Sanfilippo B syndrome (MPS III B): altered residual alpha-N-acetylglucosaminidase activity in an unusual sibship.

We studied the residual alpha-N-acetylglucosaminidase activity in two siblings with severe and mild Sanfilippo B syndrome. No striking differences were demonstrated between the mutant enzymes from the severe and the mild case. However we found an altered enzyme activity characterized by displacement of the pH optimum towards basic values compared to the pH optimum of the normal enzyme, higher stability to heat and to Hg2+ ion treatment. It is suggested that the Sanfilippo B disease in this sibship is due to a mutation of a structural gene coding for alpha-N-acetylglucosaminidase.

Partial deletion of the short arm of chromosome 3.

A case of deletion of the short arm of chromosome 3 (46,XY,del(3)(p253) is described. The patient is a youth of 18 years in an institution for the mentally retarded. Phenotypically, he presents congenital heart disease, hypertelorism, ptosis, epicanthus, blepharophimosis, strabismus, nystagmus, synophrys, low-set ears, frequent infections, epilepsy (abnormal EEG and grand mal seizures), "rocker bottom" feet, flat occiput and muscular hypotonia. The parents are healthy and with normal karyotypes. A silent allele in the GPT system was found in the mother, the propositus and 4 of the 5 siblings.

Carnitine palmityl transferase deficiency: clinical variability, carrier detection, and autosomal-recessive inheritance.

A 21-year-old man had recurrent myoglobinuria; his 28-year-old sister had symptoms of fatigability. During prolonged fasting, serum free fatty acid rose in both siblings, but only the sister produced ketone bodies and had elevated creatine phosphokinase activity. Carnitine palmityl transferase (CPT) activity was less than 30% of normal in muscle and platelets. Liver biopsy disclosed a low level of the enzyme in the brother. The parents had intermediate levels of the enzyme in platelets. CPT deficiency seems to have an autosomal-recessive pattern of inheritance and a variable phenotypic express…

Atrophic maculopathy associated with hereditary ataxia.

Two siblings had atrophic maculopathy associated with hereditary ataxia clinically defined as an olivopontocerebellar degeneration. The macular changes were suggestive of an early bull's-eye maculopathy in one case and of a late atrophic stage of bull's-eye maculopathy in the other. This suggests that concentric annular (bull's-eye) macular dystrophy may occur as part of olivopontocerebellomacular degeneration. Exogenous causes were excluded. The inheritance pattern was autosomal recessive or irregular autosomal dominant. The relationship of these two degenerative disorders is not yet clear. T…

Acro-osteolysis (report of two cases and brief review of literature.

Two siblings suffering from acro-osteolysis, a rare disorder, are discussed. Although three distinct types of acro-osteolysis, namely familial, idiopathic, and due to vinyl-chloride, are clearly defined, these patients did not fit into any category. They had intact nails, preserved tactile and sensory system, plantar ulceration and so sensory deficit. Nerve conduction studies showed prolongation of motor distal latencies in median, ulnar, posterior tibial and lateral popliteal nerves. Nerve biopsies from both patients showed severe loss of myelinated fibres of all sizes, but more so of small o…

Detection of the carrier state of Hurler's syndrome by assay of alpha-L-iduronidase in leukocytes.

The mean specific activity of alph-L-iduronidase in leukocytes from six obligate heterozygotes for Hurler's syndrome was found to be slightly less than one-half of the mean in normal controls; no overlap of normal and known heterozygote values was encountered. The assay has been applied with success to six potential heterozygotes, siblings of a child with Hurler's syndrome. Thus heterozygote detection of Hurler's syndrome is clearly possible; this finding, as well as the ready availability of leukocytes for screening tests, recommends their use for examination of potential carrier status in th…

Observations in children with congenital dislocation of the hip.

In 111 children who were diagnosed and treated for CDH, certain variables concerning their vital statistics, the laxity of their joints and circumstances at birth were recorded. It was observed that breech presentation, birth rank one and female preponderance are common traits in children with CHD, who also become taller and heavier than control children. Anomalies found at birth or later were more common among these children, in particular inguinal hernia. Also, joint laxity was more common in these children. An increased incidence of CDH was found among siblings.

Ultrastructure of articular cartilage of achondroplastic mice.

Articular chondrocytes of achondroplastic mice (cn/cn) resemble ultrastructurally those of their non-achondroplastic siblings (Cn/Cn or Cn/cn), except for premature deposition of glycogen and a tendency to undergo regression. The latter may be slight or extreme. The ultimate cause of the vulnerability of the chondrocytes and the cause of the heterogeneity of the reaction could not be determined with the method employed. Nevertheless, increased vulnerability accounts for cell death under conditions which are usually not injurious, and which may be either physiologic or altered by endogenous or …

A new familial intrauterine growth retardation syndrome the "3-M syndrome".

Two pairs of siblings are described with proportionate dwarfism due to skeletal hypoplasia of prenatal onset. The head size was normal for age and disproportionately large for height. The patients had a characteristic face different from that seen in the Silver-Russell syndrome. The family data are in accordance with autosomal recessive inheritance. In spite of some similarities, the bulk of clinical and genetic evidence suggests that the described intrauterine growth retardation syndrome is different from the Silver-Russell syndrome and presents an apparently "new" entity which has been desig…

[Infantile nephrosis. Microcystic renal disease (author's transl)].

