Traumatic bilateral quadriceps tendon rupture.

Bilateral spontaneous quadriceps tendon rupture is a very rare event, with only an estimated 40 cases reported in the literature. We report a case of bilateral spontaneous quadriceps tendon rupture and review the literature. The reviewed literature recommends early repair; therefore, early diagnosis is crucial. Reportedly, up to 50% of spontaneous bilateral quadriceps ruptures are misdiagnosed at first, resulting in poorer outcome. As spontaneous bilateral quadriceps rupture has previously been reported in association with chronic disease, with, to date, one-third of cases reported to show thi

Surgical treatment for recurrent dislocation of the extensor carpi ulnaris tendon.

Twelve patients with recurrent dislocation of the extensor carpi ulnaris tendon were treated with repair or reconstruction of its tendon sheath, and each had a satisfactory result. We found three types of disruption of the fibro-osseous sheath. Type A: the fibro-osseous sheath ruptured ulnarly and the torn sheath lay superficial to the tendon (n=5). These were treated by reconstruction of the sheath using a piece of the extensor retinaculum. Type B: the fibro-osseous sheath ruptured radially and the torn sheath lay in the ulnar groove beneath the tendon (n=3). These were treated by direct sutu

[Plastination histologic investigations on the inserting pars terminalis aponeurosis dorsalis of three-sectioned fingers].

With the help of thick transparent cross-sections of fingers, manufactured by plastination histology, it has been shown that fibres of the extensor aponeurosis insert not only the way described in current textbook on the basis phalangis distalis and on the capsule of the distal interphalangeal joint. Our studies prove that parts of the dorsal extension plate insert dorsally to the nail matrix and others run over the edge of the basis phalangis to the proximal part of the diaphysis where they intermingle with the periost of the phalanx.

Rupture of flexor tendons due to pisotriquetral osteoarthritis.

We are reporting a case of a flexor profundus tendon rupture of the little finger due to pisotriquetral osteoarthrosis. Resection of the pisiform bone and resurfacing of the carpal tunnel with the joint capsule was performed for the osteoarthrosis and free tendon grafting for the tendon rupture. The tendon grafting and a subsequent tenolysis restored the function of the little finger. Other than radiological changes in the pisotriquetral joint, the diagnostic value of preoperative radiocarpal arthrography was emphasized in this rare complication of pisotriquetral arthrosis.

Technical considerations in two-stage functioning free muscle transplantation reconstruction of both flexor and extensor functions of the forearm.

We report on two patients with severe injuries of the forearm who were reconstructed using functioning free muscle transplantation (FFMT) for individual replacement of flexor and extensor function. In both cases a two-stage procedure was performed: The extensor reconstruction preceded the flexor reconstruction by 4-6 months. The extensor digitorum communis and flexor digitorum profundus were successfully reconstructed in both cases using bilateral gracilis FFMT. In one case the flexor pollicis longus and extensor pollicis longus were also reconstructed using the adductor longus in addition to

Rotatory dislocation of the second carpometacarpal joint: case report.

Isolated dorsal dislocation and rotatory dislocation of the second carpometacarpal joint is rare. A case of rotatory dislocation of this joint is presented, where reduction was blocked by entrapment of the extensor carpi radialis longus tendon in the joint. A roentgenographic technique for evaluating dislocation of carpometacarpal joints is also described.

Opera-glass deformity and tendon rupture in a patient with systemic lupus erythematosus.

Systemic lupus erythematosus (SLE) is a disease entity that frequently becomes clinically evident as a generalized arthritis. Despite this common presentation, it is widely accepted that the arthritis of SLE rarely produces severe joint deformity with barely 10% of the patients developing swan neck, boutumiere deformity or a Jaccoud-like arthropathy. An even lesser proportion of patients develop joint erosions and there have been several clinical reports of tendon rupture in patients suffering from the above condition. However, there is still a great deal of controversy concerning the aetiolog

Sequential rupture of extensor tendons after a Colles fracture.

A case of isolated traumatic avulsion of the tendon of extensor carpi radialis longus (ECRL) is described. Surgical repair was attempted 12 days later, but an early contracture of the muscle was encountered. The tendon could not be pulled down to its insertion and was sutured side to side to the tendon of extensor carpi radialis brevis (ECRB). A year later the patient had residual weakness of wrist extension and hand grip. The likely cause of the early contracture of the muscle and the need for early and accurate reposition of extensor carpi radialis longus tendon are described.

Management options for flexion deformity of hand and wrist in patients with central cord syndrome.

A patient with ten year old central cord syndrome developed some active wrist and finger extension. A flexion deformity of the wrist, however, prevented use of this motor return. The surgical options considered were flexor slide versus STP procedure versus flexor tendon lengthening. Flexor tendon lengthening was done with good functional results.

Utilization of a silastic sheet in tendon repair of the foot.

Specific reparative processes often dictate the function of a tendon upon completion of healing. At times the surgeon must improvise when the results of a previous traumatic or iatrogenic insult to a tendon produced a less than desirable result. The following is a case in which the authors utilized Silastic sheeting as a pseudosheath in the repair of an iatrogenic laceration of the extensor hallucis longus tendon in a previous surgery.

The importance of soft tissues in certain skeletal traumatic lesions.

Soft tissue aspects of skeletal trauma are discussed according to two categories: (1) those injuries in which the significance of the soft tissue in the pathophysiology of the skeletal lesion is indicated by the characteristics of the skeletal injury (such as extension teardrop fracture, little leaguer's elbow, "baseball fracture," and Bennett's fracture); and (2) those injuries in which the associated soft tissue injury, or complication, may be reasonably inferred by the location and nature of the skeletal injury (such as major facial fractures, posterior sternoclavicular dislocations, fractu

Revision anterior cruciate ligament reconstruction in level 1 athletes: similar outcomes with bone-patellar tendon-bone and Hamstring autografts with or without lateral extra-articular tenodesis.

INTRODUCTION: This study reports revision anterior cruciate ligament reconstruction (R-ACLR) outcomes using bone-patellar tendon-bone (BPTB) vs. hamstring tendon (HT) autografts with and without lateral extra-articular tenodesis (LEAT) in a cohort of level 1 athletes. METHODS: A consecutive cohort of 262 athletes who underwent R-ACLR between 2014 and 2018 were analysed. The decision to add LEAT was based on risk factors for graft failure risk, particularly younger age and generalized hyperlaxity. There were 157 patients in the BPTB cohort (BPTB = 53 vs. BPTB + LEAT = 104) and 105 patients in

Midterm outcome and strength assessment after proximal rectus femoris refixation in athletes.

PURPOSE: Proximal rectus femoris avulsions (PRFA) are relatively rare injuries that occur predominantly among young soccer players. The aim of this study was to evaluate midterm postoperative results including strength potential via standardized strength measurements after proximal rectus femoris tendon refixation. It was hypothesized that the majority of competitive athletes return to competition (RTC) after refixation of the rectus femoris tendon without significant strength or functional deficits compared to the contralateral side. METHODS: Patients with an acute (< 6 weeks) PRFA who under
