Hypothermic Oxygenated Perfusion in Extended Criteria Donor Kidney Transplantation-A Randomized Clinical Trial.

BACKGROUND: The role of machine perfusion after kidney transplantation (KT) in extended criteria donors (ECD) is unclear, and the current evidence in the literature remains controversial. METHODS: We present an open-label single center randomized trial where 109 patients undergoing KT with ECD grafts between January 2019 and December 2022 were randomized to receive kidneys treated with either hypothermic oxygenated perfusion (HOPE, n = 54) or static cold storage (SCS, n = 55) alone. The primary endpoint was the incidence of delayed graft function (DGF). The secondary endpoints included postop

Effect of Positioning on Work of Breathing and Oxygenation in Premature Infants at Discharge: A Prospective Observational Study.

INTRODUCTION: Premature infants are predisposed to respiratory failure. Body position impacts lung volumes and pulmonary function. Respiratory inductance plethysmography (RIP) measures thoracoabdominal motion and can provide objective, noninvasive diagnostic measurements of work of breathing (WOB) indices. The objective of this study was to compare WOB indices and oxygen saturation in the semi-reclined position to the supine position for preterm infants with and without BPD at discharge. METHODS: A prospective, observational study of premature infants (<32 weeks of gestation) admitted to the

Pharmacokinetics of levosimendan in critically Ill children on extracorporeal membrane oxygenation: a prospective observational study.

BACKGROUND: Levosimendan is used off-label in pediatrics and pharmacokinetic (PK) data in this population are scarce. The only study in critically ill patients on extracorporeal membrane oxygenation (ECMO) showed altered PK parameters. Our study aimed to characterize the PK profile of levosimendan and its metabolites in critically ill children on ECMO and assess the adequacy of current dosing practices. METHODS: We conducted a prospective observational PK study in a Swiss tertiary pediatric intensive care unit. Children on ECMO for hemodynamic failure receiving continuous levosimendan infusio

Region-Dependent Responses to Oxygen-Glucose Deprivation and Melatonin in Neonatal Brain Organotypic Slices.

Hypoxic-ischemic encephalopathy remains a major cause of neonatal mortality and long-term neurological disability. Therapeutic hypothermia is currently the only available treatment in hospitals, but its efficacy is limited, making the search for alternative neuroprotective strategies essential. Melatonin has shown promising results in other models of hypoxia-ischemia, acting as a potent antioxidant and anti-inflammatory molecule. Here, we studied the effects of hypoxia-ischemia and melatonin treatment in two brain regions that are particularly vulnerable to hypoxic-ischemic injury. Neonatal ra

In-Situ Hemodynamics and Oxygen Utilization in Right Lobe Grafts During Living Donor Hepatectomy.

BACKGROUND: Several studies have examined liver graft hemodynamics during living donor liver transplants; however, to the best of our knowledge, the hemodynamics of the partial graft during donor hepatectomy have not been thoroughly explored. We analyzed intraoperative graft hemodynamics and oxygen utilization data following parenchymal dissection during living donor hepatectomy. METHODS: The following parameters were measured intraoperatively in 63 right hepatic lobe living donor hepatectomy cases: right portal vein (RPV) and right hepatic artery (RHA) blood flow after parenchymal transectio

Automated oxygen control in preterm babies on respiratory support: protocol for a randomised crossover trial.

INTRODUCTION: Respiratory support is frequently needed for babies admitted to the neonatal intensive care unit. Among them, preterm babies are most likely to have issues of respiratory distress, and they may need invasive or non-invasive breathing support. Providing respiratory support, keeping the oxygen saturation (SpO2) in the target range (TR) and preventing abnormal high and low oxygen levels should be the aim of providing respiratory therapy. Usually, this control is achieved by manual adjustment of FiO2 (fraction of inspired oxygen) by bedside staff nurses to keep SpO2 in TR. However, t

Improving sustainable isopropanol production in engineered Escherichia coli W via oxygen limitation.

BACKGROUND: Due to ecological concerns, alternative supply lines for fuel and bulk chemicals such as isopropanol are increasingly pursued. By implementing the formation pathways from natural producers like Clostridium beijerinckii and Clostridium aurantibutyricum, isopropanol can be produced in Escherichia coli. However, developing an industrially and economically feasible microbial production process requires a robust and efficient process strategy. Therefore, this study explores microaerobic conditions in combination with lactose and sour whey as sustainable carbon source as a basis for larg

Defying the Odds: Survival After Amniotic Fluid Embolism and Extracorporeal Membrane Oxygenation Support.

BACKGROUND: Amniotic fluid embolism is a rare potential complication in the peripartum period. CASE SUMMARY: A 27-year-old nulliparous woman presented for elective induction at 41 weeks' gestation. Due to nonreassuring fetal heart tracing, the patient underwent emergent caesarean section complicated by pulseless electrical activity arrest secondary to suspected amniotic fluid embolism after fetus delivery. Despite return of spontaneous circulation, the patient remained in cardiogenic shock with right ventricular failure. The patient was cannulated for venoarterial extracorporeal membrane oxyg

Oxygen during Neonatal Resuscitation: Too Much versus Too Little, Does It Matter?

