Autoimmune disease preceding amyotrophic lateral sclerosis: an epidemiologic study.

OBJECTIVE: To study whether the risk of amyotrophic lateral sclerosis (ALS) is increased in people with prior autoimmune disease. METHODS: … osis of asthma, celiac disease, younger-onset diabetes (younger than 30 years), multiple sclerosis, myasthenia gravis, myxedema, polymyositis, Sjögren syndrome, systemic lupus erythematosus, and ulcerative colitis. CONCLUSIONS: Autoimmune disease associations with ALS raise the possibility of shared genetic or environmental risk factors.

Intravenous immunoglobulin for treatment of neuromuscular disease.

Intravenous immunoglobulin (IVIg) has been widely used in the treatment of autoimmune neuromuscular diseases. Compared to other treatment modalities, such as corticosteroids and chemotherapy for autoimmune disorders, IVIg has relatively few side effects and favorable therapeutic outcomes in certain neuromuscular diseases. There is Class I evidence for IVIg as an initial treatment for patients with Guillain-Barré syndrome (GBS), chronic inflammatory demyelinating polyneuropathy (CIDP), and multifocal motor neuropathy. It is as effective as plasma exchange in GBS and CIDP. In myasthenia gravis, …

[Molecular targeted therapy for neuroimmunological diseases].

Developments in using monoclonal antibodies for molecular targeted therapy have demonstrated promising efficacy for various neuroimmunological diseases, including multiple sclerosis, neuromyelitis optica, myasthenia gravis, chronic inflammatory demyelinating polyneuropathy, and polymyositis. Natalizumab was the first monoclonal antibody to be approved for the treatment of multiple sclerosis in countries outside of Japan. Rituximab is widely used for the treatment of neuromyelitis optica, even though it has not received FDA approval for this condition anywhere in the world. Monoclonal antibodie…

The co-existence of myasthenia gravis in patients with myositis: a case series.

OBJECTIVE: Myositis and myasthenia gravis (MG) are both autoimmune disorders presenting with muscle weakness. Rarely, they occur simultaneously in the same patient. Since the management of myasthenia gravis differs from that of myositis, it is important to recognize when patients have both diseases. We reviewed the cases of 6 patients with both myositis and MG to identify clinical features that suggest the possibility of co-existing MG in myositis patients. METHODS: We identified 6 patients with dermatomyositis or polymyositis and MG. We reviewed their medical records to assess their clinical…

A survival case of invasive thymoma accompanied by acute fulminant myocarditis.

Thymomas are associated with a wide spectrum of autoimmune paraneoplastic diseases. Here we report the case of 31-year-old male with invasive thymoma, myasthenia gravis, polymyositis, and acute fulminant myocarditis that presented with cardiogenic shock requiring intra-aortic balloon pumping and percutaneous cardiopulmonary support. Corticosteroid therapy was effective. To our knowledge, this is the first case of thymoma with acute fulminant cardiomyositis that was successfully treated by assisted circulation and corticosteroids, despite a poor prognosis.

Anterior mediastinal lymphoma arising after resection of an invasive thymoma and immunosuppressive therapy for complicated myasthenia gravis.

We present the case of a 56-year-old woman with an anterior mediastinal tumor who has past history of myasthenia gravis and invasive thymoma. Furthermore, she had superior vena cava syndrome that was caused by a rapidly growing tumor. A biopsy proved diffuse large B-cell lymphoma. After 8 courses of chemotherapy, remission of the lymphoma was achieved. Because a second primary malignancy, including lymphoma, can occur in patients with thymoma, a biopsy is necessary for tumors located in the anterior mediastinum, particularly in patients with a history of treatment for thymoma, to distinguish b…

[A case of myasthenia gravis following sarcoidosis and rheumatoid arthritis].

