Hepatitis E: when to treat and how to treat.

HEV generally causes a self-limited acute infection and treatment remains supportive. However, severe hepatitis or fulminant hepatic failure may occur, more so during pregnancy. It is an important cause of acute-on-chronic liver failure in endemic areas. Chronic HEV infection and progressive disease has been reported in recipients of solid organ transplants, haematological malignancies, HIV patients and those on haemodialysis. Clearance of HEV may occur after reducing immunosuppressive therapy, especially those targeting T-cells, in about one third of cases. Antiviral therapy should be conside…

The nuclear receptor FXR regulates hepatic transport and metabolism of glutamine and glutamate.

Hepatic transport and metabolism of glutamate and glutamine are regulated by intervention of several proteins. Glutamine is taken up by periportal hepatocytes and is the major source of ammonia for urea synthesis and glutamate for N-acetylglutamate (NAG) synthesis, which is catalyzed by the N-acetylglutamate synthase (NAGS). Glutamate is taken up by perivenous hepatocytes and is the main source for the synthesis of glutamine, catalyzed by glutamine synthase (GS). Accumulation of glutamate and ammonia is a common feature of chronic liver failure, but mechanism that leads to failure of the urea …

Mechanical bridge to transplantation with the Vienna heart in TAH and LVAD configuration.

The Vienna heart uses a vacuum formed, pellethane pulsatile ventricle and is available in left ventricular assist (LVAD) and total artificial heart (TAH) configurations. This device was used as mechanical support of the failing heart in nine patients intended for heart transplantation. In two patients with cardiomyopathy an orthotopic TAH was implanted; one survived despite severe preoperative ischemic liver damage, and the other died of sepsis. In seven patients an atrio-aortic LVAD was implanted; six had suffered an acute myocardial infarction with cardiogenic shock, and one could not be wea

Human umbilical cord mesenchymal stem cell exosomes promote liver regeneration by inducing CD206+ macrophage polarization and suppressing ZBP1 PANoptosis.

Post-hepatectomy liver failure (PHLF) is a serious complication after partial hepatectomy without approved therapies to safely enhance liver regeneration. This study investigated whether exosomes derived from human umbilical cord mesenchymal stem cells promote liver regeneration by regulating a macrophage-transforming growth factor-beta (TGF-β)-ZBP1-PANoptosis axis. Using a 70% partial hepatectomy mouse model and a macrophage-hepatocyte co-culture system, we found that exosome treatment preserved liver structure, reduced serum injury markers (ALT/AST), increased liver-to-body weight ratio by 3…

Immune cells in liver transplantation: Dual roles, mechanistic insights, and targeted therapeutic strategies.

Liver transplantation is the most effective treatment for end-stage liver disease and acute liver failure; however, organ shortage and transplant rejection remain key challenges limiting its development. In recent years, the unique role of immune cells in liver transplantation has provided new insights into the mechanisms of immune rejection and tolerance. The liver's unique immune microenvironment, dominated by innate immune cells, maintains immune tolerance through complex cell-cell interactions and cytokine networks. This article systematically reviews the dynamic roles of macrophages, neut…

BRISC inactivation alleviates alcohol-induced liver injury in mice.

BRCC3 isopeptidase complex (BRISC) is a JAMM subfamily deubiquitinase that has been revealed to be required for optional activation of NLRP3 inflammasome and TLR4/NF-κB signaling pathway. BRISC plays an important role in lipopolysaccharide (LPS)/D-galactosamine-induced acute liver failure, while its functional contribution to alcoholic liver disease (ALD) is still unclear. In this study, we found that the expression of BRISC components was increased in liver tissues of alcoholic hepatitis (AH) animal models and patients with AH. Mice lacking either the scaffold subunit ABRO1 or the catalytic s…

Bile Duct Targeting or Preservation: Contrasting Liver Histology in Langerhans Cell Histiocytosis and Disseminated Juvenile Xanthogranuloma.

Liver involvement by histiocytic and dendritic cell neoplasms signals high-risk disease, often necessitating closer monitoring and aggressive management. Severe cases may progress to liver failure, requiring transplantation. Liver involvement occurs in about one-third of patients with systemic juvenile xanthogranuloma (JXG) and 20% to 60% of pediatric patients with Langerhans cell histiocytosis (LCH), particularly in multiorgan disease. Tyrosine kinase inhibitors show promise in LCH treatment, but optimal timing for treatment cessation remains uncertain. We present 2 pediatric cases, 1 with LC…

Growth differentiation factor 15 is dispensable for acetaminophen-induced liver injury in mice.

