[Subglottal stenosis as the first symptom of Wegener's granulomatosis].

A 51-year-old woman presented with a sore throat, hoarseness and difficulty in swallowing. On physical examination she was found to have stridor. Laryngoscopy revealed a subglottal stenosis. Infection was thought to be the cause but this was not confirmed by sputum or laryngeal cultures. Because of the clinical course and the presence of antineutrophil cytoplasmic proteinase-3 antibodies, Wegener's granulomatosis was diagnosed. Immunosuppressive therapy led to improvement. At 4-year follow-up the patient had scleritis but no ENT problems. Wegener's granulomatosis should always be considered in

Primary cricopharyngeal achalasia in a newborn treated by balloon dilatation: a case report and review of the literature.

Primary crycopharyngeal achalasia consists of a failure of the crycopharyngeus muscle to relax at the appropriate time during the swallowing process in the abscence of other motor abnormalities. The principle treatment options for cricopharyngeal achalasia include either dilatation of the upper esophagus or surgical myotomy of the cricopharyngeus muscle. We report a primary cricopharyngeal achalasia case, who was diagnosed early in 1st week of life and treated successfully by balloon dilatation, and discuss the treatment options for this very rare disease. The presented case is the first succe

Emerging drugs for gastro-oesophageal reflux disease.

Gastro-oesophageal reflux disease is a common medical problem caused by the exposure of the distal oesophagus to gastric contents. Existing medical therapy is very effective, but symptomatic relief with acid suppressants is often delayed. Treatment focuses on the suppression of gastric acid rather than on the underlying pathophysiological abnormalities, such as transient non-swallow-related lower oesophageal sphincter relaxation. Current pharmacological developments concentrate on drugs with lasting acid suppression and a faster onset of action. Compounds interacting with the complex neuromusc

Stab to second intercostal space: a bubbling extrapleural wound.

A 37 year old man was found collapsed at the roadside and taken to the emergency department. Communication was difficult, as the patient could not speak English. There was a wound in the left second intercostal space on the midclavicular line, which was bleeding and was bubbling air. A drain was inserted, bleeding controlled, and his wounds sutured. Chest x ray later confirmed satisfactory placement of the drain. The following day, swelling and discharge indicated oesophageal damage, which was later confirmed by gastrografin swallow. With conservative management in hospital for 2 weeks, he mad

Why do we have a uvula?: literature review and a new theory.

From ancient times up to the present day many different functions and conditions have been attributed to the uvula, many speculative and some with a more scientific basis. The uvula has been shown to have the ability to produce and secrete large quantities of thin saliva. A common complication of surgery involving removal of the uvula is pharyngeal dryness. We have observed that on phonation and swallowing the uvula swings back and forth in the oropharynx. We present a review of the literature on the uvula and propose a theory that the uvula bastes the throat and thereby helps keep it moist an

Fibromatosis of the mandible in a child.

Fibromatosis represents a group of fibrous tumors showing clinic and biologic features between benign fibrous lesions and fibrosarcoma. These locally aggressive tumors have high recurrence rates (20% to 70%). A four-year-old boy presented with mandibular fibromatosis occupying the mandible completely and extending to the submandibular gland and soft tissues. Complete hemimandibulectomy and submandibular gland excision were performed followed by reconstruction with a curved Kirschner wire. No signs of recurrence was observed during a follow-up period of 18 months. In addition, no limitations in

Colonoscopic retrieval of an appendiceal foreign body: prophylaxis for appendicitis?

Management of orally ingested foreign bodies usually consists of endoscopic retrieval while the objects reside within the esophagus or stomach. Although most foreign bodies that pass through the pylorus will be excreted without incident, some may become impacted distally, resulting in obstruction or perforation. Appendiceal foreign bodies have been reported rarely, yet have resulted in the development of acute appendicitis. We report the case of a young male who swallowed a nail that became impacted in the appendiceal lumen and was retrieved colonoscopically before the development of acute app

Fish bones in the vallecula and tongue base: removal with the rigid nasal endoscope.

The tongue base and vallecula are common anatomical sites for impaction of inadvertently swallowed fish bones. Many techniques for their removal have been described but this apparently simple procedure can still pose a surgical challenge. We describe a previously unreported technique using rigid endoscopy with the 4 mm 30 degree Hopkins' rod as a visual aid for transoral removal of the impacted bone. The technique has proved to be very well tolerated by the patient and more acceptable in terms of morbidity, time and resources than either flexible endoscopy or rigid endoscopy under general anae

[Effectiveness coefficient for laryngographic closure of the glottis as a way of evaluating airway protection against aspiration of gastric contents].

Laryngographic examination was conducted on 35 men aged 45 to 65 with good hearing and proper pronunciation. Based on the examination, the proposed by the authors of this paper laryngographic closure effectiveness coefficient of the glottis (LCEC) was calculated as a way for evaluating protection of airways against the aspiration of food. Glottis occlusion serves an essential function as a mechanism protecting airways, especially in patients who have undergone horizontal supraglottic laryngectomy. The examination was conducted with the purpose of obtaining numerical values of LCEC, which will

[Attempted suicide with 400 thyroxine pills: what happened to the physician-patient relationship?].

Patient aged 49 who developed hypothyroidism after receiving 1131 for relapsing Graves' disease after treatment with propylthiouracil followed by homeopathy. Substitution with thyroxine (0.05 mg/day) was prescribed. Depressed by the perspective of a life long treatment, the patient swallowed 400 pills (20 mg). The evolution was uncomplicated after betablockers administration at hospital. One year later she became euthyroid without further medication. The occurrence of transient hypothyroidism after curitherapy is discussed. The importance of mutual participation in the patient/physician relati

Teratoma of the tongue in neonates: report of a case and review of the literature.

