[Intensive therapy and resuscitation of heart rhythm disorders in myocardial infarct patients and the role of express methods of controlling respiratory function].

The authors used a complex of express methods to examine respiration and acid-base equilibrium in the unit of intensive care and therapy. Examination by means of these methods during intensive therapy and resuscitation helps in individualizing the therapeutic measures. Various individual disorders of respiration were revealed during electroimpulse therapy in paroxysms of tachyarrhythmia, but the final effect of this treatment in restoration of the sinus rhythm had a beneficial influence on the dynamics of cardiogenic respiratory insufficiency.

Arrhythmias in ambulatory persons. A review and experience of 1,000 consecutive recordings.

The value of 24-hour ambulatory monitoring of the ECG for the diagnosis of symptoms of dizziness, palpitations and syncope is controversial. In this study results from a hospital-based, clinical service have been analysed. Of 1,000 consecutive dynamic electrocardiograms (DCG), 678 were performed for assessment of dizziness, syncope or palpitations in 405 patients. 36 of the patients had pacemakers. 60 DCGs were technically inadequate. The recordings were classified according to the correspondence between DCG findings and symptoms noted in the patient diary: (I) Completely diagnostic: significa

Prospective Preference Assessment of Potential Participants in the Perioperative Rectal Methadone in Spine Surgery (PROMISE) Trial.

BACKGROUND/AIMS: Perioperative pain management in spine surgery is challenging, with many patients suffering from chronic pain. The favorable pharmacokinetic-dynamic profile of methadone may offer a novel approach to perioperative analgesia; however, stigma around methadone and rectal administration may bias patient enrollment. Before initiating a trial comparing rectal methadone to placebo, we conducted a prospective preference assessment (PPA) to identify barriers and motivators for participation and characterize differences between willing and unwilling participants to refine the trial prot

Evaluation of thermal camera measurement stability and factors associated with infrared eye temperature in dogs.

Non-invasive thermal monitoring, such as infrared thermography (IRT), offers a potential alternative to traditional thermometry in veterinary settings. This study aimed to characterize the relationship between maximum eye temperature (ET) and rectal temperature (RT) in dogs. Our field study included 97 observations from 57 shelter dogs whose ET was measured using IRT on 4 separate days. For each observation, 3 consecutive ET measurements were obtained at 10-min intervals, followed by a RT measurement. Device temporal stability was evaluated measurement drift using an insulated box as a control

Determining an optimal central cannulation strategy for ambulatory veno-venous extracorporeal life support.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is the most prevalent chronic end stage lung disease. There are few long-term treatment options for end stage COPD due to donor lung scarcity. Durable artificial lung therapies may provide alternatives for long-term respiratory support. Veno-venous extracorporeal membrane oxygenation can provide physiologic support, but current cannulation configurations are suboptimal for durable support. METHODS: We designed, fabricated, and evaluated four different central cannulation strategies in an acute sheep model of veno-venous extracorporeal m

Predicting pulmonary hemorrhage in very low birth weight infants.

BACKGROUND: Pulmonary hemorrhage (PH) in very low birth weight (VLBW) infants is a catastrophic event with significant mortality. Predicting PH clinically is challenging, with no tools for early detection. Our objective was to identify candidate physiologic biomarkers using heart rate (HR) and pulse oximetry (SpO2) predictive of PH events in VLBW infants. METHODS: We conducted a retrospective case-control study of VLBW infants from two level IV NICUs (2016-2023). We matched infants with PH to controls using gestational age, birth weight, and postnatal age. Taking HR and SpO2 data from 72 hour

Blood pressure trajectories in extreme preterm infants with and without bronchopulmonary dysplasia: a longitudinal cohort study.

