A unique case of C2 osteochondroma mimicking cervical dystonia.

BACKGROUND: Osteochondromas are common benign tumors of the bone that are rarely encountered in the spine. Here, a 30-year-old female presented with the gradual 5-month and then subacute 2-week onset of progressive abnormal cervical posturing mimicking dystonia that, on magnetic resonance (MR), proved to be attributed to a C2/3 facet osteochondroma. CASE DESCRIPTION: Over 2 weeks, a 30-year-old female developed progressively severe neck pain and abnormal posturing of the neck mimicking cervical dystonia. When the MR documented a C2/3 facet joint osteochondroma, the patient was managed with a

Ross Syndrome Presenting as Heat Exhaustion: A Report of Two Cases.

Ross syndrome is a rare clinical disorder of sweating associated with tonic pupil and areflexia. There are very few case reports of Ross syndrome in dermatology literature, most presenting with patchy hyperhidrosis. Here, we report two isolated cases who had presented to the emergency department with heat exhaustion. Multidisciplinary evaluations of the first case revealed focal anhidrosis, patchy hyperhidrosis, postural hypotension, absent deep tendon reflex, and tonic pupil while the second case had similar features except for postural hypotension, prompting the diagnosis of Ross syndrome. Biopsychosocial Characteristics, Using a New Functional Measure of Balance, of an Elderly Population with CLBP.

This study examined the biopsychosocial characteristics of chronic low back pain (CLBP) in an understudied but increasingly larger part of the population: the elderly (i.e., 65 years and older). A new innovative physical functioning measure (postural control, which is a proxy for the common problem of slips and falls in the elderly) was part of this biopsychosocial evaluation. Also, the National Institutes of Health (NIH)-developed Patient-Reported Outcome Measurement Information System (PROMIS) was also part of this comprehensive evaluation. Two demographically-matched groups of elderly parti

Application of Infrared Thermal Imaging in a Violinist with Temporomandibular Disorder.

Temporomandibular disorders (TMD) consist of a group of pathologies that affect the masticatory muscles, temporomandibular joints (TMJ), and/or related structures. String instrumentalists, like many orchestra musicians, can spend hours with head postures that may influence the biomechanical behavior of the TMJ and the muscles of the craniocervicomandibular complex (CCMC). The adoption of abnormal postures acquired during performance by musicians can lead to muscular hyperactivity of the head and cervical muscles, with the possible appearance of TMD. Medical infrared thermography is a non-invas…

Update on the theory and management of orthostatic intolerance and related syndromes in adolescents and children.

Orthostasis means standing upright. One speaks of orthostatic intolerance (OI) when signs, such as hypotension, and symptoms, such as lightheadedness, occur when upright and are relieved by recumbence. The experience of transient mild OI is part of daily life. 'Initial orthostatic hypotension' on rapid standing is a normal form of OI. However, other people experience OI that seriously interferes with quality of life. These include episodic acute OI, in the form of postural vasovagal syncope, and chronic OI, in the form of postural tachycardia syndrome. Less common is neurogenic orthostatic hyp

Moving beyond the neutral spine: stabilizing the dancer with lumbar extension dysfunction.

Dancers commonly suffer from low back pain. This is often associated with the extreme lumbar extension range of motion and prolonged lordotic postures required by some dance forms. There is ample literature documenting extension-related instabilities such as spondylolisthesis in dancers. The aim of this article is to review the changes in motor control that occur in patients with subtle clinical instability and recurrent low back pain. Recent studies have consistently demonstrated changes in neuromuscular function in patients with back pain, and spinal stability retraining has become a popular…

Field evaluation of a new grocery checkstand design.

A novel express checkstand with basket cut-out was developed and evaluated in a small grocery store chain. Eight checkers performed three trials of simulated grocery checkout work with actor customers using both old and new checkstand designs. Usability was assessed by survey, structured interview and observation. Time-motion video analysis was used to evaluate differences in body postures, motions, and productivity. The new express checkstand design, which lowered and angled the presentation of the grocery basket, significantly (p<0.05) reduced right wrist flexion duration and repetition, and…

[Parkinson's disease and associated disorders].

Besides the core motor features of Parkinson's disease, other disorders such as gastro-intestinal dysfunction, postural hypotension, urinary, genital, sleep problems and pain contribute to the alteration of patient's quality of life. Drooling, swallowing difficulties and constipation are the more frequent digestive problems. Aspirations may be life-threatening. Sexual dysfunction as well as iatrogenic hypersexuality may be deleterious for the couple well-being. Symptomatic postural hypotension is the main manifestation of autonomic failure and needs a specific management. Pain is frequent in P

Planning reaching and grasping movements: simulating reduced movement capabilities in spastic hemiparesis.

