Spring-mediated cranioplasty for scaphocephaly: techniques and outcomes.

PURPOSE: Spring-mediated cranioplasty (SMC) is an established minimally invasive technique for the treatment of sagittal craniosynostosis. This study describes our surgical technique and evaluates long-term morphometric outcomes following SMC. METHODS: A retrospective review was performed of subjects who underwent SMC as primary treatment of sagittal craniosynostosis at our institution between 2011 and 2019. Demographic variables, perioperative data, spring characteristics, and cranial index (CI) were obtained. Subjects were stratified by their use of post-operative helmeting. Longitudinal CI

Editorial Commentary: Revision Lateral Ankle Ligament Reconstruction Improves Symptoms, but Outcomes Reflect the Revision Context.

Chronic lateral ankle instability commonly develops after a lateral ankle sprain injury. When nonoperative treatment fails, the modified Broström procedure remains the gold standard surgical treatment, with consistently high reported success rates. Nevertheless, a proportion of patients have recurrent instability and require revision surgery. Lateral ankle ligament reconstruction (LALR) is one of the available revision options, but evidence remains limited. Recent literature showed that the primary modified Broström procedure consistently produces superior functional and pain outcomes at midte

Selective-ramus reconstruction with costochondral graft in hemifacial microsomia.

Hemifacial microsomia (HFM) presents with complex hard and soft tissue deficiencies that challenge reconstruction, especially when mandibular segments are absent. An early childhood patient with right-sided hemi-mandibular hypoplasia (not consistent with the current Pruzansky-Kaban Classification but close to type III) and a rudimentary condyle underwent mandibular reconstruction using a costochondral graft extending from the body to the rudimentary condyle restoring the entire ramal portion. Postoperatively, the patient achieved improved facial symmetry, enhanced mandibular movements and bett

The final frontier: recent advances in distal urethral strictures reconstruction.

PURPOSE OF REVIEW: Distal urethral reconstruction represents a critical juncture in urology, necessitating a strategic balance between urethral patency and preservation of glans cosmesis. RECENT FINDINGS: Surgical paradigms have shifted toward graft tunnelization and transurethral approaches. Recent refinements encompass novel dorsal inlay and ventral onlay graft configurations within tunnelization techniques, endoscopic extensions of transurethral techniques to address longer strictures, and complete segment replacement strategies. Novel publications support the feasibility of single-stage r

Regenerative strategies for esophageal and tracheal reconstruction.

Children with congenital foregut malformations including tracheal pathology or long-gap esophageal atresia have limited treatment options. Severe malformations have suboptimal replacement options for both congenital and acquired anomalies despite maximal surgical intervention. Tissue engineering offers a potential solution to address this unmet need. For esophageal reconstruction, the combination of decellularized extracellular matrix scaffolds, myogenic progenitor cells, and bioreactor pre-conditioning has shown the most promising preclinical results. For tracheal replacement, there have been

From buccal mucosa to appendix: a bibliometric and evidence-maturity analysis of autologous tissue-based robot-assisted ureteral reconstruction.

Robot-assisted ureteral reconstruction has evolved from bowel substitution to free oral mucosal grafts and vascularized appendiceal techniques, yet the intellectual structure and clinical maturity of these autologous tissue strategies remain unclear. PubMed and the Web of Science Core Collection were searched from inception through 2 August 2026. Records were merged, deduplicated, screened, and linked to OpenAlex for bibliometric enrichment. Publication trends, institutional contributions, co-citation patterns, thematic evolution, evidence maturity, and outcome reporting were assessed. Among 2

Temporomandibular Joint Defects: Etiology and Reconstruction Strategy: A Narrative Review.

Temporomandibular joint (TMJ) defects primarily arise from congenital malformations, bony ankylosis, and radical tumor resection. As a pivotal structure for regulating maxillofacial development and maintaining stomatognathic homeostasis, the anatomical restoration and functional reconstruction of the TMJ are essential for rehabilitating the dentofacial complex. Distinct from standard mandibular reconstruction, TMJ reconstruction is inherently more complex, with treatment heavily dependent on etiology, age, defect extent, and coexisting dentofacial deformities-factors that invariably require pa

Deciding when and when not to reconstruct the orbit: How I do it.

Deciding whether to reconstruct the orbit after combined transcranial and orbital surgery is uniquely challenging. Although frequently debated, upfront reconstruction is critical for lesions that have not caused bony remodeling, orbital wall hyperostosis, or periorbital tissue stiffening. A 50-year-old man developed pulsatile enophthalmos two years after planum sphenoidale meningioma resection. We detail the staged orbital reconstruction technique and discuss indications. Lesions not involving the orbital tissues themselves but compressing the orbit require orbital reconstruction to prevent de

[Reconstruction of extensive internal canthus defects involving both eyelids: A four-step surgical approach utilizing a graft-supplying flap].