OBJECTIVE : To compare longitudinal blood pressure (BP) patterns in extremely preterm (EP) infants with and without bronchopulmonary dysplasia (BPD) and examine associations with clinical characteristics. METHODS: This retrospective cohort study analysed weekly non-invasive systolic, diastolic, and mean BP measurements from 28 weeks postmenstrual age (PMA) to term infants cared for at the Women’s Wellness and Research Center, Doha. BP trajectories were compared longitudinally, with additional analysis by BPD severity. RESULTS: Among 201 infants, 95 (47%) developed BPD. These infants were born

Advancing Scoliosis Treatment with Patient-Specific Functionally Graded NiTi-SMA Rods: Key Considerations and Development Objectives.

This review develops a materials-to-clinic framework for patient-specific, functionally graded (FG) NiTi shape memory alloy (SMA) rods as a complementary paradigm for scoliosis correction that targets durable alignment with motion preservation. The article synthesizes the thermomechanical basis of NiTi (thermoelastic martensitic transformation, near constant superelastic plateau, and hysteretic damping) while leveraging additive manufacturing (AM) capabilities to spatially program transformation temperatures (e.g., Af), effective stiffness, and geometric inertia along the rod. Consolidated pro

Risk factors for post-obstructive diuresis following surgical correction of ureteropelvic junction obstruction in pediatrics: A retrospective cross-sectional study.

BACKGROUND: Post-obstructive diuresis (POD) is a potentially serious complication following surgical correction of ureteropelvic junction obstruction (UPJO) in pediatric patients. Characterized by excessive urine output after decompression of a chronically obstructed renal system, POD may result in significant fluid and electrolyte imbalance. While physiologic POD is transient, pathologic forms require careful management. This study aimed to determine clinical and demographic risk factors associated with POD following UPJO surgery in a pediatric population. METHODS: We conducted a retrospecti

Physiological changes associated with copper sulfate-induced nausea and retching in felines.

Nausea is a common disease symptom, yet there is no consensus regarding its physiological markers. In contrast, the process of vomiting is well documented as sequential muscular contractions of the diaphragm and abdominal muscles and esophageal shortening. Nausea, like other self-reported perceptions, is difficult to distinguish in preclinical models, but based on human experience emesis is usually preceded by nausea. Here we focused on measuring gastrointestinal and cardiorespiratory changes prior to emesis to provide additional insights into markers for nausea. Felines were instrumented to c

AARC Clinical Practice Guidelines: Capillary Blood Gas Sampling for Neonatal and Pediatric Patients.

In the absence of an indwelling arterial catheter, capillary blood gas sampling may be used to evaluate the acid/base and ventilation status of neonatal and pediatric patients with cardiorespiratory conditions. These guidelines were developed from a comprehensive review of the literature to provide guidance for the collection, handling, and interpretation of blood obtained from an arterialized capillary sample. Capillary and venous blood gas measurements are a useful alternative to arterial blood gas measurements for neonatal and pediatric patients who do not require close monitoring of [Formu

Portomesenteric venous thrombosis in a postmenopausal female with testosterone implant: a case report.

BACKGROUND: Acute portal vein thrombosis is a rare medical event usually seen in liver disease, but it can also occur due to any inherited or acquired procoagulable state that triggers venous occlusion. Hormonal therapies have been associated with an increased risk of prothrombotic states. This case report documents a portomesenteric venous thrombosis in a postmenopausal woman with testosterone implant for the treatment of hypoactive sexual desire and discusses the importance of identifying hypercoagulable risk factors before initiating hormone replacement therapy. We want to improve the aware

A Swine Model of Severe Propranolol Toxicity Permitting Direct Measurement of Brain Tissue Oxygenation.

INTRODUCTION: High-dose insulin (HDI) therapy has been used successfully for beta-blocker toxicity, but needs further study when hypotension persists despite HDI. The objective was to develop a model of propranolol toxicity with persistent hypotension despite HDI and to develop means to measure cerebral oxygen tension (PbrO2). METHODS: Eight anesthetized Yorkshire pigs were instrumented with a tracheostomy, Swan-Ganz catheter, arterial catheter, and intra-cerebral pressure and oxygen monitor. Intravenous propranolol was given until the initial point of toxicity (POT); 25% reduction from basel