We report an elderly woman with sarcoidosis and rheumatoid arthritis who subsequently developed myasthenia gravis. She was given a diagnose of rheumatoid arthritis at the age of 65 years and sarcoidosis, proved by multiple lymphadenopathy with noncaseating granuloma at the age of 67. Prednisolone, methotrexate, and etanercept had been administrated for rheumatoid arthritis. She consulted our hospital because of bilateral ptosis with diurnal fluctuation at the age of 72. Myasthenia gravis was confirmed by an elevated serum anti-acetylcholine receptor antibody titer (1,100 nmol/l, normal <0.2) a…

Hypokalemic periodic paralysis induced by thymic hyperplasia and relieved by thymectomy.

IMPORTANCE: Hypokalemic periodic paralysis is a muscle channelopathy based on mutations or predisposing variants or secondary to potassium wasting. In contrast to myasthenia gravis, an association with thymic hyperplasia has not yet been reported, to our knowledge. OBSERVATIONS: We report a male patient in his mid-20s with progressive episodes of flaccid muscle weakness, associated low serum potassium levels, and a pathologic decrement in the long exercise test. Because the familial inheritance in the family was initially unknown, thorough diagnostic tests were performed including contrast-en…

Thymoma associated with hypogammaglobulinaemia and pure red cell aplasia.

Thymomas are neoplasias that begin in the thymus and develop in the anterior mediastinum. They are commonly associated with a variety of systemic and autoimmune disorders, such as pure red cell aplasia, hypogammaglobulinaemia, pancytopaenia, collagen diseases, and, most commonly, myasthenia gravis. The presence of inter-current infections, especially diarrhoea and pneumonia, in the presence of lymphocyte B depletion and hypogammaglobulinaemia is known as Good's syndrome and may affect up to 5% of patients with thymoma. While anaemia is present in 50%-86% of patients with Good's syndrome, only …

Pemphigus: associations and management guidelines: facts and controversies.

Pemphigus, a prototypical organ-specific human autoimmune disease, may be associated with other immunity-related disorders, viral infections, and different types of tumors. Coexistence with immune diseases is fairly frequent and, for some of them (eg, myasthenia gravis, Basedow's disease, rheumatoid arthritis, or lupus erythematosus), common pathogenic mechanisms can be considered. The association with viral infections (mainly herpesvirus infections) raises the question of whether the virus triggers the outbreak of the disease or simply complicates its clinical course. Neoplastic proliferation…

Long-term follow-up of strabismus surgery for patients with ocular myasthenia gravis.

BACKGROUND: Patients with ocular myasthenia gravis (OMG) may develop strabismus and diplopia. We completed a retrospective observational case series to evaluate long-term surgical outcomes in patients with OMG. METHODS: The medical records of all patients with OMG who underwent strabismus surgery with at least 6 months of postoperative follow-up were reviewed. Nine patients met the study inclusion criteria. The main outcomes, including ocular alignment, number of surgeries, and sensory status were evaluated. RESULTS: Of these patients, initially 2 had horizontal strabismus alone, 3 had verti…

Progressive external ophthalmoplegia (PEO) due to a mutation in the C10orf2 (PEO1) gene mimicking a myasthenic crisis.

We described a case of a patient with autosomal dominant progressive external ophthalmoplegia (PEO) who presented with the acute onset dysphagia, quadriparesis, ptosis and respiratory insufficiency following a cardiac procedure and mimicking a myasthenic crisis. A pathogenic mutation in the C10orf2 (PEO1) gene was confirmed. The unusual presentation of our patient contributes to expand the clinical phenotype of PEO1 mutations and reinforces the need to consider mitochondrial myopathy as differential diagnosis of myasthenia gravis even in the case of acute onset symptoms.

Dyshidrotic eczema associated with the use of IVIg.

Intravenous immunoglobulin (IVIg) treatment is highly effective for autoimmune diseases including myasthenia gravis. Recovery is observed at approximately. 75% of myasthenia gravis patients through IVIg treatment. As a result of many clinical studies, the recommended dose is determined as 0.4 g/kg for 5 days (maximum total dose at 2 g/kg body weight). If an additional immunomodulatory treatment is not administered, IVIg maintenance treatment is needed mostly. However, some side effects may inhibit long-term treatment. For this reason, it is important to know the effect profile well and when th…
