Secondary Brown's syndrome following fourth nerve palsy - case series and review of literature.

Introduction: Restriction of eye movements following fourth nerve palsy has been attributed to orbital injury in the past. In this report, we challenge this view. Methods: In a retrospective case series, we analyzed seven consecutive patients with a Brown's syndrome that evolved after a fourth nerve palsy. Results: All patients presented with fourth nerve palsies following trauma or meningioma surgery. After a delay of months, the fourth nerve palsy improved but the patients developed a limitation in adduction and upgaze in the same eye. None of the cases involved an injury to the orbit and th

Long-term outcomes of inferior rectus recession with conjunctival recession in congenital monocular elevation deficiency: a case series.

Introduction: Monocular elevation deficiency (MED) can be congenital and acquired. The underlying etiology usually guides the surgical management of MED. In cases with tight inferior rectus (IR) on forced ductus test, IR recession is preferred, and in cases with Superior rectus palsy, vertical transposition of horizontal recti (Knapp's Procedure) is the treatment of choice. The purpose of this case series is to report the long-term outcomes of inferior rectus recession with conjunctival recession in treating congenital monocular elevation deficiency (MED). Methods: It included 10 patients of c

Optical coherence tomography (OCT) parameters in amblyopia.

BACKGROUND: Publications on optical coherence tomography (OCT) parameters in amblyopic eyes compared to fellow eyes are discordant. Our study not only compares OCT data of fellow and amblyopic eyes, but is the first study to deliver pilot results on OCT parameter changes after long-term electronic monitoring of occlusion therapy. METHODS: Thirty eyes with strabismic, anisometropic or mixed amblyopia and the corresponding fellow eyes underwent OCT of the macula and optic disc with a Topcon 3D-OCT-2000. In a subgroup of six patients, the same examinations were additionally performed at the end

Management and outcomes of Exotropia in hyperopic patients: systematic Review.

BACKGROUND: The management of exotropia (XT) in the presence of hyperopia is challenging, as these patients are at an increased risk of developing amblyopia and compromised binocular vision. This study aims to evaluate the management approaches and clinical outcomes in XT associated with hyperopia. METHODS: A systematic review was conducted with a comprehensive search across PubMed, Web of Science, and Google Scholar, following PRISMA 2020 guidelines. The search strategy utilized a combination of free-text keywords and MeSH terms, including "Exodeviation," "Exotropia," "Intermittent Exotropia

The risk factors increasing the recurrence of intermittent exotropia following surgery.

Purpose: To evaluate the factors and preoperative clinical features associated with recurrence after surgical correction of intermittent exotropia (IXT). Methods: The medical files of patients who underwent surgery for IXT at a single center between July 2021 and June 2024 were retrospectively reviewed. Final motor success was defined as ocular alignment within 10 prism diopters (PD) of exodeviation to 5 PD of esodeviation at both distance and near. Recurrence was defined as a postoperative deviation greater than 10 PD at distance and near during follow-up. Factors evaluated included age at su

Horizontal gaze palsy with ipsilateral esotropia with substituted convergence in a child with medulloblastoma.

Introduction: Horizontal Gaze palsy with concurrent esotropia and substituted convergence is a rare clinical presentation. Methods: We report a case of a 6-year-old child being treated for pontine medulloblastoma who presented with bilateral abducens and bilateral facial nerve palsy. The patient progressed to develop right sided horizontal gaze palsy with bilateral abducens palsy and substituted convergence. In view of limited abduction in his right eye, a modified Nishida procedure was performed followed by botulinum toxin to the right medial rectus both of which failed in improving the ocula

Managing acute acquired comitant esotropia - importance of appropriate refraction and myopic prescriptions: a case report.

PURPOSE: Acute acquired comitant esotropia (AACE) is a subtype of non-accommodative esotropia for which the exact underlying mechanism remains unclear. With most AACE adults being myopic, it is crucial to detect and rectify myopic over-correction in AACE considering its possible role in inducing and/or worsening eso-deviations. This report aims to emphasize the importance of appropriate refraction and suitable myopic prescriptions in managing AACE. RESULTS: This report presents two middle-aged patients with moderate myopia who were diagnosed with AACE. At initial presentation, both were found

Marcus Gunn jaw winking associated with Duane's retraction syndrome.

