In-office functional endoscopic sinus surgery for chronic rhinosinusitis utilizing balloon catheter dilation technology.

PURPOSE OF REVIEW: Balloon catheter dilation (BCD) technology was introduced in 2005 as a device employed to dilate maxillary, sphenoid, and frontal sinus outflow tracts in patients with chronic sinus disease. With the evolution of the technology, BCD has been utilized in the office setting. The purpose of this review is to discuss the safety, tolerability, and technical success of this change of venue and to consider the possible significant cost savings in the current healthcare environment. RECENT FINDINGS: Recent studies have shown that BCD technology can be safely and successfully used i

Rhinosinusitis and asthma-microbiome and new perspectives.

PURPOSE OF REVIEW: Microbiome is one of the new perspectives in human health research, including airway diseases. There are several publications about the relationship of the microbiome and allergic diseases. Although pathogenesis of chronic rhinosinusitis (CRS) as well as its relationship with asthma has been widely investigated, the relationship of the microbiome and CRS is not yet well known. RECENT FINDINGS: The relationship between the hygiene hypothesis and microorganisms inside the human body and in the environment around it has been clearly shown. Furthermore, several researchers have

Medical therapy as the primary modality for the management of chronic rhinosinusitis.

Chronic rhinosinusitis (CRS) is a prevalent disease with many potential interventions including medical and surgical treatments. Because CRS is a chronic condition it is essential that therapy limits exacerbations. The purpose of this article is to show that literature supports the implementation of aggressive medical management as the mainstay of therapy for CRS. Scientific literature on the use of intranasal and systemic corticosteroids, antibiotics, nasal saline lavages, and unique therapies for individuals with CRS (both with and without nasal polyps) are reviewed. In addition, literature

Functional endoscopic sinus surgery improves sinus-related symptoms and quality of life in children with chronic rhinosinusitis: a systematic analysis and meta-analysis of published interventional studies.

AIM: To assess the current knowledge and evaluate the quality of evidence in the use of FESS for the treatment of chronic rhinosinusitis in children, regarding the respective changes in the quality-of-their-life (QoL) and the outcome that follows the operation. MATERIALS/METHODS: Systematic literature review in Medline and other database sources and meta-analysis of pooled data. RESULTS: 15 studies were systematically analyzed. Four represented Level II, five Level III, and six Level IV evidence. The total number of treated patients was 1301. Thirteen research groups reported that pediatric

[Birch pollen allergy].

In the North-East of France, birch is the main tree responsible of spring pollen allergy. However, the epidemiology of sensitization to birch pollen remains unclear. Monosensitization to birch pollen seems rare because of the frequency of cross-reactions with other pollens of the same botanical family via the major allergen Bet v 1. Around one third of patients with allergic rhinoconjunctivitis due to birch pollen are also asthmatics and a half suffer from a food allergy, essentially an oral syndrome due to rosaceae fruits eaten raw. The molecular allergens of birch pollen are well-known and h

Olfactory hallucinations as a manifestation of hidden rhinosinusitis.

The exact anatomical origin of olfactory epileptic seizures is controversial. Partial seizures can be an uncommon manifestation of brain tumours involving the medial temporal lobe and the orbitofrontal structures or as a marker of mesial temporal sclerosis. We present a patient suffering from olfactory hallucinations associated with inflammatory tissue in the anterior ethmoidal cells, in the right fronto-ethmoidal recess and in the maxillary sinus. Our report supports the opinion that the involvement of the olfactory bulb is crucial for developing so-called uncinate fits and is independent of

Medical therapy and smell dysfunction.

Olfactory dysfunction is deemed to be a significant contributor to poor quality of life in different nasal inflammatory conditions like common cold, allergic rhinitis, and acute and chronic rhinosinusitis with and without nasal polyps (NP). The mechanism underlying olfactory impairment in inflammatory sinonasal disease relates to either the obstruction of the olfactory cleft due to congestion of the nasal mucosa, the presence of secretions or polyps inside the nasal cavity, or to dysfunction of the sensory mucosa of the olfactory bulb resulting from local inflammation. The reduction of smell c…

Symptom-specific outcomes of endoscopic sinus surgery: a systematic review.

BACKGROUND: Although multiple studies have demonstrated that symptoms of chronic rhinosinusitis (CRS) improve after endoscopic sinus surgery (ESS), a systematic large-scale evaluation of specific symptom response has not been performed. OBJECTIVE: To analyze the relative effectiveness of surgery in the improvement of individual CRS symptoms. STUDY DESIGN: A literature search of MEDLINE, EMBASE, Web of Science, Cochrane databases, and other Web-based sources from January 1, 1980 through June 1, 2008 was performed. Studies of 20 or more adult patients with CRS that used symptom severity scores

[Scedosporium apiospermum rhinosinusal infection in an immunocompetent host].

Scedosporium species can cause colonization, superficial and deep localized infection or systemic disease, especially in immunocompromised hosts. We report a case of localized infection due to Scedosporium apiospermum in a 47 year old woman, with previous nasal surgery. She consulted for recurrent mucopurulent post-nasal discharge not responding to antibiotics. Computed tomography showed opacification of right maxillary sinus. Surgery was performed to removed abnormal tissue from sinus; biopsy revealed chronic sinusitis with aggregate of tightly packed hyphae suggestive of filamentous fungi. T

Pathogenesis of chronic rhinosinusitis.

Chronic rhinosinusitis (CRS) is a heterogenous disorder and represents a major public health problem. Although insights into the pathophysiology of CRS have largely expanded over the last two decades, the exact etiology and mechanism of persistence is still unrevealed. CRS is a multifactorial disease, and, with variable evidence, impaired ostial patency, mucociliary impairment, allergy, bacterial or fungal infection (or triggering), immunocompromised state, and environmental and genetic factors have been suggested to be associated or risk factors. Pathomechanisms in CRS are better understood c

[Changes in the cytological analysis of nasal secretions during a nasal secretion].

Changes of cytological parameters in the course of the nasal provocative test (NPT) we studied in 30 patients aged 14-43 years with chronic rhinitis of unknown etiology. Solutions of allergens in various concentrations were used for NPT and skin testing. NPT results were estimated clinically according to four-point scale and rhinocytogram 20 minutes after injection of different concentrations of allergens to nasal cavity. The results of the provocative tests coincide with case history data more frequently than skin test results. The discrepancy between anamnesis and results of skin tests in th…

Allergic fungal sinusitis. A report of two cases with diagnosis by intraoperative aspiration cytology.

BACKGROUND: Allergic fungal sinusitis (AFS) is a newly recognized form of sinusitis characterized by opacification of the paranasal sinuses by "allergic mucin" (AM) admixed with scattered fungal organisms. AM consists of necrotic, or partially necrotic, eosinophils and Charcot-Leyden crystals suspended in lakes of laminated, mucinous material. AFS is characterized by the absence of bone or soft tissue invasion, purulent exudate or granulomatous inflammation. Clinically, it is important to differentiate AFS from both acute invasive fungal sinusitis and noninvasive mycetoma because the three dis
