The importance of recognizing and managing a rare form of angioedema: hereditary angioedema due to C1-inhibitor deficiency.

The majority of angioedema cases encountered in clinical practice are histamine-mediated (allergic); however, some cases are bradykinin-related (non-allergic) and do not respond to standard anti-allergy medications. Among bradykinin-related angioedema, hereditary angioedema (HAE) is a rare, but chronic and debilitating condition. The majority of HAE is caused by deficiency (type 1) or abnormal function (type 2) of the naturally occurring protein, C1-inhibitor (C1-INH)-a major inhibitor of proteases in the contact (kallikrein-bradykinin cascade), fibrinolytic pathway, and complement systems. The microbiology and the efficacy of antibiotic-based medical treatment of chronic rhinosinusitis in Singapore.

BACKGROUND: Medical therapy including appropriate antibiotic treatment is advocated for the management of chronic rhinosinusitis (CRS), with sinus surgery reserved for treatment failures. This study investigates the microbiology of CRS and their response to culture-directed antibiotic treatment. METHODS: Sinus aspirates of mucopus from 172 consecutive CRS patients, with (n=89) and without (n=83) previous antibiotic treatment, were obtained for bacterial culture at their first visit. Medical treatment which included initial empirical and subsequent culture-directed antibiotics was instituted.

Ultrasound-Guided Stellate Ganglion Block for the Treatment of Chronic Migraine in Adults: Study Protocol for a Randomized, Open-Label, Blinded Endpoint Trial.

BACKGROUND: Chronic migraine (CM) is a recurring disorder with a relatively poor prognosis. Microinvasive and nonpharmacological therapies are essential for refractory patients with this condition. To date, only nonrandomized trials have reported the efficacy of the stellate ganglion block (SGB) for CM patients. Whether a combination of SGB and drug therapy is an optimal treatment for CM patients must still be confirmed with randomized controlled studies utilizing standardized oral medications as a control. OBJECTIVE: To assess the efficacy and safety of the repeated administration of SGBs ov

Primary cutaneous B-cell lymphomas: part II. Therapy and future directions.

The choice of therapy for primary cutaneous B-cell lymphoma (PCBCL) relies on correct histopathologic classification and the exclusion of systemic disease. In part II of this continuing medical education article, we will review the available therapies for the different types of PCBCL. Primary cutaneous follicle center lymphoma (PCFCL) and primary cutaneous marginal zone lymphoma (PCMZL) are indolent tumors with an excellent prognosis. They are managed similarly with local therapy, such as radiotherapy or surgical excision, for isolated disease and observation for asymptomatic multifocal presen

Antithrombotic therapy: triple therapy or triple threat?

Antithrombotic therapy plays an essential role in the management of some of the most common and morbid medical conditions. Triple oral antithrombotic therapy (TOAT) is defined as the administration of both therapeutic oral anticoagulation (OAC) and dual antiplatelet therapy (DAPT) to patients with indications for both treatments. The current societal guidelines regarding TOAT are derived from observational studies and some trials of the use of warfarin in addition to antiplatelet therapy in patients with atrial fibrillation and a recent acute coronary syndrome or percutaneous coronary interven

Botulinum toxin-A for the treatment of hemifacial spasm.

Management of hemifacial spasm can actually be done medically, surgically and with Botulinum-A Toxin. The Botulinum-A Toxin treatment locally injected into the involved facial muscles offers a useful alternative to medical and surgical therapy. The objective of this study was to evaluate the efficacy of Botulinum-A Toxin for the treatment of hemifacial spasm in those subjects for whom the presently available medical therapy is inadequate. A total of 28 individuals were enrolled in the clinical study. Patients were evaluated using the Fahn's blepharospasm rating and disability scales. Efficacy

Intrahepatic amebic abscesses: indications for and results of percutaneous catheter drainage.

