Reduction of freezing of gait in Parkinson's disease by repetitive robot-assisted treadmill training: a pilot study.

BACKGROUND: Parkinson's disease is a chronic, neurodegenerative disease characterized by gait abnormalities. Freezing of gait (FOG), an episodic inability to generate effective stepping, is reported as one of the most disabling and distressing parkinsonian symptoms. While there are no specific therapies to treat FOG, some external physical cues may alleviate these types of motor disruptions. The purpose of this study was to examine the potential effect of continuous physical cueing using robot-assisted sensorimotor gait training on reducing FOG episodes and improving gait. METHODS: Four indiv

Towards a modified consumer haptic device for robotic-assisted fine-motor repetitive motion training.

PURPOSE: To develop, test and evaluate affordable haptic technology to provide robotic-assisted repetitive motion fine-motor training. METHODS: A haptic computer/user interface was modified by adding a pantograph to hold a pen and to increase the haptic workspace. Custom software moves a pen attached to the device through prescribed three-dimensional (3D) stroke sequences to create two-dimensional glyphs. The pen's position is recorded in 3D coordinates at 1 kHz. Twenty-one healthy child volunteers were taught a standard handwriting curriculum in a group setting, two times per week for 45-60

Training of urologic oncology fellows does not adversely impact outcomes of robot-assisted laparoscopic prostatectomy.

PURPOSE: Robot-assisted laparoscopic prostatectomy (RALP) is an increasingly popular treatment choice among men with clinically localized prostate cancer and has resulted in the need to adequately train urologists to perform the procedure. We reviewed the City of Hope experience to determine if the extent of fellow involvement in the procedure has an adverse effect on surgical outcomes. PATIENTS AND METHODS: We reviewed the charts of 1833 patients who underwent RALP at the City of Hope from January 2004 to September 2007. During the academic year, each fellow has participated in 300 or more R

Totally robotic surgery for rectal cancer: from splenic flexure to pelvic floor in one setup.

BACKGROUND: Robotic surgery for rectal cancer may be a way to overcome the limitations of laparoscopic surgery. However, totally robotic surgery for rectal cancer is still technically challenging. This report describes the technical details and outcomes of totally robotic rectal surgery. METHODS: The authors developed a totally robotic surgery technique for rectal cancer and performed it for 45 patients. We designed a six-port system, including a camera port, to perform rectal cancer surgery from the splenic flexure to the pelvic diaphragm in one setup. To check the feasibility and safety of

Japan's first robot-assisted totally endoscopic mitral valve repair with a novel atrial retractor.

This case report presents the first robot-assisted totally endoscopic mitral valve plasty in Japan. A 54-year-old woman was found by echocardiography to have grade III mitral valve regurgitation because of prolapse of the posterior leaflet. Surgical repair was performed using the da Vinci Surgical System. For the totally endoscopic mitral valve repair, a right-sided approach was used through four ports. A transthoracic aortic cross-clamp and novel flexible port access retractor were inserted through a 5-mm skin incision. Quadrangular resection of the posterior leaflet was performed, and an ann

Surgical Retraction of Non-Uniform Deformable Layers of Tissue: 2D Robot Grasping and Path Planning.

Robotic surgical assistants (RSAs) have the potential to facilitate surgeries and reduce human fatigue. In this paper we focus on surgical retraction, the common surgical primitive of grasping and lifting a thin layer of tissue to expose an underlying area. Given a 2D cross-sectional model of heterogeneous tissue with embedded structures (such as veins) and a desired underlying exposure region, we present an algorithm that computes a set of stable and secure grasp-and-retract trajectories, and runs a 3D finite element (FEM) simulation to certify the quality of each trajectory. To choose secure

3D visualized robot assisted reduction of femoral shaft fractures: evaluation in exposed cadaveric bones.

The main problems in intra-medullary nailing of femoral shaft-fractures are leg-length discrepancies and rotational differences with an incidence of 2-18% and 20-40% respectively. These may lead to severe postoperative sequelae such as additional correctional operations and difficult rehabilitation. Insufficient visualization can be considered the main reason for these complications. Finally, retention of the fragments in the correct alignment before nail insertion is difficult. To overcome these problems we established a robotic telemanipulator system to support the reduction process. It was

Does a history of previous surgery or radiation to the prostate affect outcomes of robot-assisted radical prostatectomy?

