Rare presentation of necrotising otitis externa: isolated abducens nerve palsy.

An immunocompetent, non-diabetic man in his 90s presented to A&E with headache and sudden-onset left abducens nerve palsy. He was treated for recurrent left otitis externa six months previously, with a normal CT of the petrous bone at the time.Ear appearances were unremarkable, but CT petrous bones revealed extensive skull base osteomyelitis extending to the petrous apex. MRI skull base and nuclear medicine technetium-99m hexamethylpropyleneamine-oxime white cell scanning confirmed active osteomyelitis.Necrotising otitis externa (NOE) is a complication of otitis externa and is more common in t…

Phenotype response for the invasive Petaurus notatus in Tasmania.

A mainland native nocturnal arboreal glider Petaurus notatus (Krefft's Glider) was introduced to Tasmania in the 1830s as a pet and its subsequent escape and landscape spread has been associated with the predation of endemic and migratory birds. Using geometric morphometrics, we assessed whether P. notatus experienced a phenotypic response, visible in their skull, after introduction to Tasmania. We found significant skull form variation between Tasmanian (n = 57) and Victorian (n = 102) glider specimens. Specifically, there is an increase in rostrum length-particularly at the mid-cranium-a dor…

Highlighting the importance of X-ray diagnostics for targeted molecular genetic analysis in the diagnosis of rare autosomal dominant craniometaphyseal dysplasia.

Autosomal dominant craniometaphyseal dysplasia (AD-CMD) is a rare condition defined by the occurrence of progressive diffuse hyperostosis of cranial bones and abnormal metaphyseal widening of the tubular bones. ANKH is known to be the only gene associated with AD-CMD. We present a case of a toddler boy with macrodolichocephaly, asymmetry of the skull, wide bulging forehead, gingival hypertrophy and irregular teeth. Physical examination, X-ray and DNA analysis were performed. All exons and flanking intron regions of ANKH were amplified by PCR and directly sequenced using the Sanger method. X-ra…

Perinatal palliative care in an infant with exencephaly: Supporting life beyond 3 years of age.

Neural tube defects are the second most common type of fetal malformation in humans. Encephalocele, a type of neural tube defect, can be defined as a sack-like protrusion of brain tissue herniating through an opening hole in the skull. Its prognosis depends on the location, size, the amount of brain tissue involved, and the presence of other intracranial anomalies. Exencephaly, another type of neural tube defect, is characterized by an incomplete covering of the skull, resulting in an exposed fetal brain. It is described in the literature as incompatible with extrauterine life and, in most cas…

Acute soft head syndrome in a sickle cell disease patient.

Acute soft head syndrome is an extremely rare complication in children with sickle cell anemia. We present a 16-year old male patient known to have sickle cell anemia who presented to our emergency department with low grade fever, headache, skull pain and swelling. Magnetic Resonance of the brain was done demonstrating subgaleal collections overlying calvarial signal abnormalities. The combination of clinical and radiological findings were indicative of acute soft head syndrome. Acute soft head syndrome is a rare complication in children with sickle cell anemia and the pathophysiology is relat…

Spontaneous Unilateral Intrasphenoidal Meningocele.

The sphenoid sinus is an uncommon location for protrusion of a meningocele. When this does occur, it nearly always presents with leakage of … within the sphenoid sinus, but did not reveal the communication through a defect in the base of the skull. Thus, it was assumed to be strictly related to sinus disease in the past. Our case represents a phenomenon whereby meningoceles protruding through the basilar skull into the sphenoid sinus or any other location are potentially misdiagnosed due to p …

Altered bone growth dynamics prefigure craniosynostosis in a zebrafish model of Saethre-Chotzen syndrome.

Cranial sutures separate the skull bones and house stem cells for bone growth and repair. In Saethre-Chotzen syndrome, mutations in TCF12 or TWIST1 ablate a specific suture, the coronal. This suture forms at a neural-crest/mesoderm interface in mammals and a mesoderm/mesoderm interface in zebrafish. Despite this difference, we show that combinatorial loss of TCF12 and TWIST1 homologs in zebrafish also results in specific loss of the coronal suture. Sequential bone staining reveals an initial, directional acceleration of bone production in the mutant skull, with subsequent localized stalling of…

Alternative option for osteogenesis imperfecta and trigeminal neuralgia.

Osteogenesis imperfecta (OI) is a bone disorder that can lead to skull base deformities such as basilar invagination, which can cause compression of cranial nerves, including the trigeminal nerve. Trigeminal neuralgia in such cases remains a challenge, given distorted anatomy and deformities. We present an alternative option, consisting in cannulation of the foramen ovale and classical percutaneous treatment. Percutaneous balloon microcompression was performed in a 28 year-old woman with OI and severe trigeminal neuralgia using computed tomography (CT) and radiographic-guided cannulation of th…

Composition and dosage of a multipartite enhancer cluster control developmental expression of Ihh (Indian hedgehog).

Copy number variations (CNVs) often include noncoding sequences and putative enhancers, but how these rearrangements induce disease is poorly understood. Here we investigate CNVs involving the regulatory landscape of IHH (encoding Indian hedgehog), which cause multiple, highly localized phenotypes including craniosynostosis and synpolydactyly. We show through transgenic reporter and genome-editing studies in mice that Ihh is regulated by a constellation of at least nine enhancers with individual tissue specificities in the digit anlagen, growth plates, skull sutures and fingertips. Consecutive…

Congenital muscular torticollis concurrent with sagittal synostosis: a case report.

Congenital muscular torticollis (CMT) and craniosynostosis are diseases that cause plagiocephaly and craniofacial asymmetry in children. In … s confirmed by magnetic resonance imaging of the neck. Three-dimensional computed tomography of the skull, which was conducted due to the unusual appearance of the skull with a large head circumference, mild brachycephaly, as well as left plagiocephaly, revealed premature closure of the sagittal suture. Imaging and resectability issues of sinonasal tumors.

Sinonasal tumors can invade into the critical structures of the anterior and central skull base. Although the determination of precise tumor histology is difficult with imaging, radiology is important in helping differentiate malignant from benign disease. Imaging helps to map the anatomical extent of intracranial and intraorbital tumor, which has important implications for staging, treatment and prognosis. Imaging also helps to facilitate and plan for craniofacial or endoscopic surgical approaches and radiation planning. This paper will review the locoregional invasion patterns for sinonasal …

Coronal suture patency shows marked variation that is not strongly correlated with skull dysmorphology. The craniofacial effects of the FGFR2 mutations are similar, but Fgfr2(+/S252W) mutant mice display significantly more severe dysmorphology localized to the posterior palate. Our results demonstrate that coronal suture closure is neit …

Persistent CSF Rhinorrhoea, Pneumocephalus, and Recurrent Meningitis Following Misdiagnosis of Olfactory Neuroblastoma.

A 41-year-old female patient was admitted with streptococcal meningitis on a background of 5-month history of CSF rhinorrhoea. Imaging revealed an extensive skull base lesion involving the sphenoid and ethmoid sinuses, the pituitary fossa with suprasellar extension and bony destruction. Histological examination of an endonasal transethmoidal biopsy suggested a diagnosis of olfactory neuroblastoma. A profuse CSF leak occurred and the patient developed coliform meningitis. A second endonasal endoscopic biopsy was undertaken which demonstrated the tumour to be a prolactinoma. Following endonasal …
