Health Care Analytics Challenges: A 3-Pillar Framework Connecting Analytics Maturity, Workforce Agility, and Technical Enablement.

Health care organizations face a "triple threat" of low analytics maturity, high workforce instability, and semantic technical barriers that together produce a crisis of "institutional amnesia." Leadership turnover, workforce shortages, and widespread intent to leave among informatics specialists systematically erase the tacit knowledge required to navigate complex clinical data schemas, trapping organizations in a cycle where knowledge loss outpaces knowledge capture. Viewed through the socialization, externalization, combination, and internalization model of knowledge creation by Nonaka, the

Factors affecting representativeness in home-based dementia activity interventions: a literature review.

Most persons living with dementia (PLwD) reside in community settings, which has driven the development of a wide range of home-based activity interventions. However, the external validity of these studies remains uncertain because the representativeness of study samples is rarely reported. Assessing representativeness is essential for estimating intervention reach and informing health equity and resource allocation decisions. This review evaluates the extent to which participants in home-based activity trials reflect the population of community-dwelling PLwD and identifies sources of selectio

A health care record review of early mobility activities after fragility hip fracture: Utilizing the French systematic method to inform future interventions.

BACKGROUND: A fragility hip fracture is a serious injury in older adults. After experiencing a fracture, a large percentage of patients do not regain their pre-fracture level of mobility. There are several international guidelines recommending early mobility after surgery. We do not know the usage of these early mobility recommendations by health care providers within our institution. An evidence-to-practice gap occurs when there is a failure to implement best practices. Utilization of a systematic method allows for a strategic approach to assessment of an evidence-to-practice gap. There were

Seeking vs Receiving Emotional Support in Cancer: Implications for Psychological Adjustment and Quality of Life.

BACKGROUND: Emotional support (ES) is central to cancer care, yet its associations with health-related quality of life (HRQoL) and psychological outcomes remain inconsistent. A key source of heterogeneity may be the frequent conflation of ES seeking (ESS) and ES receipt (ESR), which represent distinct psychosocial processes. OBJECTIVES: (1) Identify sociodemographic and clinical correlates of ESS and ESR; (2) examine associations of ESS, ESR, and their equilibrium (ES-Eq = ESR - ESS) with coping and psychological adjustment; (3) assess their relationships with HRQoL and distress; (4) compare

Using Natural Language Processing to Improve Fall Documentation in VA Nursing Home Residents.

OBJECTIVE: Falls can be deadly for older adults, making accurate documentation essential for prevention and quality improvement. The Minimum Data Set (MDS) is used in nursing homes to document falls; however, it fails to capture fall dates and has been critiqued for underreporting and misclassification. We developed a natural language processing (NLP) algorithm to extract fall dates from electronic health records (EHRs). This study outlines the NLP algorithm development and compares its findings with the MDS to evaluate misclassification and underreporting. DESIGN: Retrospective cohort study.

Psychosocial barriers to follow-up adherence after an abnormal cervical cytology test result among low-income, inner-city women.

OBJECTIVES: Low-income, inner-city women bear a disproportionate burden of cervical cancer in both incidence and mortality rates in the United States, largely because of low adherence to follow-up recommendations after an abnormal cervical cytology result in the primary care setting. The goals of the present study were to delineate the theory-based psychosocial barriers underlying these persistent low follow-up rates and their sociodemographic correlates. METHODS: Guided by a well-validated psychosocial theory of health behaviors, this cross-sectional, correlational study assessed the barrier

Three measures of physical rehabilitation effectiveness in elderly patients: a prospective, longitudinal, comparative analysis.

BACKGROUND: Rehabilitation success is measured by instruments that assess performance of activities of daily living. Guidelines on the use and choice of these instruments are lacking. The present study aimed to analyse prognostic indicators of physical rehabilitation effectiveness in elderly patients according to three rehabilitation impact indices. METHODS: Prospective, longitudinal study in a post-acute care unit. The study included rehabilitation-eligible deconditioned elderly in-patients prospectively admitted to post-acute care (n = 685, aged 83.2 ± 8.3 years, mean length of stay 15 ± 9.

The feasibility of conducting a randomized controlled trial that compares immediate versus optional delayed surgical repair for treatment of acute Anterior cruciate ligament injury-results of the IODA pilot trial.

BACKGROUND: Standard care for anterior cruciate ligament (ACL) injuries often includes surgical reconstruction of the ACL. However, two randomized controlled trials (RCT) concluded that conservative treatment does not result in inferior clinical outcomes compared to immediate ACL reconstruction. More research is needed to verify these results and to assess whether patient-specific parameters can predict whether a patient would benefit from immediate surgery or conservative treatment. However, before running such an RCT, we performed this pilot study to assess the feasibility of recruiting pati

Periodontitis Prediction Model Using Linked Electronic Health and Dental Records.

INTRODUCTION: Periodontal disease (PD) is closely linked to systemic health, with established associations with chronic conditions (eg, diabetes, cardiovascular disease). However, most predictive models rely solely on dental data, limiting the consideration of systemic factors such as medical conditions. OBJECTIVES: This study aimed to enhance PD risk prediction by using linked electronic dental records (EDRs) with electronic health records (EHRs) and machine learning (ML). METHODS: We used EDR data from 20,946 adult patients at Temple University School of Dentistry's (2022-2023) axiUm®, lin

Immediate help through group therapy for patients with somatic diseases and depressive or adjustment disorders in outpatient care: study protocol for a randomized controlled trial.

BACKGROUND: One in three people with a chronic somatic disease suffer from a comorbid mental disorder. Most common comorbidities are depressive, anxiety and adjustment disorders. These lead to an increase in morbidity and mortality, and a deterioration of quality of life and healthcare costs. Treatment of mental disorders is of great importance, but the waiting time for outpatient individual psychotherapy can be up to six months in Germany. Group therapy has comparable treatment effects and is considerably more economic than individual therapy; however, it is still almost unused in the outpati

Research on the road: Partnering with community emergency medical services to expand access to clinical trials.

Traditional site-based clinical trials impose substantial logistical burdens that limit participation among individuals with geographic, mobility, transportation, and socioeconomic constraints, thereby undermining research equity and generalizability. Although pragmatic and decentralized trial designs seek to address these barriers, many health systems lack a mobile clinical workforce capable of conducting in-person research activities in community settings. Mobile Integrated Health (MIH) and Community Paramedicine (CP) programs, which leverage community paramedics and EMTs (MIH-CPs) to provid
