and frequently is associated with neck pain. Disability and social withdrawal are common. Most cases are idiopathic, and generally it is a life-long disorder. In recent years, botulinum toxin type A (BtA) has become first line therapy for cervical dystonia. However, not all patients respond well to BtA, and 5 to 10% become resistant to it. Botulinum toxin B (BtB) is an alternative to BtA and offers the potential to help patients who do not respond to BtA. At present

Lifestyle and dietary interventions for Ménière's disease.

BACKGROUND: Ménière's disease is a condition that causes recurrent episodes of vertigo, associated with hearing loss and tinnitus. Lifestyle or dietary modifications (including reducing the amount of salt or caffeine in the diet) are sometimes suggested to be of benefit for this condition. The underlying cause of Ménière's disease is unknown, as is the way in which these interventions may work. The efficacy of these different interventions at preventing vertigo attacks, and their associated symptoms, is currently unclear. OBJECTIVES: To evaluate the benefits and harms of lifestyle and dietary

Bone Recovery After Treatment Study (BRATS) - A Protocol for a Prospective, Observational Study in Multiple Myeloma

Cancer-induced bone disease is a huge burden on patient lives and costs the NHS millions of pounds every year. Breast, prostate and lung cancer can all lead to poor skeletal outcomes, but patients particularly at risk are those with a diagnosis of multiple myeloma (1). Despite response to tumour targeting treatments, patients experience debilitating bone pain and fractures, affecting quality of life (2, 3). Currently, myeloma patients who are eligible, are offered treatment with induction chemotherapy followed by autologous stem cell transplant (ASCT). Most eligible patients also receive bisph

Fucoidan as an anti-inflammatory and immunomodulatory agent in humans: a systematic review of clinical evidence and translational potential.

Purpose: fucoidan is a sulphated polysaccharide from brown seaweeds, commonly used as a dietary supplement due to its proposed anti-inflammatory, immunomodulatory, and potential tumour-related properties. However, despite substantial preclinical evidence, its clinical effects in humans remain unclear. This systematic review aims to evaluate and synthesise current clinical evidence on the effects of fucoidan on inflammatory, immune, metabolic, and tumour-related outcomes in humans. Methods: a systematic literature search was conducted in PubMed, Scopus, and Web of Science in accordance with the

Health-related quality of life with adjuvant immune checkpoint inhibition in renal cell carcinoma: A qualitative study of UK participants in the phase III RAMPART trial.

INTRODUCTION: RAMPART is an international phase III trial evaluating adjuvant immune checkpoint inhibitors (ICIs) after nephrectomy for renal cell carcinoma. ICIs have a distinct toxicity profile that can significantly impact health-related quality of life (HRQoL); however existing HRQoL questionnaires were developed in a pre-ICI era and may not fully capture the patient experience. To address this, we embedded a qualitative interview sub-study within RAMPART among UK participants to explore the impact of ICIs on HRQoL in depth. METHODS: Eligible participants were enrolled in RAMPART at UK si