In this paper we describe how a theory of posture-based motion planning recently applied to human grasping may contribute to the understanding of grasping pathology. The theory is implemented as a computer model rendered as a stick-figure animation capable of generating realistic multi-joint grasping movements. As shown here, the model can also be used to simulate grasping movements whose kinematics resemble those of grasps performed by people with spastic hemiparesis. The simulations demonstrate effects of: (a) reduced ranges of motion of arm joints on the size of the reachable workspace, (b)…

Role of vision on early motor development: lessons from the blind.

For a better understanding of the contribution vision makes to the development of other sensory systems and to movement and posture, we studied effects of early blindness by examining video recordings of 14 totally blind infants. Infants were born at term or preterm and showed no evidence of brain damage. During preterm and term periods no noticeable changes in motor activity were observed. Around 2 months postterm all infants showed clear delay in head control and abnormal, exaggerated type of 'fidgety movements'. Later, postural control was characterized by a prolonged period of ataxic featu…

[Drug-induced asterixis amplified by relative hypoglycemia].

A 58-year-old man with chronic paranoid-hallucinatoric psychosis had transient episodes with marked paranoid delusions, auditory hallucinations without confusion, shakiness of both upper extremities, tachycardia and sweating. EMG performed with surface electrodes revealed many silent periods in postural active muscles with maximum duration of 120 ms; blood glucose was 65-75 mg/dl. At other times, blood glucose was 135-140 mg/dl, EMG revealed few silent periods in postural active muscles with maximal duration of 50 ms and the patient noted some vibration in his outstretched hands only. Preliminary report: activation of the cerebellum in essential tremor.

Several images of cerebral blood flow were recorded during inhalation of carbon-15-labelled carbon dioxide by positron emission tomography in four patients with essential tremor and four normal controls. Unilateral involuntary postural tremor in essential tremor patients was associated with blood flow significantly greater than that at rest in the contralateral sensorimotor cortex, both lateral premotor regions, and both cerebellar hemispheres. Of these regions, only the cerebellum was not activated in normal controls holding a posture without tremor or in essential tremor patients undergoing …

Tremor disorders of aging: diagnosis and management.

Tremor disorders are commonly encountered in the elderly. Physiological tremor is present in all of us and may be enhanced by drugs or other circumstances to cause symptomatic dysfunction. Essential tremor consists of postural and kinetic tremors which may involve the hands, head, and voice. Approximately 50% of cases are hereditary. Significant disability may occur. Propranolol and primidone provide effective treatment for some patients. The tremor of Parkinson's disease occurs in resting and postural positions. Treatment with levodopa usually reduces the tremor. Anticholinergics may also dec

18-Hydroxycorticosterone as a marker for primary hyperaldosteronism.

An assay for the measurement of 18-hydroxycorticosterone (18-OHB) in plasma has been validated. The method involves extraction of plasma with dichloromethane, thin layer chromatography and radioimmunoassay with an iodinated 18-hydroxycorticosterone-3-carboxymethyloxime ligand. The plasma concentration of 18-OHB was measured in 16 patients with primary hyperaldosteronism and 20 control subjects. After overnight recumbency a significantly higher mean concentration of 18-OHB was found in the samples taken from the patients than in those from the control subjects. The degree of elevation did not c

Hemodynamic effects of lysine-vasopressin in orthostatic hypotension.

Previous investigations have demonstrated that the postural stimulation of arginine vasopressin (AVP) release is reduced in patients with Shy-Drager syndrome. We studied the effects of lypressin nasal spray on hemodynamic parameters in ten patients with chronic orthostatic hypotension. Heart rate, blood pressure, and stroke volume were measured at 0 degrees, 45 degrees and 70 degrees tilt before and one-half hour after two nasal sprays (four United States Pharmacopeia posterior pituitary pressor units, 7.4 micrograms) of lypressin. Lypressin did not significantly alter the effect of tilt on he

Primary aldosteronism: diagnosis, localization, and treatment.

New diagnostic techniques have enhanced the detection of primary aldosteronism. However, the response of blood pressure after operation in unilateral and bilateral adrenal disease is different. We have compared four localizing techniques--adrenal venography, adrenal isotopic scanning, a modified adrenal venous sampling for steroid measurements, and the anomalous postural decrease in plasma aldosterone concentration--in 51 patients with primary aldosteronism, all of whom had undergone operative confirmation. Adrenalectomy resulted in normal blood pressure in 59%, improvement in 25%, and no chan

Neurorehabilitation and Functional Improvement in Joubert Syndrome: A 12-Month Case Report.

BACKGROUND: Joubert syndrome (JS) is a rare ciliopathy characterized by cerebellar and brainstem malformations and the molar tooth sign on magnetic resonance imaging. Motor impairment is primarily driven by axial hypotonia, impaired postural control, and disrupted respiratory-postural integration. Longitudinal reports describing structured neurorehabilitation with standardized functional outcomes remain limited. CASE PRESENTATION: We report a female child with prenatally suspected vermian hypoplasia and postnatally MRI-confirmed Joubert syndrome. Subsequent molecular testing performed at the
