Approximately 1 in ten patients with irritable bowel syndrome (IBS) believe their IBS began with an infectious illness. Prospective studies have shown that 3% to 36% of enteric infections lead to persistent new IBS symptoms; the precise incidence depends on the infecting organism. Whereas viral gastroenteritis seems to have only short-term effects, bacterial enteritis and protozoan and helminth infections are followed by prolonged postinfective IBS (PI-IBS). Risk factors for developing PI-IBS include, in order of importance, prolonged duration of initial illness, toxicity of infecting bacteria…

BACKGROUND: Gilles de la Tourette Syndrome (GTS) is a developmental neuropsychiatric disorder characterised by the presence of chronic motor and phonic tics. Drugs currently used in the treatment of GTS either lack efficacy or are associated with intolerable side effects. There is some anecdotal and experimental evidence that cannabinoids may be effective in treating tics and compulsive behaviour in patients with GTS. There are currently no systematic Cochrane reviews of treatments used in GTS. There is one other Cochrane review being undertaken at present, on the use of fluoxetine for tics in…

Lambert-Eaton myasthenic syndrome is a disorder of neuromuscular transmission in which an autoantibody is directed against the pre-synaptic calcium channel. It presents with proximal limb weakness, greater in the legs, with or without peripheral and autonomic neuropathy. Bulbar involvement is rare and fatigueability is not a feature.

Cogan's syndrome is a rare autoimmune disorder characterized by systemic vasculitis as well as eye and ear inflammation. A 33-year-old G1 P0 with Cogan's syndrome experienced an uneventful pregnancy while being treated with hydroxychloroquine and prednisone. In the absence of systemic vasculitis, Cogan's syndrome may be associated with favorable pregnancy outcome.

INTRODUCTION: Asperger's syndrome is one of the autism spectrum disorders. Affected individuals display considerably impaired capacity for social interaction, unusual special interests, and a tendency towards ritualized behavior.
METHODS: The etiology, symptoms, diagnosis, and treatment of Asperger's syndrome in adulthood are outlined on the basis of a selective literature review via Medline and information in relevant reference books. Furthermore, the authors report their personal experience at a special clinic for adults.
RESULTS: Asperger's syndrome in adulthood can be diagnosed by thorou…

The authors have analyzed the ethology, pathogenesis, treatment of Lynch Syndrome and described a case history of a 65-year-old woman with right breast cancer who was admitted into a hospital with intestines impassability and operation was performed. During the operation tumour, which caused the acute intestine impassability was identified. Apart from this, after the inner organ examination metastasis on stomach and tumour in upper and middle rectum were found. Histology and colonoscopy revealed the rectum adenocarcinoma (morphologic code 8140/3) with mild differentiation. Examination revealed…

Hepatorenal syndrome (HRS) is a type of renal failure that occurs in patients with advanced cirrhosis. It is a result of splanchnic arterial vasodilation, renal vasoconstriction, reduced effective arterial volume, and potentially reduced cardiac output. Often, HRS is a fatal complication, and the only definitive treatment currently available is liver or liver-kidney transplantation. A number of other treatment modalities have been tested for the management of HRS, but most evidence is derived from small noncontrolled studies. The primary role of these treatment options is to provide a bridge t…

The present article reviews: Corticotrophin (ACTH) independent bilateral macronodular adrenal hyperplasia, which is characterized by aberrant adrenal receptors due to either ectopic expression or to overexpression (eutopic expression). Micronodular adrenal hyperplasia, which provokes small pigmented nodules in the adrenal gland with atrophy of the internodal tissue. These nodules may not be visible on imaging tests. The term subclinical Cushing's syndrome, coined in 1981, should be used in patients with clinically non-functioning adrenal adenomas but who show autonomous cortisol production tha…

Hypoventilation in obese patients includes a diversity of mechanisms frequently imbricated, among which the two most frequent are mechanical limitation and blunted ventilatory drive. PURPOSE OF REVIEW: The present review addresses the mechanisms, genetics and pathogenesis of Usher syndrome.
RECENT FINDINGS: Recent molecular findings have provided more information regarding the pathogenesis of this disorder and the wide phenotypic variation in both audiovestibular and/or visual systems. Evidence has begun to emerge supporting a theory of a protein interactome involving the Usher proteins in both the inner ear and the retina. This interactome appears to be important for hair cell development in the ear but its role in the retina remains unclear.
SUMMARY: Understanding clin…

We recently managed a patient with the HELLP syndrome (Haemolysis, Elevated Liver enzymes and Low Platelet count) where there was a delay in diagnosis due to gastroenteritis. This case also reiterates the varied or lack of symptomatology in patients developing HELLP and obscuring the initial diagnosis. Patients with HELLP syndrome have significant maternal morbidity and mortality, hence clinical vigilance and high suspicion play a key role in the diagnosis and subsequent management.

Reversible cerebral vasoconstriction syndrome (RCVS) is more frequent than previously thought and is probably underdiagnosed. The mean age of onset is 42 years, and it affects slightly more women than men. RCVS is attributed to a transient, reversible dysregulation of cerebral vascular tone, which leads to multifocal arterial constriction and dilation. More than half the cases (60%) are secondary to exposure to vasoactive substances (e.g., cannabis, antidepressants, and nasal decongestants) or occur in the postpartum period. RCVS has a characteristic clinical and radiological course, developin…

Hypoplastic left heart syndrome is a rare congenital heart defect in which the left side of the heart is underdeveloped. Surgical management of hypoplastic left heart syndrome has changed the prognosis of the condition that was previously regarded as fatal. We discuss surgical strategies based on staged procedures, with the right ventricle supporting both systemic and pulmonary circulation. We also discuss other management options, such as neonatal transplantation and the recent innovation of hybrid techniques. Surgical techniques and the understanding of the pathophysiology of this condition …

The complex neural coordination and physiology of normal defecation has not yet been completely explained. A defecation event can be disturbed for many reasons, the most common being associated with pregnancy and childbirth, gynaecological descent or neurogenic disturbances of the brain-bowel axis. Causes of obstructed defecation are elucidated by functional imaging and functional investigations of the pelvic floor. Biofeedback treatment can be applied to paradoxal puborectal contraction (anismus), a coordination disturbance of pelvic floor muscles. A new surgical procedure has been developed

Since the first gene (MYO7A) for Usher syndrome was identified 14 years ago, there has been substantial progress in the elucidation of the genetic basis of this disorder, revealing extensive genetic heterogeneity (with nine genes known to date). Most Usher genes have similar functions, localize to similar regions in inner ear hair cells and retinal photoreceptors, and interact with each other. Approximately 80% of the patients carry mutations in one of the known Usher genes. One major challenge for the scientific community is to identify the remaining causative genes. Moreover, it is still lar…

Persistent Müllerian duct syndrome (PMDS) is a rare form of male pseudohermaphroditism characterized by the presence of the Momicronllerian duct structures in an otherwise phenotypically as well as genotypically normal male. We report a case of 40-year-old cryptorchid male who was clinically diagnosed as seminoma in the undescended abdominal testis. A diagnosis of PMDS was made on histological evaluation subsequent to abdominal orchidectomy.
