The larynx serves three basic functions in humans. In order of functional priority they are protective, respiratory, and phonatory. This article discusses these three roles in terms of phylogeny, developmental anatomy, and neuromuscular reflex activity.

Baclofen toxicity is a severe condition that can suppress brainstem reflexes, leading to coma and death. Therefore, emergency evaluation, correct diagnosis, and treatment are crucial to prevent any possible neurological sequelae. Here, we present a case of a patient with a previous baclofen pump, who was brought to the Emergency Department after being found unresponsive in bed. Brainstem reflexes were absent. OBJECTIVES: Inspiratory activation of the vocal cord adductor, which causes paradoxical vocal cord motion, develops in patients with multiple system atrophy (MSA). To confirm the hypothesis that airway reflexes trigger such paradoxical activation, we investigated the effects of tracheostomy on the adductor activation in a MSA patient.
METHODS: We compared the adductor electromyograms before and after breathing was diverted to a tracheostoma under propofol anesthesia.

Electrophysiological studies of a child with presumed botulism showed that the amplitude of the serially and electrically elicited blink reflexes Rl, R2 and R2' was reduced during recovery. These findings suggest a conduction block of the facial nerves. We investigated cardiac sympathetic innervation by metaiodobenzylguanidine (MIBG) imaging in a patient with tonic pupils, loss of tendon reflexes, and segmental anhidrosis (Ross syndrome). Despite normal cardiovascular reflex tests, we observed a reduced global myocardial MIBG uptake as well as a regional uptake defect over the posterolateral cardiac territory indicating left ventricular peripheral sympathetic denervation. Pain thresholds to a pinch stimulus were measured at eight sites on the dorsum of both feet from 17 diabetic patients. Sixteen feet had neuropathic ulcers and seven had Charcot arthropathy. Vibration perception thresholds, radial nerve action potentials, muscle action potentials of flexor digitorum brevis, autonomic cardiovascular reflexes and reflex sweat output were also measured. Pinch pain threshold was abnormal in 15 diabetic feet, 11 having one or more insensitive sites. There was a greater variability in threshold between sites tested in diabetic than control subjects.

Hyperekplexia is a rare disorder characterized by an exaggerated startle response to noise and handling and by neonatal hypertonia. It is predominantly an autosomal dominant disease; however, atypical cases with additional variable manifestations have been reported.