Acetaminophen (APAP)-induced liver injury (AILI) has been recognized as a pivotal contributor to drug-induced liver failure in Western countries, but its molecular mechanism remains poorly understood. Growth differentiation factor 15 (GDF15) is a pleiotropic factor that alleviates non-alcoholic liver steatohepatitis, liver fibrosis and liver injury. The aim of the present study was to examine the possibility whether GDF15 confers protection against AILI. We found that the gene expression of Gdf15 was increased significantly after APAP overdose in mice. Next, the role of Gdf15 in AILI was evalu…

Acute hepatitis-like presentation with cholestasis of CBFB-MYH11-positive acute myeloid leukemia in an adult male: a case report.

BACKGROUND: Liver involvement in adults with acute myeloid leukemia is uncommon. Most of the case reports describe acute liver failure or obstructive jaundice, while acute hepatitis is rarely mentioned. We report a patient with acute myeloid leukemia who presented with clinical, biochemical, and radiological signs of acute hepatitis that totally regressed after chemotherapy. CASE PRESENTATION: A 38-year-old Caucasian man presented with fever, cough, and mild fatigue. Laboratory workup showed anemia, thrombocytopenia, severe leukocytosis, transaminitis, and hyperbilirubinemia. Imaging of the a…

Something old, something new: liver injury associated with total parenteral nutrition therapy and immune checkpoint inhibitors.

Drug-induced liver injury (DILI) is a challenging and constantly changing field. The pathologist plays a key role in interpreting liver biopsies by classifying the pattern of injury, grading the severity of injury, and evaluating for other possible causes. Reports of iatrogenic liver injury are reviewed here with a focus on total parenteral nutrition (ie, intestinal failure-associated liver disease [IFALD]) and immune checkpoint inhibitors (ICIs). The hallmark features of IFALD are cholestasis and steatosis. Cholestasis is more common in infants, whereas steatosis and steatohepatitis are more

Hepatic sinusoidal obstruction after gemtuzumab ozogamicin (Mylotarg) therapy.

Gemtuzumab ozogamicin (Mylotarg) targets leukemia cells expressing the CD33 receptor by means of a monoclonal antibody conjugated to a cytotoxic agent, calicheamicin. Treatment of acute myeloid leukemia (AML) with gemtuzumab ozogamicin may result in liver injury. We reviewed the course of 23 patients who were given gemtuzumab ozogamicin for AML that had relapsed after hematopoietic cell transplantation. Liver toxicity was assessed through physical examination, serum tests, histologic examination, and hepatic venous pressure measurements. Liver injury developed in 11 patients after gemtuzumab o

Hepatic veno-occlusive lesions in severe alcoholic hepatitis and alcoholic liver cirrhosis: a comparative histopathological study in autopsy cases.

Clinicopathological features of veno-occlusive lesions in hepatic veins were studied in autopsy cases of severe alcoholic hepatitis (15 cases) and alcoholic liver cirrhosis (15 cases). All the cases were heavy drinkers and died of liver failure or variceal rupture. The frequency and degree of veno-occlusive lesions, and the diameter and number of hepatic veins were studied from stained sections of liver blocks from each case. The hepatic veins observed ranged from 60 to 3000 microm in diameter. The veno-occlusive lesions were found in hepatic veins mainly 60 to 1200 microm in diameter. These f…

Liver abnormalities associated with celiac sprue. How common are they, what is their significance, and what do we do about them?

We prospectively measured serum alkaline phosphatase (ALP), aspartate and alanine transaminase (AST/ALT), and tested sera for antinuclear, smooth-muscle, and antimitochondrial antibodies (ANA, SMA, AMA) in our patients with celiac sprue to determine the prevalence of associated liver abnormalities and its relevance to clinical management. Of 129 patients, ALP was the only elevated enzyme in 12 (9%) and in most cases was not thought to reflect significant liver disease. Seventeen (13%) had elevated AST and/or ALT with normal ALP. Levels normalized in 15 patients after dietary gluten exclusion a

[A death caused by fulminant B-hepatitis which was latent preoperatively].

It is not always easy to evaluate the preoperative liver functions exactly. In this paper we report a patient who died of fulminant hepatitis postoperatively in spite of careful examination of preoperative liver functions. A 60-year-old male had severe bleeding following total cystectomy, resulting in cardiac arrest. He was resuscitated successfully but developed multiple organ failure. He came through it but thereafter he had repeated infectious episodes from uretero-cutaneous fistula and fluctuation of liver functions. Seven months later he had repair of postoperative ventral hernia. His liv

Impaired Fatty Liver Regeneration Post-major Resection: A Mitochondrial Problem.

Fatty liver disease is associated with a markedly increased risk of liver dysfunction and death after major hepatectomy (PHx). We previously demonstrated impaired hepatocyte proliferation, delayed liver regeneration, and increased mortality in post-hepatectomy murine fatty liver. However, the underlying mechanism(s) remain unclear. In this study, we sought to define the mechanisms underlying fatty liver regenerative failure following resection. The hepatic transcriptome was analyzed after 70% or 80% hepatectomy in lean and diet-induced obese (DIO) mice. A gene array analysis was conducted. Hum