Purpose: We report a case of an unusual association between Duane retraction syndrome and Marcus Gunn jaw-winking syndrome in the eye of a patient without other anomalies. Methods: Clinical case. A four-year-old boy presented mild blepharoptosis of the left upper eyelid. The infant momentarily elevated the upper eyelid to a higher level by opening the mouth and thrusting the jaw to the right side. While attempting to abduct the left eye, there was no movement with the widening of the left palpebral fissure. Still, the normal movement of the right eye was consistent with the left rectus paralysi

A study of the relative orientation of the extraocular rectus muscles: an advanced cadaveric approach.

Purpose: The anatomy of the extraocular rectus muscle insertions is clinically relevant in the field of ophthalmology. This descriptive cadaveric study determines the relative degree orientation of the superior, lateral, and inferior rectus muscles with respect to the medial rectus and investigates the distances between the rectus muscle insertions. Method: Thirty cadavers (50% female, mean age = 81.86 years, SD 12.16) were included for a total of 60 eyes. For each eye, a lateral canthotomy and cantholysis were performed followed by a peritomy. Muscle hooks were then used to access and isolate

Stability of the angle of deviation in basic intermittent exotropia (IXT) following surgical correction: a retrospective observational study from southern India.

Introduction: Intermittent exotropia (IXT) is commonly seen in areas with more sunlight, females and Asians. Surgical alignment is recommended to improve binocular function, but a high recurrence rate has been reported. This study aimed to assess the stability of ocular alignment and factors contributing to exodrift post-surgery for IXT over 6-month follow-up. Methods: We retrospectively reviewed the medical records of patients with IXT who underwent unilateral recess resect procedures and attended follow-up examinations for at least six months post-surgery. A significant exodrift was defined

The Spiral of Tillaux: dead on accurate; a cadaveric study.

INTRODUCTION: The Spiral of Tillaux describes the historically accepted insertion distances of the medial rectus, inferior rectus, lateral rectus, and superior rectus from the limbus: 5.5 mm, 6.5 mm, 6.9 mm, and 7.7 mm, respectively. The corneal diameters are historically accepted to be 11.7 mm horizontally and 10.6 mm vertically in adults. We investigated the variability of the insertion distances of the extraocular rectus muscles from the limbus as well as the corneal diameters using eyes from human cadavers and compared our measurements to these historically accepted measurements. METHODS:

Subnormal visual acuity after compliant amblyopia therapy: residual/refractory amblyopia or co-existing pathology? - a retrospective analysis.

Purpose: To assess the prevalence of alternate etiology/co-existing pathology among patients with amblyopia, and to characterize factors contributing to over-diagnosis of amblyopia. Methods: We retrospectively reviewed records of children (from 1 January 2016 to 31 December 2019) who were initially diagnosed as "amblyopia" but later an alternate diagnosis for subnormal vision was established. Patients who had a best corrected visual acuity (BCVA) of ≤20/32 (0.2 logMAR) after compliant amblyopia therapy were divided into 2 groups: those with refractory amblyopia (BCVA improvement from baseline

Approach to abnormal head posture.

An abnormal head position (AHP) refers to a condition where the head is deviated from the usual, upright posture considered normal. AHPs can manifest as the chin being raised or lowered, the head tilted to the right or left, the face turned to either side, or a combination of these movements. Patients with AHP may present to ophthalmologic clinics; however, there are several etiologies for AHP that may not be commonly recognized by ophthalmologists. Key words from this article were searched in PubMed, Scopus, and Google Scholar search engines from 1975 to December 2023. Various etiologies were

Amblyopia and physical activity in adolescents.

Introduction: Physical activity in adolescence plays an important role in reducing the lifetime risk of chronic disease. Recent studies have shown that individuals with amblyopia may have difficulties with coordinating balance and may be at greater risk of developing fine motor impairments. The purpose of this study was to determine if there is an association between the presence of amblyopia and physical activity (PA) in adolescents. Methods: Adolescent patients aged 12-18 years with a history of childhood amblyopia were age- and gender-matched to control patients with no other ocular patholo

Secondary IOL implantation for an aphakic patient with congenital cataract living in Bonin Islands; follow-up study.

Introduction: We previously reported a case of a patient with bilateral congenital cataract identified in the Ogasawara village, Bonin Islands, Japan, on a visit by an ophthalmologist and describe its course over 17 years from initial surgery. Here, we report on a secondary intraocular lens (IOL) implantation that was subsequently performed at 22 years and 5 months of age. Methods: After cataract surgery at 7 months, the aphakic patient began amblyopia treatment using extended-wear soft contact lenses (SCLs). After 10 years of age, SCLs were chosen to achieve crossed monovision considering the