A female newborn presented with a huge mass protruding from the mouth. She had no respiratory distress but had difficulty swallowing. The mass originated from the tongue and was completely extirpated on the day of birth. The histologic diagnosis was mature teratoma. Three days later, another mass measuring 1 cm in diameter was found attached to the upper pharyngeal wall and was also completely extirpated 20 days after birth. The histologic diagnosis was also mature teratoma. The infant had a complete cleft palate, but no other malformation was found. Two years after surgery there is no sign of

Prevention of severe mucosecretory ear disease and its complications in patients with cleft lip and palate malformations.

Morphological and functional alterations in patients with cleft lip and palate act as predisposing factors in eustachian tube dysfunctions and as a result thereof in the occurrence of mucosecretory ear disease (MSED) and its complications. Development of MSED is the result of tube dysfunction and it consists of: (a) fluctuating conductive hearing loss, (b) swallowing alterations and (c) respiratory functional disorders. Once this syndrome is confirmed in children with clefts, action must be taken through a coordinated multidisciplinary approach with plastic surgery, odontology, and speech ther

The first case of type B infant botulism in Japan.

A six-month-old girl with a 5 consecutive day history of constipation and poor feeding developed generalized weakness, poor head control, difficulties in sucking and swallowing, and cranial nerve dysfunction within a few days. These characteristic manifestations and clinical course prompted examination of the possibility of infant botulism, although no history of eating honey was obtained. Mouse bioassay performed with enema effluent demonstrated type B botulinum toxin. Culture of the effluent was positive for Clostridium botulinum type B. This is the first case of type B infant botulism in Ja

Detecting oesophageal-related changes using electrical impedance tomography.

Preliminary work has been carried out using the Sheffield mark II real time EIT system, looking for changes in conductivity which occur in the stomach and oesophagus following a swallow of a small volume of either a conducting or a non-conducting liquid. This has been done using three different configurations: a conventional transverse array placed around the thorax, a rosette array on the abdomen, and a rosette array placed on the back. Results show a significant difference between the two liquids, which can be detected equally well by the three electrode configurations.

Endoscopic clipping of esophageal perforation after pneumatic dilation for achalasia.

A 67-year-old man with a long history of achalasia underwent pneumatic dilation of the lower esophageal sphincter due to increasing dysphagia. During the procedure, a small perforation of the thoracic part of the distal esophagus occurred. Since the rupture was small, well-confined, and detected immediately, the lesion was closed using endoscopically applied metallic clips. The patient did very well, and a contrast swallow three days later showed no leakage of the esophagus. This procedure has not yet been described for the esophagus in the literature, but it may be considered in selected case

A 42-year-old man with giant bronchogenic cyst occupied from the middle and posterior mediastinum was treated surgically. The tumor was stoutly adhered to anterior aspect of the esophagus and membranous portion of the right bronchus. In dissecting the tumor, muscular coat of the esophagus was partly severed, which was repaired interruptedly with absorbable suture material. However, esophageal perforation was detected by barium swallow on the 3rd postoperative day. Conservative therapy included intrathoracic drainage and antibiotics was successfully undertaken. It is imperative to dissect the t

Diaphragmatic herniation of the large bowel.

We present 19 patients who were found to have colonic herniation into the chest via the oesophageal hiatus on barium enema. This was demonstrated by showing the position of the hernia and its intimate association with the oesophagus at barium swallow. In nearly all cases there was a co-existent partially volved hernia of the stomach. It is concluded that in adults, colonic herniation into the chest noted at routine barium enema can occur via the oesophageal hiatus and is invariably associated with herniation of the stomach which in many cases is partially volved. The patients are usually elder

GI motility disorders: diagnostic workup and use of prokinetic therapy.

GI motility changes little--if at all--with age in healthy patients. However, a variety of diseases, including diabetes and Parkinson's disease, may cause autonomic neuropathy that is manifest as a motility disorder in the GI tract. Autonomic neuropathy can cause dysmotility in the esophagus, stomach, and gut. Symptoms are often nonspecific, including difficulty in swallowing, nausea, vomiting, heartburn, indigestion, diarrhea, and constipation. Nonpharmacologic treatment includes management of underlying diseases, avoidance of anticholinergic medications, and dietary changes. Agents with prok

[Piriform sinus fistulas as rare hypopharyngeal abnormalities, diagnosis and therapy].

We report on the history, clinical findings, diagnostic examinations and the therapy of three patients with a left piriform sinus fistula, probably a fourth pharyngeal pouch. Our patients show recurrent inflammation and abscesses of the left lateral neck. X-ray (gastrografin swallow) showed the fistula originating from the floor of the left piriform sinus. CT-scan was helpful in the previously operated area, although the fistula could be shown by ultrasonic examination. Recurrent abscesses of the lateral neck may be caused by sinus piriform fistula. It is important to search for this rare malf

Neurofibroma of the hypopharynx: a case report.

A 45-year-old female patient with a neurofibroma of the hypopharynx is reported. She had complained of swallowing discomfort for 3 months. Indirect laryngoscopy showed an oval tumor with a smooth surface in the posterior wall of the hypopharynx. Xeroradiography was effective for defining the tumor. Removal of the tumor was performed by an infrahyoid horizontal pharyngotomy under general anesthesia. The tumor had an indistinct capsule. The nerve from which the tumor originated was not clear. Histologically, the tumor was diagnosed as neurofibroma. The patient had no other neurogenic tumor and h
