Progressive isolated lower-lip oedema after mRNA COVID-19 booster complicated by methicillin-resistant Staphylococcus aureus superinfection: a necrotizing fasciitis mimic.

Vaccine-related mucocutaneous reactions are usually mild, with atypical presentations and superinfections that can mimic soft-tissue emergencies. A previously healthy 19-year-old man developed isolated lower-lip oedema 3-4 h after an mRNA COVID-19 booster, peaking at 96 h. Initial blood test results were unremarkable, and angioedema therapy (antihistamines and a short course of corticosteroid) was ineffective. On day 12, he re-presented with marked swelling and necrotic slough but no systemic toxicity [Laboratory Risk Indicator for Necrotising Fasciitis (LRINEC) score of 4]. Debridement and hi…

Early-life stress signatures in deciduous incisor microstructure: An odontochronological case study of monozygotic twins discordant for cleft lip and palate.

OBJECTIVES: This study examined whether early-life stress events can be detected and chronologically dated through microscopic analysis of deciduous teeth. A unique case of monozygotic twins was used-one born with cleft lip and palate (CLP) who underwent early surgical interventions, and the other born without major health conditions. DESIGN: Histological thin sections of four naturally shed deciduous incisors were analyzed using polarized light microscopy. Accentuated growth lines-microscopic features that form in response to systemic stress during tooth development-were compared with medica…

Rare missense variants in NECTIN1 alter local protein structure and may contribute to non-syndromic cleft lip with or without palate.

INTRODUCTION: Orofacial cleft is a congenital anomaly influenced by genetic and environmental factors. NECTIN1 encodes an adhesion protein critical for the adherens junctions and has been associated with orofacial clefts. This study aimed to investigate the contribution of NECTIN1 to the etiology of NSCL/P in a population from Patagonia, Argentina, a region with a high prevalence of orofacial clefts. METHODS: First, an association study was conducted to evaluate the relationship between orofacial clefts and two NECTIN1 single nucleotide variants: rs3829260 (G > C) and rs7940667 (C > A). Gen

When Air Leaks Meet Innovation: Obturator-Assisted Non-invasive Ventilation in a Preterm Infant With a Unilateral Cleft Lip and Palate.

Preterm infants often have respiratory distress requiring non-invasive respiratory support. Those born with a cleft lip and palate pose significant challenges in maintaining positive pressure due to anatomic differences that can cause substantial air leak when using typical mask/cannula interfaces. This leads to difficulties with ventilation and oxygenation for these infants whose lung disease would otherwise not necessitate invasive ventilation. The purpose of this case report is to provide a multidisciplinary approach to a preterm infant with a unilateral cleft lip and palate for innovative …

The narrowing sex gap in lip and oral cavity cancer incidence in Spain: Thirty years of divergent age-period-cohort trends.

BACKGROUND: Lip and oral cavity cancer (LOCC) accounts for a significant proportion of the overall burden of head and neck cancers, yet long-term sex-specific trends remain unclear. This study aimed to analyse sex-specific LOCC incidence trends in Spain from 1994 to 2023. METHODS: An ecological study was conducted using modelled incidence data from the Global Burden of Disease 2023 (GBD). Age-standardised incidence rates (ASIRs) were calculated using the European Standard Population. Joinpoint regression was used to estimate annual percent changes (APCs) and average annual percent changes (AA…

Pronounced expression of extracellular matrix proteoglycans regulated by Wnt pathway underlies the parallel evolution of lip hypertrophy in East African cichlids.

Cichlid fishes inhabiting the East African Great Lakes, Victoria, Malawi, and Tanganyika, are textbook examples of parallel evolution, as they have acquired similar traits independently in each of the three lakes during the process of adaptive radiation. In particular, 'hypertrophied lip' has been highlighted as a prominent example of parallel evolution. However, the underlying molecular mechanisms remain poorly understood. In this study, we conducted an integrated comparative analysis between the hypertrophied and normal lips of cichlids across three lakes based on histology, proteomics, and …

Orthodontic Management of Class III Malocclusion and Maxillary Deviation in Unilateral Cleft Lip and Palate Using Maxillary Anterior Segmental Distraction Osteogenes.

We report the successful treatment of an adolescent with unilateral cleft lip and palate using asymmetric maxillary anterior segmental distraction osteogenesis (MASDO). The patient presented with severe skeletal Class III malocclusion, retruded maxilla, and leftward midline deviation. MASDO effectively addressed both anteroposterior and transverse discrepancies without compromising speech function. The space created by MASDO facilitated prosthetic restoration and mesial movement of maxillary molars, with distractors serving as bone anchorage to prevent reactive effects. As a result, good facia…

Data-driven surgical planning for cleft lip repair: Optimization of individualized incisions using 3D finite element simulation and clinical translational validation.

Straight-line closure is a fundamental technique for repairing cleft lip (CLP), particularly suitable for mild unilateral clefts, valued for its simplicity and natural scar alignment. Understanding the biomechanical impact of surgical design, specifically incision area, is crucial for outcomes. This study utilized patient-specific 3D finite element (FE) modeling based on CT data to simulate the mechanical responses during and after straight-line repair with varying excision areas. Simulations analyzed stress/strain distributions, tissue deformation, displacement of key anatomical points (e.g.,…
