Neurogenic bladder dysfunction in lumbar intervertebral disc prolapse.

Lumbar disc prolapse with urinary dysfunction is an uncommon condition. The clinical, pathological and follow-up details of 30 patients are presented. Detrusor recovery is rare and most patients are left with an areflexic bladder. Female patients with lumbar disc prolapse and an areflexic detrusor who strain to empty their bladders are likely to develop genuine stress incontinence. This may be managed by the early institution of intermittent self-catheterisation, which may reduce the need for incontinence surgery.

Reported recreational drug and new psychoactive substance use versus laboratory detection of substances by high-resolution mass spectrometry in patients presenting to an emergency department in London with acute drug toxicity.

INTRODUCTION: Clinicians managing patients with acute recreational drug or new psychoactive substance toxicity typically depend on self-reported drug(s) used. This study compares patient self-report (and/or from other sources) to the substance(s) that were subsequently identified in serum. METHODS: A prospective sample of 1,000 adults presenting to a tertiary care, urban emergency department in London, United Kingdom, with acute recreational drug/new psychoactive substance toxicity was collected from 1 February 2019 to 2 February 2020. A total of 939 appropriate samples underwent qualitative

[The value of an aseptic intermittent catheterization program in the early management of spinal cord injury patients].

From 1979 to 1989, 269 patients with spinal cord injury were managed by an aseptic intermittent catheterization program during the acute phase of their injuries at the Spinal Injuries Center. One hundred fifty one patients with incomplete cord lesion and 36 males with complete tetraplegia were managed by program I, which protects the shocked bladder from overdistention. In contrast, 82 patients with complete cord lesion excluding male tetraplegia were managed by program II, which allows overdistention of the bladder. Of the 187 patients managed by program I, 137 (73.3%) achieved trigger voidin

A mixed method study on the impact of living with spinal muscular atrophy in Malaysia from patients' and caregivers' perspectives.

BACKGROUND: Spinal Muscular Atrophy (SMA) is a rare, recessively inherited neuromuscular disorder that causes progressive muscle weakness. There is a low degree of awareness about SMA amongst the public and healthcare providers, which may impact the perception of the disease and its proper management. To understand how this lack of awareness may have affected diagnosis, care and support for SMA patients and their caregivers, this study aims to investigate the impact of SMA on the lives and daily activities of SMA patients and their caregivers in Malaysia. METHODS: Nationwide recruitment was c

Somatosensory Evoked Potentials and Neuroprognostication After Cardiac Arrest.

Improved understanding of post-cardiac arrest syndrome and clinical practices such as targeted temperature management have led to improved mortality in this cohort. Attention has now been placed on development of tools to aid in predicting functional outcome in comatose cardiac arrest survivors. Current practice uses a multimodal approach including physical examination, neuroimaging, and electrophysiologic data, with a primary utility in predicting poor functional outcome. These modalities remain confounded by self-fulfilling prophecy and the withdrawal of life-sustaining therapies. To date, a

Ambulatory Toxicity Management (AToM) in patients receiving adjuvant or neo-adjuvant chemotherapy for early stage breast cancer - a pragmatic cluster randomized trial protocol.

BACKGROUND: Population-based studies suggest that emergency department visits and hospitalizations are common among patients receiving chemotherapy and that rates in routine practice are higher than expected from clinical trials. Chemotherapy-related toxicities are often predictable and, consequently, acute care visits may be preventable with adequate treatment planning and support between visits to the cancer centre. We will evaluate the impact of proactive telephone-based toxicity management on emergency department visits and hospitalizations in women with early stage breast cancer receiving

Efficacy of the Intervention Against the Stigmatization of Men With Eating Disorders in Primary Healthcare (iSMEsH): Results From a Randomized Waitlist-Controlled Study.

OBJECTIVE: Eating disorders (EDs) in men are underdiagnosed and undertreated, partly due to stigma hindering help-seeking. This randomized waitlist-controlled study tested the efficacy of the iSMEsH online anti-stigma intervention targeting German general practitioners (GPs) and medical students. The program aimed to reduce stigmatizing attitudes toward men with EDs and improve knowledge and self-efficacy in managing ED symptoms. METHOD: A total of 292 participants (130 GPs, 162 medical students) were randomly assigned to immediate intervention or waitlist control. The intervention consisted

Training Communication Skills and Mentalization in Group Schema Therapy for Patients with Borderline Personality Disorder.

BACKGROUND: This article introduces a structured method for communication skills training within group schema therapy (GST) specifically tailored for individuals with borderline personality disorder (BPD). Patients with BPD often face relational difficulties due to intense emotional responses and maladaptive schema modes, thus creating a need for targeted communication training to promote healthier interpersonal relationships. METHODS: This training approach was developed based on interviews with six experienced schema therapists specialising in GST for BPD. The therapists shared practical in

Road users' safety attitudes towards speeding across 39 countries.

Speeding remains a major contributor to road traffic crash risk and injury severity, yet its behavioural, attitudinal, and cultural determinants vary across populations and contexts. This study analyses speeding-related behaviours and safety attitudes using the ESRA3 survey, a harmonised international questionnaire administered across 39 countries in 2023. Self-reported speeding behaviours (both exceeding posted limits and driving too fast for conditions), acceptability beliefs, perceived social norms, risk perceptions, self-efficacy, and support for enforcement and policy measures were examin

Predictors of self-reported adherence to COVID-19 guidelines. A longitudinal observational study of 51,600 UK adults.

BACKGROUND: In the absence of a vaccine, governments have focused on social distancing, self-isolation, and increased hygiene procedures to reduce the transmission of SARS-CoV-2 (COVID-19). Compliance with these measures requires voluntary cooperation from citizens. Yet, compliance is not complete. Existing research on the predictors of compliance is almost exclusively based on cross-sectional data, raising the possibility of reverse causality and confounding. METHODS: Using data from the UCL COVID-19 Social Study, a large weekly online panel of UK adults from first three months of lockdown i
