Current and future care of patients with the cancer anorexia-cachexia syndrome.

Many important advances have occurred in the field of cancer cachexia over the past decade, including progress in understanding the mechanisms of the cancer anorexia-cachexia syndrome (CACS) and the development of promising pharmacologic and supportive care interventions. However, no approved agents for cancer cachexia currently exist, emphasizing the unmet need for an effective pharmacologic therapy. This article reviews the key elements of CACS assessment in daily practice, the contribution of nutritional impact symptoms (NIS), the evidence for current pharmacologic options, and promising an

Hepatic infarction and acute liver failure in children with extrahepatic biliary atresia and cirrhosis.

Acute hepatic failure developed in four patients (aged 7 to 13 months) who had extrahepatic biliary atresia treated initially by portoenterostomy. Two were stable outpatients with minimal jaundice, while the other two were hospitalized for metabolic or nutritional complications. Postmortem examination in each patient revealed massive acute hepatic infarction with few surviving hepatocytes. In all cases, the hepatic failure had been preceded by an episode of hypotension and/or hypovolemia. The exact pathogenesis of the infarction remains unclear but it may be related to decreased hepatic blood

Evaluation of Dietary Supplementation with a Blend of Organic Acids and Cinnamaldehyde on Performance and Health Status in Weaned Piglets.

The post-weaning period represents a critical phase in swine production, often associated with increased disease susceptibility, impaired growth performance, and a consequent high antibiotic use. Therefore, innovative nutritional strategies based on functional feed additives are gaining increasing attention as strategies to support intestinal health and reduce the need for antimicrobials. This study evaluated the effects of dietary supplementation with a blend of organic acids and cinnamaldehyde on growth performance, health status, and antibiotic use in weaned piglets. A total of 148 piglets

Combined leucine supplementation and exercise to counteract sarcopenia in patients with end-stage kidney disease undergoing maintenance hemodialysis: a single-center randomized pilot study.

BACKGROUND: Sarcopenia affects approximately 30%-40% of patients with end-stage kidney disease (ESKD) undergoing maintenance hemodialysis (HD), a prevalence substantially higher than that observed in community-dwelling older adults. Muscle wasting in this population is driven by chronic inflammation, amino acid losses during dialysis, and anabolic resistance, which blunt muscle protein synthesis despite nutritional intake or exercise. Leucine, a branched-chain amino acid that activates mechanistic target of rapamycin complex 1 signaling, plays a key role in muscle anabolism but is often deplet

A cacao by-product ingredient prevents locomotor circadian disruption due to high-fat diet administration by regulating clock genes.

Cacao by-products are frequently discarded as waste despite representing a sustainable source of bioactive compounds, including dietary fiber and phenolic constituents. A cacao by-product ingredient (CBI), designed based on in silico predictions, has been proposed to contain components capable of modulating core clock proteins. Although high-fat diets (HFD) are known to disrupt circadian organization, the potential of CBI to counteract HFD-induced circadian alterations remains unexplored. This study aimed to evaluate the effects of CBI on circadian disruption induced by HFD consumption. Four e

Optimizing perioperative care in esophagectomy: a narrative review.

BACKGROUND AND OBJECTIVE: Perioperative management of esophageal cancer surgery is continuously evolving. Advances in patient preparation, surgical techniques, and the implementation of enhanced recovery after surgery (ERAS) protocols have collectively improved postoperative outcomes and recovery. Minimally invasive esophagectomy has been widely adopted due to its association with reduced surgical trauma, less blood loss, shorter hospital stay, and, in some studies, lower complication rates. Despite these advances, esophagectomy remains a high-risk procedure with substantial morbidity, undersc

From environmental perception to reproductive allocation: the microbiome-membrane-mitochondria axis in poultry.

Reproductive performance has traditionally been interpreted through the physiology of reproductive organs, endocrine regulation, and genetics. Although these approaches have substantially advanced poultry production, they do not fully explain why reproduction consistently reflects the overall physiological condition of the organism. This review proposes an integrative conceptual framework in which reproductive success emerges from the coordinated activity of the entire host-microbiome ecosystem, not from the reproductive system alone. We examine current evidence supporting the concept that the

Selenium nutritional status and cardiovascular diseases: mechanisms of cardioprotection and 21 years of global research trends in precision prevention (2005-2026).

BACKGROUND: Selenium contributes to selenoprotein-dependent redox regulation, thyroid hormone metabolism, and cellular defense, but its role in cardiovascular disease remains clinically uncertain. Observational studies have associated low or atypical selenium status with adverse cardiovascular outcomes, whereas supplementation trials have generally produced neutral or inconclusive results. This study mapped the development of selenium-cardiovascular research and integrated the identified themes with a clinically oriented narrative synthesis of selenium exposure, functional biomarkers, cardiova

Prophylactic vs. reactive percutaneous endoscopic gastrostomy (PEG) tubes in head and neck cancer: a meta-analysis of survival and supportive care outcomes.

PURPOSE: The optimal timing of percutaneous endoscopic gastrostomy (PEG) tube insertion in head and neck cancer (HNC) remains controversial. While prophylactic PEG placement may prevent treatment-related malnutrition, its true clinical value compared with a reactive strategy is unclear. This systematic review and meta-analysis aimed to evaluate whether prophylactic PEG offers advantages in PEG tube dependence, hospitalization rates, and overall survival among patients undergoing chemoradiotherapy for HNC. METHODS: A comprehensive search of PubMed, Scopus, and Web of Science was conducted thro

Effects of Malnutrition on In-Hospital and Discharge Outcomes Among Young Adults Hospitalized with Gastrointestinal Cancers: National Estimates from the United States.

PURPOSE: Protein-energy malnutrition (PEM) is common in gastrointestinal (GI) cancers and may worsen inpatient outcomes. Contemporary national data describing the impact of PEM among young adults with GI malignancies are limited. METHODS: We conducted a retrospective cohort study using HCUP NIS data from 2018 to 2021. We identified hospitalizations of adults aged 18 to 39 years with GI cancers using ICD 10 CM codes C15 to C26. We defined PEM using ICD-10-CM diagnosis codes recorded during the index hospitalization; therefore, PEM reflects clinically documented/coded malnutrition rather than t

Environmental enteric dysfunction in well-nourished and undernourished women in Bangladesh.

INTRODUCTION: Environmental Enteric Dysfunction (EED) is a subacute inflammation of the small intestinal (SI) mucosa associated with undernutrition and perturbation of the SI microbiota. Its diagnosis relies on histopathological assessment of small intestinal biopsies, with a lack of validated biomarkers contributing to underdiagnosis. This study assessed the prevalence and histological features of EED in undernourished Bangladeshi women living in an urban slum in Dhaka, compared to well-nourished women from slum and non-slum areas, and examined the factors associated with histologically confi

Muscle ultrasonography in costello syndrome: unveiling new clinical insights of a complex muscular phenotype.

BACKGROUND: Costello syndrome (CS) is a rare genetic disorder within the spectrum of RASopathies, caused by activating mutations in the HRAS gene, leading to constitutive dysregulation of the RAS/MAPK signalling pathway. Among its multisystemic manifestations, a distinctive musculoskeletal involvement is frequently observed with reduction in muscle force, pain of musculoskeletal origin and muscular hypotrophy. Moreover, abnormal histological findings with variability in size, atrophy, and prevalence of type 2-fibers have been detected on anecdotal muscular biopsies of affected individuals. ME
