Importance of a fifth parathyroid gland in the surgical treatment of hyperparathyroidism.

Of 365 patients surgically treated for hyperparathyroidism at the University of Toronto hospitals, 3 had hypercalcemia due to an abnormal and ectopic fifth parathyroid gland. Autopsy studies have shown that a fifth gland may be present in 3% to 5% of patients with hyperparathyroidism. The possibility of an abnormal fifth gland as the cause of primary hyperparathyroidism should be considered when four glands of normal size and histology have been found in the neck, and such a gland should be sought in all patients with the diffuse hyperplasia of secondary hyperparathyroidism. The fifth gland is

Parathyroid autotransplantation.

Autotransplantation of the parathyroid to the forearm has been performed in eight patients following total or subtotal parathyroidectomy. The mass of gland implanted was approximately one half that used in other series. Bilateral simultaneous parathormone levels drawn at three months after autografting several higher levels in the autografted arm in every patient examined. Replacement calcium and vitamin D therapy were withdrawn from two patients within eight months after transplant, and it is anticipated that all patients will be off maintenance at 12 months. Electron and light microscopy of

Deficient erythrocyte membrane Gs alpha activity and resistance to trophic hormones of multiple endocrine organs in two cases of pseudohypoparathyroidism.

Patients with pseudohypoparathyroidism (PHPT) have been shown to have deficient cell membrane stimulatory GTP-binding regulatory protein activity (Gs activity) and other defects, which cause renal resistance to parathyroid hormone (PTH). Those patients also have a higher incidence of deficiency of other endocrine axes. To investigate the mechanisms of this syndrome on 2 patients, we measured erythrocytic ghost Gs activity by ADP-ribosylation method, which showed low and low normal levels for the 2 patients who were from the same family, suggesting type Ia of this syndrome. While basal T3, T4 a…

[Are all animal and human organs and tissues endocrine in nature and are they secreting peptide hormones into blood?].

Endocrinology was born and developed as the science of endocrine glands and the human and animal diseases induced by the gland dysfunctions. The complex of endocrine glands included pituitary, thyroid and parathyroid glands, adrenals, pancreas and gonads. Further, it has been shown that certain brain structures, hypothalamus in particular, are also endocrine organs which secrete in blood a large number of various peptide hormones. Heart secretes in blood the peptide compound natriuretic hormone which regulates the sodium balance. Recent studies have been demonstrated that the adipose tissue ce

Familial acromegaly: studies in three families.

Acromegaly is usually regarded as a disease which arises sporadically except on those rare occasions in which it occurs as one component of an inherited pluriglandular syndrome. Familial acromegaly occurring in isolation has been reported only rarely. We have studied three families in each of which two first-degree relatives have developed acromegaly without clinical evidence of other endocrinopathies, in order to clarify the relationship of familial acromegaly to multiple endocrine neoplasia, type 1 (MEN 1). No further subclinical cases of acromegaly have been detected in those first-degree r

[Hormones and the cardiovascular system].

Hormones have an influence on many tissues and organs, including the cardio-vascular system (CVS). Depending on their activity on CVS, they … calcitonin gene-related peptide (CGRP) family, angiotensin 1-7, substance P, neurokinin A, ghrelin, Parathyroid hormone-related protein (PTHrP), oxytocin, and, sex hormones. Hormones which when appearing in excess increase the heart rate are: catecholamines, endothelins, glucocorticosteroids, thyroid hormones, leptin and PTHrP. Those which decrease the heart rate inc …

The Hidden Dangers of Hypercalcemia: A Case Report of Undiagnosed Symptoms Leading to Complications.

Hypercalcemia is a relatively common metabolic abnormality; however, it is frequently undiagnosed. It often manifests with neuropsychiatric, gastrointestinal, renal, and musculoskeletal symptoms. We report the case of a 75-year-old woman with a history of syncope, anorexia, asthenia, weight loss, polydipsia, polyuria, visual hallucinations, confusion, generalized tonic-clonic seizures, and depression. Laboratory workup revealed severe hypercalcemia (ionized calcium: 1.81 mmol/L) with normal renal function and significantly elevated parathyroid hormone (PTH) levels. A diagnosis of primary hyper…

[Hypercalcemia: a practical review].