<p>Background: Oxygen has been a key component of neonatal resuscitation for nearly two centuries. Based on clinical trials that demonstrated worse outcomes when neonatal resuscitation was initiated with 100% oxygen, there was a change in approach to using 21% oxygen at the initiation of ventilation for newborns at birth. However, for extremely preterm newborns, lower oxygen levels lead to early hypoxia and bradycardia, leading to higher rates of severe intraventricular hemorrhage and death. The balance between hyperoxia and hypoxia-related injury needs further refinement and may not be genera

Long-Term Impairment of Retinal Ganglion Cell Function After Oxygen-Induced Retinopathy.

Premature infants with retinopathy of prematurity (ROP) have neovascularization of the retina, potentially resulting in low vision and even blindness. Some of these infants still have visual impairment, even if ROP resolves as they age. However, the mechanisms underlying the visual problems post-ROP are poorly understood. Because the pathological neovascularization in ROP infants can be mimicked in a mouse model with oxygen-induced retinopathy (OIR), we recapitulated post-ROP with post-OIR mice a few months after spontaneous regression of retinal neovascularization. Our pattern electroretinogr

Effectiveness of ultrasonography-guided periforaminal oxygen-ozone therapy in chronic low back pain.

BACKGROUND/AIM: Treatment of chronic low back pain (LBP) includes various minimally invasive image-guided interventional techniques. The aim of this study, which was planned based on the idea that periforaminal oxygen-ozone (O2-O3) gas application with ultrasonography (USG) could be an alternative treatment option, was to compare the therapeutic efficacy of USG-guided periforaminal ozone infiltration (PFOI) or transforaminal epidural steroid injection (TFESI) in patients with chronic radicular LBP on pain, disability, and quality of life (QoL). MATERIALS AND METHODS: A total of 50 patients we

Early Restrictive vs Liberal Oxygen for Trauma Patients: The TRAUMOX2 Randomized Clinical Trial.

IMPORTANCE: Early administration of supplemental oxygen for all severely injured trauma patients is recommended, but liberal oxygen treatment has been associated with increased risk of death and respiratory complications. OBJECTIVE: To determine whether an early 8-hour restrictive oxygen strategy compared with a liberal oxygen strategy in adult trauma patients would reduce death and/or major respiratory complications. DESIGN, SETTING, AND PARTICIPANTS: This randomized controlled trial enrolled adult trauma patients transferred directly to hospitals, triggering a full trauma team activation w

Regulation of reactive oxygen species and the role of mitochondrial apoptotic-related genes in rheumatoid arthritis.

Previous research suggests mitochondrial apoptosis alleviates rheumatoid arthritis (RA), but the role of mitochondrial apoptosis-related genes (MARGs) is unclear. Urgent exploration of RA-related mitochondrial apoptosis biomarkers is needed. Gene Expression Ontology (GEO)-derived RA datasets were used to identify differentially expressed genes (DEGs) compared to normal controls, intersected with MARGs to obtain differentially expressed mitochondrial apoptosis-related genes (DE-MARGs). Three ML algorithms screened diagnostic biomarkers. A nomogram was built and validated by receiver operating c

Reactive oxygen species production by monocytes negatively correlates with disease activity in rheumatoid arthritis.

Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by severe joint inflammation, synovial hyperplasia and degradation of the cartilage and bone in the joint. Patients with RA have an amplified T helper (Th) 1 and Th17 immune response and production of autoantibodies by autoreactive B cells. In the joint, macrophages mediate bone destruction and maintain the inflammatory process in RA. There is an increasing body of evidence indicating that NADPH oxidase (NOX2)-derived reactive oxygen species (ROS), mainly produced by macrophages and neutrophils, may have effector functions

Reactive oxygen species-associated molecular landscape uncovers diagnostic and immune features in osteoarthritis.

BACKGROUND: Osteoarthritis (OA) is a prevalent degenerative joint disease characterized by joint inflammation and degradation. Reactive oxygen species (ROS) are implicated in the pathogenesis of OA, yet the molecular pathways linking ROS-related genes to the immune microenvironment remain poorly understood. METHODS: We integrated publicly available OA gene expression datasets and performed batch correction. ROS-related pathways were curated from the MSigDB, and pathway activity was assessed using ssGSEA. We identified differentially expressed genes (DEGs) in both training and validation cohor

Effective Treatment of Nicolau Syndrome Induced by Etofenamate Injection Using Hyperbaric Oxygen Therapy.

Nicolau syndrome (NS) is a rare but serious iatrogenic complication characterized by cutaneous necrosis following intramuscular injections. Hyperbaric oxygen therapy (HBOT) has shown promise in enhancing tissue oxygenation and promoting wound healing in NS cases. The authors report the case of a 51-year-old female who developed a painful necrotic ulcer on her left gluteal region, diagnosed as NS following an intramuscular injection of etofenamate administered for knee osteoarthritis. The patient was treated with a combination of local wound care and a series of 30 HBOT sessions. This treatment

Insights From Bone and Muscle Oxygenation Toward a More Comprehensive Model of Tissue Healing.

INTRODUCTION: Predicting healing after amputation and revascularization in diabetes and PAD patients with tissue loss is challenging. Current methods, like transcutaneous oximetry, only measure skin oxygenation, overlooking deeper tissues critical for wound healing. Near-infrared spectroscopy (NIRS) during postocclusive reactive hyperemia (PORH) measures muscle oxygenation under ischemic stress but does not account for bone, which is crucial in healing of complex wounds. This study employed NIRS during PORH to evaluate both bone and muscle perfusion in a swine model of hindlimb ischemia. METH
