Clinical manifestations and sequential computed tomography scans of cerebral cysticercosis in childhood.

Cerebral cysticercosis was studied in 131 children. The patients were divided into two groups, confirmed and probable cysticercosis. The clinical picture may be divided into two stages: initial stage of acute invasion and chronic encephalitic stage. Symptoms in the initial stage are headache, vomiting, fever, myalgia, abdominal pain, seizures and psychotic reactions. In the initial stage only cerebral edema can be seen in the computed tomography (CT) scan. In the chronic stage, variability of symptomatology is great, and it is in this phase that cysticerci can be seen in the CT scan. Only 66 o…

[Surgical treatment in acute pancreatitis. The change as influenced by sonography and computer tomography].

The diagnosis of acute pancreatitis is based on anamnestic, clinical and chemical data. Ultrasound and computed tomography permit direct visualisation of the pancreas and establish the diagnosis. In cases of haemorrhagic-necrotising pancreatitis they demonstrate the extent of morphological changes and permit exclusion of other causes of an acute abdomen. The imaging methods support indications for operation in cases of subtotal or total parenchymatous necrosis and in pancreatic abscesses. Conservative expectant approaches in patients with severe clinical course and slight morphological changes…

Computed tomography and transrectal ultrasound in the diagnosis of prostatic disease--a comparative study.

Computed tomography (CT) and transrectal ultrasound were performed in 88 patients with clinically and histologically proven prostatic disease. An ultrasound diagnosis of prostatic cancer was suggested in 49 of 54 patients with proven disease, whereas CT identified only 21 of those patients. There were no specific features of confined prostatic malignancy on CT and the most reliable feature of an unconfined tumour was infiltration of the posterior aspect of the bladder base. It was not possible on ultrasound to differentiate between prostatic inflammatory disease and a confined carcinoma but ul…

Computed tomography. The technique and its use for the evaluation of cardiocirculatory anatomy and function.

Computed tomography, a promising new imaging technique, has not yet been widely applied to cardiac disease. There is, furthermore, very little experience with CT scanning for patients with congenital cardiovascular malformations. Nevertheless, early studies with this remarkable modality indicate great promise for future application at low levels of radiation--approximately 10 to 15 per cent that of a conventional angiogram. The feasibility studies so far performed should be considered in that context. Nevertheless, relatively slow, whole body CT scanners can contribute useful clinical diagnost…

High resolution computed tomography in the diagnosis of laterally herniated lumbar discs.

Fourteen patients presenting with lumbar radiculopathy had normal or nondiagnostic plain radiographs, myelograms and polytomes of lumbar spine. The high resolution computed axial tomography (HR CAT) demonstrated laterally herniated discs in all cases. The CAT findings correlated well with the clinical presentations and were proven at surgery to be the cause of lumbar nerve entrapment. The preoperative accurate localization of the lesion resulted in complete postoperative relief of lumbar radiculopathy and back pain in 12 cases with two cases, though relieved of the radiculopathy, had residual …

Computed tomography in diagnosis of intracranial tumors versus tubers in tuberous sclerosis.

Intraventricular gliomas producing obstruction of the foramen of Monro were observed by computed tomography in 4 of 8 patients with tuberous sclerosis. Bilateral tumors arising from the caudate nuclei were present in one patient. Characteristic subependymal calcified tubers were also seen, but contrast enhancement occurred only in the tumors and not in the tubers. The location of the tumor prevented communication between the 2 lateral ventricles so that surgical placement of a single ventricular shunt resulted in only unilateral relief of hydrocephalus. Shunting in one case was ineffective bec…

Ultrasonography, 99mTc-DIDA cholescintigraphy, and infusion tomography in the diagnosis of acute cholecystitis.

Ultrasonography, 99mTc-DIDA cholescintigraphy, and infusion tomography of the gallbladder were compared in a prospective study comprising 45 consecutive patients clinically suspected of having acute cholecystitis. The diagnosis of acute cholecystitis was established or excluded by operation in 35 patients, by oral cholecystography in 5, and by intravenous cholangiography in 5. The predictive values of a positive and a negative test were 100% and 83%, respectively, by ultrasonography and 98% and 88% by cholescintigraphy. Corresponding values for infusion tomography were 87% and 33%. The results…

[131I-cholesterol scintigraphy and computerized tomography of the adrenals in the diagnosis of Conn's syndrome].