OBJECTIVE: To evaluate retrospectively whether or not previous treatment to the prostate alters the perioperative outcomes from robot-assisted radical prostatectomy (RARP) after the initial 'learning curve', as there are conflicting data on outcomes of RP in patients with previous treatment to the prostate. PATIENTS AND METHODS: We retrospectively reviewed the charts of patients who had RARP between March 2005 and August 2007, and analysed demographic, perioperative variables and pathological data. In all, 510 patient charts were reviewed, identifying 24 patients with a history of previous tr

An analysis of the causes of bladder neck contracture after open and robot-assisted laparoscopic radical prostatectomy.

OBJECTIVES: To evaluate the difference in outcome of bladder neck contracture (BNC) and its causes between large groups of patients undergoing open radical prostatectomy (ORP) and robot-assisted laparoscopic prostatectomy (RALP). PATIENTS AND METHODS: We analysed 200 consecutive RPs performed by one surgeon for prostate cancer, 100 by ORP and 100 by RALP, between March 2003 and September 2007. The operative techniques of bladder neck repair and urethro-vesical anastomosis were different. The ORP patients had a conventional stomatization and 'racquet handle' repair of the bladder if necessary,

Robot-assisted vs. conventional laparoscopic rectopexy for rectal prolapse: a comparative study on costs and time.

PURPOSE: Laparoscopic rectopexy has become one of the most advocated treatments for full-thickness rectal prolapse, offering good functional results compared with open surgery and resulting in less postoperative pain and faster convalescence. However, laparoscopic rectopexy can be technically demanding. Once having mastered dexterity, with robotic assistance, laparoscopic rectopexy can be performed faster. Moreover, it shortens the learning curve in simple laparoscopic tasks. This may lead to faster and safer laparoscopic surgery. Robot-assisted rectopexy has been proven safe and feasible; how

Positive surgical margins in robotic-assisted radical prostatectomy: impact of learning curve on oncologic outcomes.

OBJECTIVE: The presence of positive surgical margins following radical prostatectomy is a known risk factor for disease recurrence and may lead to adjuvant treatment. Our goal was to assess the incidence of positive surgical margins in our series of robotic-assisted radical prostatectomy (RARP) and its relationship to our learning curve. METHODS: Between February 2003 and August 2005, 140 patients underwent RARP by the same surgical team at our institution. The records of our first 100 consecutive RARPs were retrospectively reviewed. The patients were divided into three groups based on the ti

Anatomical foundations and surgical manoeuvres for precise identification of the prostatovesical junction during robotic radical prostatectomy.

OBJECTIVE: To develop a technique to identify the bladder neck during robotic radical prostatectomy (RRP) using anatomical and patient studies, and to evaluate its efficacy during and after surgery. PATIENTS AND METHODS: The data for this study were from 10 fresh cadaveric dissections and 50 consecutive athermal RRPs performed at our institution. We used a technique we term 'the bimanual bladder neck pinch'. RESULTS: The technique helped us to identify the prostatovesical junction and to decrease the time required to dissect the bladder neck, and the time for urethrovesical anastomosis. Urin

A novel robot training system designed to supplement upper limb physiotherapy of patients with spastic hemiparesis.

Spasticity is velocity and acceleration dependent, and it is therefore important to execute physiotherapeutic exercises at a relatively low and constant velocity. This can be more accurately managed by a robot than by a person when such exercises are administered continuously for more than 15-20 min. The purpose of this project was to construct a robot-mediated system that could support upper limb physiotherapy of patients with spastic hemiparesis. The system, unlike any known robotic therapeutic system, uses unmodified industrial robots to carry out passive physiotherapy on the upper limb (in

Robotically assisted perventricular closure of perimembranous ventricular septal defects: preliminary results in Yucatan pigs.

OBJECTIVE: Robotic systems allow surgeons to perform minimally invasive cardiac surgery in adults. Experience in the pediatric population, however, is limited. Perventricular closure of muscular ventricular septal defects has been reported in humans but requires a median sternotomy. The objective of this study was to assess the feasibility of robotically assisted closure of perimembranous ventricular septal defects by using the perventricular approach. METHODS: The procedure was attempted in 7 pigs with naturally occurring perimembranous ventricular septal defects. Echocardiography was perfor

Robotic-assisted left atrial ligation for stroke reduction in chronic atrial fibrillation: a case report.

Patients with atrial fibrillation are at significant risk for sustaining a thromboembolic stroke. More than 90% of thromboemboli form in the left atrial appendage. Ligation of the left atrial appendage to reduce the risk of stroke is often performed in connection with other cardiac surgical procedures. As a stand-alone procedure, however, left atrial ligation has generally been deemed too invasive and has gained little support as an alternative therapeutic option. We report a case of port-access robotic-assisted left atrial ligation as a stand-alone procedure in a patient with chronic atrial f