Medical therapy is standard for intrahepatic amebic abscess and generally is effective. However, we have encountered a group of patients in whom percutaneous aspiration and drainage was indicated due to uncertainty of diagnosis or clinical deterioration of the patient. Twenty such patients underwent percutaneous drainage with ultrasound or CT guidance, and each patient was cured (appropriate antibiotics were administered concomitantly). The specific indications for intervention were to differentiate pyogenic from amebic abscess, pain and imminent rupture, poor response to medical therapy, fals

Narrative review of the current management of ectopic varices: an update.

BACKGROUND AND OBJECTIVE: Ectopic varices are a rare complication of portal hypertension that occur outside the esophagogastric region and represent a potentially life-threatening source of gastrointestinal bleeding. Treatments are frequently guided by institutional experience rather than standardized treatment algorithms due to their rarity and variable anatomy. This narrative review summarizes the epidemiology, clinical presentation, diagnostic evaluation, and management strategies. METHODS: A literature review was performed using PubMed and Google Scholar. Given that much of the available

Psychological therapies for thalassaemia.

BACKGROUND: Thalassaemia is a group of genetic blood disorders characterised by the absence or reduction in the production of haemoglobin. Severity is variable from less severe anaemia, through thalassaemia intermedia, to profound severe anaemia (thalassaemia major). In thalassaemia major other complications include growth retardation, bone deformation, and enlarged spleen. Blood transfusion is required to treat severe forms of thalassaemia, but this results in excessive accumulation of iron in the body (iron overload), removed mostly by a drug called desferrioxamine through 'chelation therapy

Depigmentation Outcomes in Pigmented Oral Lichen Planus Managed With Mycophenolate Mofetil.

Pigmented oral lichen planus (OLP) is a chronic, immune-mediated mucosal disorder associated with dark black or brown discoloration secondary to the excess melanin deposition. This medical condition is often accompanied by the typical white (reticular) or red (erosive) OLP features and is refractory to conventional therapy. We report a case of OLP which was successfully managed with oral mycophenolate mofetil (MMF), resulting in significant symptomatic relief and reduction of hyperpigmentation. MMF offers a safe and effective systemic option for persistent pigmented OLP.

SEOM-TTCC clinical guideline for nasopharyngeal carcinoma (update 2025).

Nasopharyngeal carcinoma is distinct from other cancers of the head and neck in biology, epidemiology, histology, natural history, and response to treatment. Radiotherapy is the cornerstone of locoregional treatment of non-disseminated disease and, in combination with chemotherapy, improves survival rates. In the case of metastatic disease stages, treatment requires platinum/gemcitabine-based chemotherapy, and patients may achieve a long survival time. In these guidelines (updated in 2025), we summarize current evidence and available therapies for the medical management of advanced nasopharyng

Acute Heart Failure: From The Emergency Department to the Intensive Care Unit.

Acute heart failure (AHF) is a frequent cause of hospitalization around the world and is associated with high in-hospital and post-discharge morbidity and mortality. This review summarizes data on diagnosis and management of AHF from the emergency department to the intensive care unit. While more evidence is needed to guide risk stratification and care of patients with AHF, hospitalization is a key opportunity to optimize evidence-based medical therapy for heart failure. Close linkage to outpatient care is essential to improve post-hospitalization outcomes.

Pain management in hidradenitis suppurativa.

Pain is the most impactful symptom in hidradenitis suppurativa (HS), contributing to physical limitations, worsened mental health, and impaired social relationships. Some HS-directed therapies ameliorate HS pain; however, these medications provide insufficient relief for many people living with HS. This article reviews what is currently known about HS pain phenotypes and pathophysiology, the effectiveness of HS treatments for pain, and practical suggestions for dermatologists to help patients manage the symptoms of acute and chronic HS pain.

Venous Leg Ulcers: The Need to Incorporate Age-Friendly 4M's in Management.

Venous leg ulcers develop due to a complex set of conditions routed in lower extremity edema and inflammation. Even though older adults are disproportionally affected by these wounds, the guidelines discuss best practices without keeping in mind the age-friendly 4M's-what Matters Most, Mobility, Medications, and Mentation/Mood. This article reviews the management and treatment of venous leg ulcers, but with a geriatric medicine focus. Compression therapy, mobility optimization, and social isolation reduction are highlighted for our older adult population.
