Settings, characteristics, and experiences of stigma among people with tuberculosis in Kenya: National survey results.

Tuberculosis (TB) related stigma remains a significant barrier to TB care and treatment adherence in Kenya. Despite progress in TB control, stigma continues to affect individuals diagnosed with TB and their families, leading to delayed healthcare-seeking behaviors, social exclusion, and economic consequences. This study examines the prevalence and dimensions of TB-related stigma among people infected and affected by TB in Kenya. A cross-sectional study was conducted in November 2023. Data were collected from people with TB (PWTB) in 180 health facilities across 11 counties in Kenya. Recruitmen

Nutrition security screening in clinical care: a feasibility and implementation evaluation in family medicine and subspecialty clinics.

BACKGROUND: Nutrition insecurity (NI), or limited access to nutritious foods that support health, is increasingly recognized as relevant to health and distinct from food insecurity (FI). However, its integration into clinical assessment remains largely untested. OBJECTIVES: Our objective was to evaluate the feasibility of implementing a 2-item Nutrition Security Screener (NSS) alongside the Hunger Vital Sign, an existing FI screener, within a large integrated health system. METHODS: The NSS and FI screener were embedded into both Epic (electronic health record) and MyChart (patient portal) i

National and subnational burden of disease attributable to risk factors in Italy, 1990-2023: a systematic analysis from the Global Burden of Disease Study 2023.

BACKGROUND: The increasing prevalence of most non-communicable diseases in Italy represents a major public health challenge, largely influenced by modifiable risk factors. This study aims to analyse time trends and subnational differences in the burden of disease attributable to risk factors in Italy, from 1990 to 2023. METHODS: We used estimates from the Global Burden of Diseases, Injuries, and Risk Factors Study 2023 to assess the disease burden attributable to risk factors across five Italian macro regions between 1990 and 2023. Burden was measured using disability-adjusted life years (DAL

How do I screen for tuberculosis and provide tuberculosis preventive treatment in people with HIV?

BACKGROUND: Tuberculosis infection (TBI) remains a major cause of preventable morbidity and mortality in people with HIV (PWH). Compared with the general population, PWH have a substantially higher risk of progression from TBI to active tuberculosis (TB), especially in the setting of advanced immunosuppression, delayed antiretroviral therapy (ART), social vulnerability, and barriers to continuity of care. Early diagnosis and timely TB preventive treatment (TPT) are therefore essential components of comprehensive HIV care. OBJECTIVES: To summarize the current approach to the diagnosis and trea

"We are blind to quite a lot of things": perspectives of a communications curriculum enlisting persons with disability (PWD) as patient educators and its impact on medical professional identity formation.

BACKGROUND: Attitudinal barriers toward persons with disability (PWD) persist in healthcare systems, with lack of awareness and experience impeding optimal treatment of PWD. Medical professional identity formation, which can be described as the transformative integration of both clinical acumen and humanistic attitudes, needs to include exposure to and interactions with PWD. A communications workshop was devised for third-year medical students with PWD enlisted as patient educators. We aimed to understand students' and tutors' perspectives of the workshop, the meaning they gained from interact

The psychosocial impact of disability and stigma among persons affected by leprosy and lymphatic filariasis in India.

Neglected tropical diseases (NTDs), such as leprosy and lymphatic filariasis (LF), can cause disabilities despite available treatment. Both diseases disproportionately affect India's poorest populations, with high numbers in Jharkhand and Uttar Pradesh. Those affected experience severe psychosocial consequences, including participation restrictions, depression, and poor mental health. This study investigates the extent to which stigma affects social participation and mental wellbeing among persons with leprosy- and LF-related disabilities in two Indian states. In a cross-sectional design in Bo

Psychological distress and quality of life in women with infertility due to polycystic ovarian syndrome versus unexplained infertility: a cross-sectional study with phenotype-based analysis.

BACKGROUND: Infertility is associated with significant psychological distress and impaired quality of life (QoL) in women. Polycystic Ovarian Syndrome (PCOS), a common cause of infertility, is frequently associated with metabolic and psychological symptoms. However, limited evidence directly compares psychological symptom burden and QoL between women with PCOS-related infertility and unexplained infertility, including across PCOS phenotypes. METHODS: This cross-sectional observational study included 240 women aged 18-40 years with infertility due to PCOS or unexplained causes, conducted at a

Patient perspectives and barriers to effective home-based care in lymphatic filariasis: A mixed methods study from Puducherry, India.

Morbidity Management and Disability Prevention (MMDP) remain an under-addressed component of lymphatic filariasis elimination. Despite the simplicity of the hygiene-based regimen, adherence to these practices remains low due to multiple barriers, exacerbating chronic suffering. As the primary role remains vested in the patients, this study explores their perspectives and barriers to home-based care to inform strategies to improve outcomes. This study employed a mixed-methods approach using an explanatory sequential design. The quantitative phase involved a cross-sectional survey of 321 adult p

Deep learning for cardiovascular disease: a comprehensive review of detection and risk forecasting.

The biggest health threat to the global population is cardiovascular disease (CVD), which afflicts almost one-third of the global population and causes considerable monetary and social losses. Risk analysis should be performed in a timely and appropriate manner to enhance clinical practice and preventive interventions. The emergence of advanced data modalities, such as wearable sensors, medical imaging, electronic health records (EHRs), and genomic platforms, has led to a paradigm shift in the holistic assessment of CVD risk through multimodal data integration. This systematic review is a meth

Tourscape, cultural identity, and loyalty in a themed cultural precinct: A study of Datang Everbright City in Xi'an, China.

The archaized block, a typical form of themed cultural precinct in China, creates a consumable and experiential "sense of history" through themed physical environments and symbolic performances. This type of urban space has emerged under the country's cultural-tourism integration policy. In the field of cultural identity research on heritage sites with authentic historical remains, existing studies have confirmed the positive effects of factors such as authenticity of cultural heritage tourism, tourism experience, place attachment, and value perception on cultural identity. However, most of th

Transgender and Gender-Diverse Patient Perspectives on Hereditary Breast Cancer Risk Assessment in Gender-Affirming Top Surgeries.

PURPOSE: Transgender and gender-diverse (TGD) people have increased cancer morbidity and mortality relative to cisgender individuals. Existing breast cancer prevention guidelines fail to adequately address gender-affirming care implications, especially in the context of gender-affirming mastectomies (top surgery). Evaluation of familial and germline genetic risk before top surgery is recommended by some transgender health guidelines. This evaluation can facilitate shared surgical decision making regarding breast tissue resection extent. In this study, we use in-depth interviews with TGD people

Emergency Medicine Residency Website Wellness Pages: A Content Analysis.

INTRODUCTION: The COVID-19 pandemic impacted the way medical students seek residency positions. In 2020, the Accreditation Council for Graduate Medical Education advocated for virtual interviews. Most emergency medicine (EM) interviews in 2023 remained virtual, and this format will persist for the foreseeable future. Since students are not evaluating programs in person in most cases, residency websites are crucial for prospective residents. Resident wellness is critical for resident training and important to prospective residents; it follows that programs must be transparent about resident wel