Thirty patients with severe arterial hypertension of supposedly adrenal genesis underwent computer tomography (CTG) and scintigraphy of the adrenal glands by I131-cholesterol. Fifteen patients were not found to have hypertension of adrenal genesis. According to the CTG and scintigraphy data, 10 patients were found to have adenomas in one or two adrenal glands, the diagnoses in 8 of them being verified during operations. In 3 patients with normal CTG of the adrenal glands, hyperplasia of their glomerular zone was revealed. In 2 patients with changed CTG and normal scintigraphic picture of the a…

[Importance of computerized tomography for surgical treatment of intracranial hematomas (author's transl)].

The improved treatment by using computerized tomography (CT) located between operating theater and intensive care unit is reported in comparison of 3,382 patients with severe head injuries. By preoperative use of CT the frequency of diagnosed hematomas was about the same (29.5% before CT; 27.0% with CT) but the preoperative diagnosed multiple hematomas were much more frequent (14.0% resp. 27.0%). The most important advantage however is the saving of time by CT in this localisation by which the chance of survival has increased from 54% to 69%. Without any doubt everywhere in neurotraumatology t…

Computed tomography of intracranial epidermoid tumours with special reference to atypical features.

Intracranial intradural epidermoid tumours have been known to show characteristic CT features consisting of non-enhancing lucent lesions with sharply defined margins that are often irregular and scalloped. Since the epidermoid tumours are benign, potentially curable lesions, it should be also noted that they may occasionally show atypical CT features such as dense lesion, definite marginal enhancement following contrast medium injection, or tumour associated with large, heavy calcifications. Four such atypical cases are reported, and the literature is reviewed.

The clinical usefulness of routine coronal and sagittal reconstructions in cranial computed tomography.

In order to test a hypothesis that the routine use of one midsagittal and four preselected coronal reconstruction images would disclose lesions unsuspected on standard cranial computed tomography (CT) scans, 204 patient examinations were subjected to such multiplanar reconstruction manipulation; the original CT diagnoses, reconstruction diagnoses, and final clinical diagnoses were compared. The two CT diagnoses agreed in 144 cases. Lesions well shown on the standard CT examination were not seen on the reconstruction images in 23 cases (11% of the total), and reconstruction revealed brainstem o…

The use of computerised tomography in dislocation of the hip and femoral neck anteversion in children.

Computerised tomography is useful in the diagnosis of abnormalities of the hip in children, particularly in assessing the size and shape of the acetabulum, the position and congruity of the femoral head relative to the acetabulum, and the degree of femoral anteversion or retroversion. It is most useful when limited hip movement and previous operations preclude adequate clinical examination and assessment by routine radiographic techniques. It is not recommended for routine use in screening congenital dislocation of the hip or in diagnosis or follow-up of Perthes' disease or slipped capital fem…

The role of computerized tomography scan in preoperative evaluation of the adult dislocated hip.

The case of a 42-year-old woman with coxarthroses secondary to old congenital dislocation of the hip illustrates that the value of computerized tomography scan in the preoperative evaluation of replacement of the dislocated hip is mainly in the tridimensional measurements of the acetabular and femoral bone stock.

Cranial computed tomography in the diagnosis of symptomatic indirect trauma to the carotid artery.

Five patients who were involved in motor vehicle accidents sustained indirect, nonpenetrating carotid artery injuries leading to cerebral infarction. Four patients were conscious when a severe lateralized neurological deficit developed three to 12 hours after injury indicating the presence of a vascular injury and not a mass lesion. In 3 patients with occlusion of the main stem middle cerebral artery, computed tomographic (CT) scans performed between 1 and 7 hours after the onset of the ischemic signs were normal, but all scans performed after 11 or more hours revealed progressive edema in a v