Two siblings, male and female with nephrotic syndrome of neonatal onset are reported. Both parents were healthy as well as another 5 year-old brother. Both died under two months. Their clinical, biochemical and immunological data are described, and the more common causes of N.S. (i.e. syphilis, toxoplasmosis, tuberculosis) ruled out. Pathology was available only from one of the cases, and the lesions were comparable to those described by finnish authors as corresponding to "congenital" N.S. This report is an additional evidence of the presence of this entity out of Finland.

A familial syndrome of central nervous system and ocular malformations.

A family is reported in which three of seven siblings were affected with an association of severe cerebral, cerebellar and ocular malformations. Brain malformations consisted of lissencephaly and the Dandy-Walker anomaly; congenital cataracts, retinal dysgenesis and coloboma of the choroid were found in the eyes. The pathogenesis of these developmental anomalies is probably related to abnormal neuron migration and abnormal closure of fetal fissures, occurring at an early stage of embryonic development. The association of these malformations is unique, and may point to a new malformation syndro…

Familial paralysis of horizontal gaze. Associated with pendular nystagmus, progressive scoliosis, and facial contraction with myokymia.

Paralysis of horizontal gaze, pendular nystagmus, and progressive scoliosis were manifestations of an autosomal recessive genetic disease in four siblings. Bilateral facial myokymia with continuous facial contraction developed in the oldest patient. Electromyographic examination of his facial muscles after facial nerve block at the stylomastoid foramen showed absence of all muscle potentials, consistent with a supranuclear origin of the myokymia. Normality of convergence, vertical gaze, and pupillary constrictor reflex activity assured integrity of midbrain ocular motor function. Absence of ho…

Rifampin-resistant meningococcal disease.

Rifampin-resistant meningococcal disease occurred in a child who had completed rifampin chemoprophylaxis for exposure to a sibling with meningococcemia. Susceptibility testing of 331 case isolates found only 1 other case of rifampin-resistant disease in Minnesota, USA, during 11 years of statewide surveillance. Point mutations in the RNA polymerase Beta subunit (rpoB) gene were found in isolates from each rifampin-resistant case-patient.

[Donor cell-derived acute myeloid leukemia developed shortly after allogeneic bone marrow transplantation for MDS overt leukemia].

We report a patient with myelodysplastic syndrome (MDS) overt leukemia who developed an acute myeloid leukemia (AML) in donor cells shortly after bone marrow transplantation (BMT) from his HLA-matched sibling. Molecular analysis using microsatellite repeats by polymerase chain reaction proved the new leukemia to be of donor cell origin. The patient received chemotherapy with idarubicin and ara-C, but he died due to mucormycosis.

[Alteration of erythrocyte membrane proteins in a family with hereditary spherocytosis].

Erythrocyte membrane proteins in a family of nine with hereditary spherocytosis in Okinawa were analyzed by polyacrylamide gel electrophoresis in the presence of 0.1% SDS. An abnormality in the membrane protein component band 4.2 (MW 72 kDa) on the electrophoresis was observed. Deficiency in band 4.2 was found in three sibling in the family and a small but significant decrease was noted in three other members. However, change of this component was not found in the remaining members of the family or in normal subjects.

Mutant fetal hemoglobin causing cyanosis in a newborn.

A well but cyanotic newborn was found to have a mutant gamma-globin chain, leading to a functionally abnormal fetal hemoglobin. A single amino acid substitution was found in a site consistent with known adult M hemoglobins. This patient showed no clinical evidence of cyanosis at 5 weeks of age as gamma-chain synthesis was replaced by beta-chain synthesis. A sibling born 20 months later was also cyanotic and the same mutant hemoglobin was found.

[Prenatal diagnosis of Sandhoff's disease (GM2-gangliosidosis, type 2)].

The diagnosis of GM2-gangliosidosis type 2 (Sandhoff's disease) was made prenatally (23rd week of pregnancy) by amniocentsis. A sibling with "Tay-Sachs disease" had died shortly before. Severe deficiency of total beta-hexosaminidase was found in amniotic fluid and amnion-cell culture. After interruption of the pregnancy the enzyme defect was also found in the fetal brain tissue and the concentration of ganglioside GM2 was three times normal, confirming the diagnosis.

Two Novel SLC5A5 Variants (Q263L and G350D) Causing Congenital Hypothyroidism.

SLC5A5 encodes sodium-iodide symporter (NIS), which transports inorganic iodide into thyroid cells. Biallelic loss-of-function variants in SLC5A5 cause thyroid dyshormonogenesis due to iodide transport defect (ITD). We report a Japanese sibling with ITD carrying novel compound heterozygous SLC5A5 variants (p. [Gln263Leu]; [Gly350Asp]). The elder brother was diagnosed with congenital hypothyroidism (CH) through newborn screening (NBS), while the younger brother, with a negative NBS result, developed CH-related symptoms at age 3 months. We characterized the two variant NIS proteins in vitro and