Hypercalcemia, defined as an abnormal elevation of serum calcium, is a common electrolyte anomaly in primary care, affecting almost 1% of the worldwide population. Clinical manifestations concern the neuromuscular, cardiovascular, gastrointestinal, renal and skeletal systems. Among the causes, the main ones are primary hyperparathyroidism, and malignancies. Le initial workup should include the measurement of parathyroid hormone (PTH), and the discontinuation of any medication likely to be involved in iatrogenic hypercalcemia. The chosen treatments and their speed of introduction depend mainly …

[Malignant hypercalcemia].

Malignant hypercalcemia is the main metabolic complication of cancer. Clinical presentation varies from asymptomatic to a medical emergency. The main causes include parathyroid hormone-related protein production and bone metastases, while hypervitaminosis D and hyperparathyroidism are rarer causes. Diagnosis is based on the demonstration of elevated serum calcium and low PTH in the context of known or suspected cancer. Treatment is aimed at correcting hypovolemia by hydration, reducing bone resorption, and treating the underlying cancer. The aims are to improve patients' quality of life, minim…

A patient with pseudohypoparathyroidism type 1A previously misdiagnosed as hereditary multiple exostosis: A case report.

Pseudohypoparathyroidism type 1A (PHP1A), a rare hereditary disorder, is featured by end-organ resistance to parathyroid hormone and Albright's hereditary osteodystrophy. Heterozygous mutation of guanine nucleotide-binding protein α stimulating (GNAS) gene causes the half decreased bioactivity of the Gsα protein levels. Due to the diverse early clinical manifestations of PHP1A, a diagnosis of PHP1A is often easily overlooked and misdiagnosis or missed diagnosis is common. The present study described a girl who was initially diagnosed with hereditary multiple exostoses, but was afterwards confi…

Tension-Sided Femoral Neck Stress Fracture in an Adolescent with Vitamin D Deficiency and Osteomalacia: A Case Report.

CASE: An adolescent girl presented with groin pain without any history of trauma. Imaging showed a tension-sided stress fracture of the femoral neck. Vitamin D deficiency (VDD), and raised alkaline phosphatase and parathyroid hormone levels were found. Pain relief was not achieved with nonoperative treatment. Considering the risk of fracture progression and displacement, the fracture was fixed with cannulated cancellous screws. Fracture healed without any complications. CONCLUSION: A tension-sided femoral neck stress fracture, with VDD and osteomalacia, is a rare cause of hip pain in an adole…

Seizures and Basal Ganglia Calcification Caused by Hypoparathyroidism.

Seizures, not a typical feature of hypoparathyroidism, may present along with myalgia and tetany in patients of hypoparathyroidism. Thorough history and examination, derangement of biochemical parameters such as hypocalcemia, hyperphosphatemia, and inappropriately low levels of parathyroid hormone, along with basal ganglia calcification as seen on imaging, lead to the diagnosis of hypoparathyroidism in a 10-year-old child who presented to us. Treatment with calcium and active vitamin D significantly improved his condition. In this report, we discuss the presentation and treatment of hypoparath…

Administration of Etelcalcetide for the Treatment of Secondary Hyperparathyroidism in Patients with CKD-MBD on Hemodialysis: A Nephrology Nursing Perspective.

The number, volume, and timing of oral medications prescribed to treat secondary hyperparathyroidism can add to the burden of disease management for both the patient and the nurse. Administering intravenous (IV) medication when possible has the potential of reducing the burden of medication management. Data on the use of IV calcimimetic etelcalcetide has shown improvement in blood calcium, phosphorus, and parathyroid hormone levels. IV administration of etelcalcetide at the end of each hemodialysis session may reduce the pill burden for patients and has the potential to help improve disease ma…