INTRODUCTION: The reconstruction of extensive medial canthal defects involving both eyelids is a rare and complex surgical challenge. The objectives are both functional (preserving mobility and corneal protection) and aesthetic (avoiding a patch-like appearance and restoring the concavity of the canthus). METHOD: We propose, through a clinical case, a four-step reconstruction procedure: 1) temporary canthopexy: fixation of the tarsal stumps to the orbital rim to protect the cornea while awaiting histopathological results; 2) buccal mucosa graft and frontal flap: a buccal mucosa graft reconstr

A prospective study of the medial cleft edge as a cause of soft palate shortening and a simple technique to restore length during palatoplasty.

INTRODUCTION AND OBJECTIVES: A thin rim of fibrotic thickening of the medial cleft edge with distinctive histopathological features is a constant finding in palatal clefts. We hypothesized that this scar-like tissue acts as a contracture band by tethering the soft palate and contributes to its anteroposterior shortening. We aimed to quantify the lengthening achieved by releasing it during palatoplasty. DESIGN: Prospective observational study. SETTING: Specialized cleft center in a tertiary care hospital in Southern India. PARTICIPANTS: Consecutive patients who underwent cleft palate repair

Robotic tracheal resection and reconstruction for tracheobronchial mucoepidermoid carcinoma.

Tracheobronchial mucoepidermoid carcinoma is a rare malignancy characterized by indolent growth and often delayed clinical presentation. While surgical resection remains the definitive treatment with excellent long-term outcomes, the surgical approach continues to evolve. While traditional techniques include thoracotomy or sternotomy, robotic-assisted surgery is emerging as a compelling alternative, offering enhanced three-dimensional visualization, superior instrument precision, and improved ergonomic control for complex airway reconstructions. This video tutorial details a robotic approach f

Novel flap designs for reconstructing the exenterated orbital cavity - A systematic review.

Orbital exenteration is an extremely mutilating procedure for patients, and reconstruction is mandatory in most cases. The inverted cone shape of the exenterated orbit poses a significant challenge for reconstructive surgeons and anaplastologists, who must collaborate closely to achieve a satisfactory outcome that enables patients to reintegrate social life more easily. This systematic review followed the 2020 PRISMA guidelines and its protocol was pre-registered in PROSPERO (ID: CRD420251149891). The study focuses on the latest flap design strategies aimed at meeting the complex geometric req

Potential techniques to facilitate dissection in patients undergoing cranioplasty following decompressive craniectomy.

Decompressive craniectomy is a common neurosurgical procedure for cases of increased intracranial pressure. In patients undergoing cranioplasty, dissection planes are usually difficult to identify due to adhesions. We highlight the possible ways to make dissection easier in cranioplasty. A systematic search was performed on PubMed, Web of Science, Scopus, and Embase from inception until January 2025 to include studies that reported the possible techniques to mitigate the risk for adhesions following decompressive craniectomy and during cranioplasty. Operative metrics including the surgical tim

Indications for the use of dermal substitutes in patients with acute burns and in reconstructive surgery after burns: A systematic review.

Deep dermal and full-thickness burns often result in scar sequelae such as contractures, hypertrophy, pain and itching following split-thickness skin grafting. Dermal substitutes are currently employed alongside split-thickness skin grafting to enhance clinical outcomes, though their indications remain a subject of ongoing debate. This systematic review aims to clarify the indications for the application of dermal substitutes in burn patients, in both acute and reconstructive settings. A comprehensive search across various databases was conducted. Studies (n = 190) assessing the indications an

Impact of cement cranioplasty on cerebrospinal fluid leaks after retrosigmoid craniotomy - A systematic review and meta-analysis.

BACKGROUND: Cerebrospinal fluid (CSF) leaks and related complications (CLRC) are common after retrosigmoid approaches for cerebellopontine angle (CPA) surgeries. Bone cement cranioplasty (CCP) may provide additional sealing benefits over reconstruction without cement (RWC) in reducing these complications. This study aimed to compare the outcomes of CCP versus RWC in CPA surgery. METHODS: A systematic review and meta-analysis following PRISMA guidelines was conducted using three databases (PubMed, Scopus, Web of Science). Studies were included comparing CCP and RWC in retrosigmoid craniotomies
